When a Heart Never Stops: A 5-Bypass Life-Saving Surgery at 78
When advanced age meets severe, multi-vessel heart disease, the margins for error disappear. At Tazma, we combine surgical innovation with relentless dedication to turn high-risk challenges into second chances.
For the past seven years, our 78-year-old patient had been managing ischemic heart disease and long-standing hypertension. Despite taking eight different daily medications to protect his heart, his condition deteriorated sharply over the last three months. Everyday activities left him exhausted, breathless, and gripped by worsening chest pain.
A comprehensive evaluation by our cardiac team revealed the root of his struggle: Severe Triple-Vessel Coronary Artery Disease (TVD)—critical blockages across all three major arterial territories supplying his heart—alongside a mildly dilated left ventricle and left atrium.
To restore full blood flow, he needed five separate bypass grafts. However, in a 78-year-old patient with enlarged heart chambers, stopping the heart completely for a prolonged five-graft surgery carries significant risks, while operating without support can cause dangerous drops in blood pressure when lifting the heart to reach the back walls.
Once our Cardiac Team determined the safest path forward, we prepared the patient for a highly specialized hybrid approach. Under the leadership of our Adult and Pediatric cardiothoracic surgeon, Dr. Sisay Bekele, assisted by Dr. Mohammed, the team executed a complex 5-Graft On-Pump Beating-Heart CABG:
Beating-Heart Preservation (0-Minute Cross-Clamp): Using specialized heart-lung machine support (managed by clinical perfusionist Tigist) alongside suction tissue stabilizers, the team unloaded the strain on his heart while keeping it beating naturally throughout the entire procedure—avoiding the trauma of stopping and restarting an elderly heart.
Sequential Arterial Grafting (LIMA to Ramus & LAD): Using a single internal mammary artery to restore blood flow to two major vessels on the front of the heart.
Triple Venous Bypass (Sequential OM_1, OM_2 ) & Distal RCA: Utilizing saphenous vein grafts to sequentially bypass two blocked branches on the lateral wall and the distal right coronary artery at the base of the heart.
Executing five delicate coronary bypasses on a beating heart requires more than elite surgical skill—it demands seamless coordination across cardiac anesthesiology (Dr. Getu and Le'elina), perfusion, and our dedicated operating room nursing team (Sr. Betty, Sr. Nitsuh, and Sr. Meti).
We are proud to share that our patient came through the procedure smoothly and was transferred to our ICU in stable condition. His journey stands as a testament to what is possible when advanced surgical technique meets a world-class multidisciplinary team: age and complexity do not have to be the final chapter.

| 2 |  +1A NEW VISION FOR TIGRAY'S HEALTH SECTOR: MY NOMINATION FOR LEADERSHIP
Tigray's health system stands at a historic crossroads. Years of devastating conflict have left our healthcare institutions, health infrastructure, human resources and public health services facing enormous challenges.
Rebuilding this system requires more than administrative appointments. It requires visionary leadership, professional competence, integrity and an unwavering commitment to public service.
For this reason, I strongly propose Dr. Yemane Gebremariam for the position of Head of the Tigray Regional Health Bureau.
I believe Tigray needs leadership that understands the realities of healthcare delivery, appreciates the importance of evidence-based decision-making, values health professionals and can mobilize institutions toward a common vision.
Dr. Yemane's medical background, academic experience and institutional leadership provide a meaningful foundation for this responsibility.
However, this nomination is about more than an individual. It represents the kind of leadership I believe Tigray needs: leadership built on merit rather than political loyalty, competence rather than connections, accountability rather than personal interests, and service rather than privilege.
Our priorities must be clear:
- Rebuilding a resilient and accessible healthcare system.
- Restoring the dignity and professional rights of healthcare workers.
- Strengthening primary healthcare and referral systems.
- Reducing preventable maternal and child mortality.
- Ensuring equitable healthcare access for every community.
- Re-establishing institutional accountability and public confidence.
- Building meaningful partnerships for sustainable health-sector recovery.
Tigray does not simply need to restore what was destroyed. It needs to build a stronger, more equitable and more resilient health system than the one it had before.
I believe Dr. Yemane deserves serious consideration for this responsibility.
Let competence lead. Let integrity guide. Let public service come first.
The health of a nation is too important to be compromised by anything less than exceptional leadership.

የማህጸን ጫፍ ካንሰር መከላከያ ክትባት አታስከትቡ ለሚሉት ጥቂት ጥያቄዌች።
በእርግጥ ማንም ሰው አዲስ የሆነ ነገር ራሱም ላይ ሆነ ልጁ ላይ ሲደረግለት ቢጠራጠር ወይም ቢጠይቅና ጥርጣሬ ቢያሳይም ጤናማ ነገርና የሚጠበቅ ነው። ነገር ግን እርግጠኛ ሆናችሁ "አይ መሆን የለበትም አታስከትቡ" ለሚል ሰው ግን አሳማኝ ምክንያት ይኖረዋል ብዬ አስባለሁ።
እስኪ እናንተም አሳማኝና ተጨባጭ መረጃ አቅርቡልንና እኛም እንመናችሁ አታስከትቡ ካላችሁ። የምር አሳማኝ ምክንያት ወይም በቂ መረጃ ኖሯችሁ ነው ወይስ መሬት ላይ በሌለ አሉባልታና ጥርጣሬ ብቻ ነው በድፍረት አታስከትቡ ማለታችሁ?
ወይም ደግሞ እስኪ እነዚህን ጥያቄዎች ታማሚዎች ቦታ ሁናችሁ መልሱልኝ
1) በያመቱ ኢትዬጵያ ውስጥ ብቻ ወደ 6ሺ እናቶች በማህፀን ካንሰር ሲያዙና ሲሞቱ ከክትባቱ ሌላ የተሻለና የተረጋገጠ መፍትሄ አላችሁ? ወይስ ያሁኑ ህፃናትም ተመሳሳይ እጣ ቢገጥማቸውም አያሳስብም ነው የምትሉት።
የተሻለ አማራጭ ካላችሁ ደስ ይለናል ንገሩንና ሌሎቻችንም እንጠቀመው። ነው ወይስ የግድ በያንዳንዳችን ቤት ህመሙ ካልገባ አናምንም?
2) ደማቸው እየወረደ በህክምና ለማቆም እንኳን አስቸግሮ በትላልቅ ሆስፒታሎች የሚሰቃዩትን እናቶች አይታችሁ ነው ይህን ማለታችሁ?
3) በበሽታው ምክንያት የሽንት ቧንቧቸው ተዘግቶ በሆዳቸው በኩል በቱቦ እንዲሸኑ የተደረጉትን እናቶች ጠይቃችኋቸው ነው ስለክትባቱ ይህን ማለታችሁ?
4) ካንሰሩ ኢንፌክሽንና ለመታከም አስቸጋሪ በሆነ ፊስቱላ እየተሰቃዩ ሞታቸውን ቢያቀርበው የሚመኙ እናቶቻችን ስቃይ እያወቃችሁ ነው አታስከትቡ ማለታችሁ?
5) ካንሰሩ ኦፕሬሽን መሰራት በማይቻልበት ደረጃ ላይ ሆኖ ለጨረር ህክምና ብዙ ወራትን አንዳንዴም አመታትን በትንሽ ተስፋ ወረፋ ሲጠባበቁና ወረፋ ደርሷቸው ሲደወልላችው "አይ... እሷኮ ካረፈች ቆየች😥" ካሉ ቤተሰቦች ጫማ ራሳችሁን አድርጋችሁ ነው አታስከትቡ ማለታችሁ ?
በጣም ብዙ ሀገራት ላይ በሚሊዮኖች ላይ ተሞክሮ አዋጭነቱ የታወቀን ክትባት ለታዳጊዎች መከልከል ከሆነ ጊዜ በኋላ (በህይወት ከቆየን) ጸጸት እንደማያመጣብን እርግጠኞች ነን?
6) ፈጣሪ ከኛ ይልቅ ሌሎች ያደጉ የሚባሉ ሀገሮችን ስለሚወድ ነው ይሄ ሁሉ የማህጸን በር ካንሰር ህመምና ሞት "ካደጉት ሀገሮች" ይልቅ በእኛ ሃገር የበዛው ወይስ ክትባቱና የቅድመ ካንሰር ምርመራው በበቂ ሁኔታ ባለመደረጉ?

ዛሬ ስለ ሂውማን ፓፒሎማ ቫይረስ እንማማራለን።
አንድ ቫይረስ፤ አንድ ካንሰር። ነገር ግን ብዙ የተቀጠፉ የሴት ልጅ ህይወቶች!
ወጣት እና ልጅ አገረድ ልጆች ሊያውቋቸው የሚገባ እውነቶች፦
I) አብዛኛው የማህፀን ጫፍ ካንሰር የሚከሰተው በሂውማን ፓፒሎማ ቫይረስ መሆኑን።
II) ሂውማን ፓፒሎማ ቫይረስ በፆታዊ ግንኙነት/ወሲብ ወቅት መተላለፉን።
III) የፍቅር ወይም የወሲብ አጋርሽ በበዛ ቁጥር በማህፀን ጫፍ ካንሰር የመያዝ ዕድልሽም ይጨምራል።
ማህበረሰባችን ውስጥ የሚታየው የግንዛቤ አናሳ መሆን፤ ለቫይረሱ የሚደረገውን ጥንቃቄ አስተጓግሎት ይገኛል። ይህ ቫይረስ በጣም በሰፊው የሚገኝ እና ብዙ ወጣት ወንዶች እና ሴቶችን የሚያጠቃም ሆኖ እናገኘዋለን። በዚህ ቫይረስ የተጠቃች እንስት ሰውነቷ በአግባቡ ካልተቆጣጠረው እና ካላጠፋው ወደ ካንሰር የማደግ ዕድሉ ይበልጡን ያይላል።
በሀገራችን ኢትዮጵያ ችግሩን አሳሳቢ ከሚያደርጉት ነገሮች መኃከል፤ አብዛኛው ሴቶች ወደ ህክምና ተቋም ሲመጡ በመጨረሻው የበሽታ ደረጃዎች ላይ መገኘታቸው ነው። ይህም የህክምናውን ሂደት የሚገታ እና የመዳንን ዕድልንም በጣም የሚቀንስ ሆኖ እናገኘዋለን።
ታድያ እዚህ አስከፊ የበሸታ ደረጃ ላይ ላለመገኘት የሚረዱ ምርጦቹን ዘዴዎች እነሆ።
ሀ) የሂውማን ፓፒሎማ ቫይረስ ክትባትን መውሰድ፤ በተለይ ልጅ አገረድ እንስቶች ከ 9 አመት ጀምሮ ቢወስዱት ይመከራል።
ለ) በተወሰኑ ግዜያት ልዩነት የማህፀን ጫፍ ካንሰር ቅድመ ምርመራ ማድረግ
ሐ) አንድ ለአንድ መወሰን ወይም መታቀብ
መ) በወጣት ሴቶች መኃል የእርስ በእርስ ንግግሮች እና ምክክሮችን መከወን። ይሄ ሴት ልጅን ለተሻለ ግንዛቤ የማብቃት ዕድል አለው!!!
ከምንም በላይ እርስ በራስ ከመወነጃጀል ወይም ከመተማማት፤ ቀላል ውይይቶች ማድረግ ያለንን የዕውቀት ምህዳር ያሰፋል።
ዛሬውኑ እራስሽን እወቂ፤ በካንሰር የህክምና ማዕከሎችም በመገኘት በቂ ምርመራዎችንም አድርጊ።
ስለ ቅድመ ምርመራ፣ ስለ ፆታዊ ግንኙነት ያለመተፋፈር መወያየት ስልጡንነት ነው!!! እስኪ ለዛሬ እቺን ተወያዩባት?

ይህ አንድ አስፈላጊ የሕክምና ጥያቄ ያስነሳል። አንድ ድንገተኛ የደረት ምት በጤናማ ወጣት ሰው ላይ እንዴት የልብ ምት መቆምን እና ሞትን ሊያስከትል ይችላል?
በሕክምና ይህንን ሊያብራሩ የሚችሉ በርካታ ምክንያቶች አሉ።
1. COMMOTIO CORDIS – የደረት ምት የልብን ኤሌክትሪካዊ ሥርዓት ሲያውክ
Commotio Cordis የሚባለው ከባድ የልብ ጉዳት ሳይኖር እንኳን ድንገተኛ የደረት ምት የልብን መደበኛ የኤሌክትሪክ ሥርዓት በማወክ ለሞት የሚያደርስ ሁኔታ ነው።
እንዴት ይከሰታል?
ምቱ በደረት መሃል ልብ በሚገኝበት አካባቢ ይደርሳል ፣ ይህ ምት ከልብ የኤሌክትሪክ ሥራ ዑደት በጣም አጭር እና ልዩ በሆነ የጊዜ ክፍል ላይ ሲደርስ አደገኛ ይሆናል።
በተለይም በECG ላይ የT wave መውጣት በሚጀምርበት የጊዜ ክፍል ላይ የሚደርስ ምት የልብ ኤሌክትሪካዊ ሥርዓትን ሊያውክ ይችላል።
በዚህም ምክንያት Ventricular Fibrillation (VF) የሚባል ከባድ የልብ የኤሌክትሪክ መዛባት ሊከሰት ይችላል።
ልብ በተደራጀ ሁኔታ ደም መርጨትያቆማል።
በጥቂት ሰከንዶች ውስጥ ሰውየው ራሱን ሊስት እና የልብ ምቱ ሊቆም ይችላል።
አስገራሚው ነገር፦ ይህ ሁኔታ የልብ ደም ሥር መዘጋት (Heart Attack) ሳይኖር ፣ የልብ ጡንቻ ከባድ መዋቅራዊ ጉዳት ሳይኖር ሊከሰት ይችላል።
2. MYOCARDIAL CONTUSION – የልብ ጡንቻ በቀጥታ መጎዳት
ከባድ የደረት ምት ሲደርስ ልብ በደረት አጥንት (Sternum) እና በጀርባ አጥንት (Spine) መካከል ሊጨመቅ ይችላል። ይህም፦

3. TRAUMATIC CORONARY ARTERY INJURY – የልብ ደም ሥር በአደጋ መጎዳት
የደረት ግጭት የልብ ደም ሥሮችንም ሊጎዳ ይችላል። ምክንያቱም ደም ቧንቧዎች ከጡንቻው በላይ ነው ያሉት
የደም ሥር ውስጣዊ ክፍል መቀደድ (Coronary artery dissection)
የደም መርጋት።
የደም ሥር ሙሉ በሙሉ መዘጋት።
- መጨረሻም ድንገተኛ የልብ ስስትሮክ ( ጥቃት) (Myocardial Infarction) ያስከትላል
4. MYOCARDIAL FREE WALL RUPTURE AND TAMPONADE – የልብ ጡንቻ መቀደድና የልብ መጨመቅ
ከባድ ጉዳት የልብን ግድግዳ ሊቀድድ ይችላል። ደም በልብ ዙሪያ ባለው ሽፋን ውስጥ ሲከማች ልብ በተለመደው ሁኔታ መሙላትና ደም መርጨት አይችልም።
የልብ ምት ከተመለሰ በኋላስ ለምን ሞት ይከሰታል?
የልብ ምት ቆሞ ከነበረ በኋላ በ CPR የደም ዝውውር ቢመለስም፣ አንጎል በቂ ኦክስጅን ባለማግኘቱ ከባድ ጉዳት ሊደርስበት ይችላል። ይህ Hypoxic-Ischemic Brain Injury ይባላል። ስለዚህ የልብ ምት መመለስ ማለት የአንጎል ሥራ ሙሉ በሙሉ ተመልሷል ማለት አይደለም።
አንድ ተጫዋች በደረት ግጭት ምክንያት ቢወድቅ ምን ማድረግ አለብን?
በእንዲህ ዓይነት ሁኔታ እያንዳንዱ ሰከንድ ወሳኝ ነው።
1. የተጎዳው ሰው ምላሽ እና መተንፈስ መኖሩን ወዲያውኑ መመርመር
2. የድንገተኛ እርዳታ ቡድን መጥራት
3. የልብ ምት መቆሙ ከተጠረጠረ ወዲያውኑ CPR መጀመር
4. AED (Automated External Defibrillator) መጠቀም
5. VF ወይም Pulseless VT ከተገኘ በፍጥነት የኤሌክትሪክ ድንጋጤ (ሾክ) (Defibrillation) መስጠት
6. ወደ ተገቢው ሆስፒታል በአስቸኳይ ማድረስ።
ለስፖርት ተቋማት እንዲሁም ሰፈር ውስጥ ለሚካሄዱ ስፓርታዊ እንቅስቃሴወች የሚሰጠው የሚሰጠው ትምህርት
ይህ አሳዛኝ ክስተት አንድ አስፈላጊ እውነታ ያስታውሰናል።
ድንገተኛ የልብ ምት መቆም ሁልጊዜ በቀድሞ የነበረ የልብ በሽታ ምክንያት ብቻ አይከሰትም። አንዳንድ ጊዜ በደረት ላይ የሚደርስ አንድ ሜካኒካዊ ምት ብቻ የልብን ኤሌክትሪካዊ ሥርዓት በማወክ ለሞት ሊያደርስ ይችላል።
ስለዚህ በስፖርት ሜዳዎች ላይ፦
- AED መኖር አለበት
- CPR የሚያውቁ ሰዎች መኖር አለባቸው
- የድንገተኛ አደጋ ምላሽ ዕቅድ መኖር አለበት
- ወደ ሕክምና ተቋም ፈጣን የማድረስ ሥርዓት መዘጋጀት አለበት

| 10 |  +3CT-Guided Localization Using an Endoscopically Placed Metallic Clip to Facilitate Laparoscopic Colectomy for an Early Colonic Lesion
Case Presentation
A 45-year-old female presented with nonspecific abdominal pain and discomfort. Colonoscopy performed during a general health check-up revealed a partially sessile colonic polyp, for which piecemeal polypectomy was performed.
Unexpectedly, histopathological examination revealed invasive adenocarcinoma, with uncertain resection margins, raising concerns about complete excision.
Following shared decision-making, the patient initially chose endoscopic surveillance. To facilitate potential future surgical intervention, an endoscopically placed metallic clip was used to mark the original polypectomy site.
CT-Guided Localization
CT imaging successfully identified the metallic clip at the sigmoid-descending colon junction, providing a precise anatomical landmark for future surgical planning.
Three months later, follow-up colonoscopy revealed a recurrent lesion at the same site, with biopsy demonstrating tubulovillous adenoma.
After discussing the available treatment options, including EMR, ESD, and surgical resection, the patient elected to undergo surgery.
Surgical Management and Outcome
A laparoscopic left hemicolectomy was successfully performed, with the previously placed metallic clip serving as an important landmark for intraoperative identification of the original lesion site.
Clinical Significance
This case demonstrates the value of combining endoscopic metallic clip placement with CT imaging for preoperative localization of small or difficult-to-identify colonic lesions, supporting accurate surgical planning and facilitating laparoscopic resection.

Burnout is affecting Ethiopia’s future doctors. The numbers show increasing trend which needs attention.
2019 : Debre Berhan University
*34% prevalence of burnout among medical students
*61.8% high emotional exhaustion
*47.9% high depersonalization
*59.7% low personal accomplishment
2021 : St. Paul’s Hospital Millennium Medical College (Interns):
*69.4% emotional exhaustion
*41.7% depersonalization
*44.4% reduced personal accomplishment
*37% high perceived stress
Financial stress and thoughts of leaving medicine were strongly associated with burnout.
A 2026 study from Tikur Anbessa Specialized Hospital on Interns showed high levels of emotional exhaustion and depersonalization among medical interns, with workload pressure and psychological stress as key contributing factors.
Burnout is not an individual weakness. It is a system level issue. The health work force is showing burnout even before starting.
If we want resilient doctors and safe patient care we must:
* Strengthen mentorship and supervision
* Integrate mental health support services into medical schools
* Address excessive workload and stress
* Promote open conversations about well being
The well being of medical students and interns is not optional. It is essential for the future of healthcare in Ethiopia.
