Rumusan Format Pentaksiran Matematik Tahun 4 Pentaksiran Matematik Tahun 4 dilaksanakan dalam bentuk ujian bertulis yang mengandungi item Subjektif Respons Terhad (SRT) dan Subjektif Respons Terbuka (SRTb). Jumlah keseluruhan soalan ialah 15 soalan, terdiri daripada Bahagian A (13 soalan, 26 markah) dan Bahagian B (2 soalan, 24 markah) dengan jumlah keseluruhan 50 markah. Pentaksiran ini mengukur pelbagai tahap pemikiran murid iaitu mengingat, memahami, mengaplikasi, menganalisis dan menilai. Tempoh ujian ialah 1 jam 15 minit dan meliputi kandungan dalam DSKP Matematik KSSR Tahun 1 hingga Tahun 4. Aras kesukaran soalan dibahagikan mengikut nisbah rendah : sederhana : tinggi = 5 : 3 : 2, manakala pemarkahan menggunakan kaedah analitikal.
