Goal: improve natriuresis and diuresis.
Class IIb, Level B1.
𝐀𝐬𝐬𝐞𝐬𝐬 𝐭𝐡𝐞 𝐞𝐚𝐫𝐥𝐲 𝐫𝐞𝐬𝐩𝐨𝐧𝐬𝐞 𝐭𝐨 𝐈𝐕 𝐥𝐨𝐨𝐩 𝐝𝐢𝐮𝐫𝐞𝐭𝐢𝐜𝐬
-Check spot urine Na⁺ about 2 hours after treatment.
-A good response is approximately urine Na⁺ ≥70 mmol/L.
-Urine output is another practical marker of response.
𝐈𝐧𝐢𝐭𝐢𝐚𝐥 𝐈𝐕 𝐥𝐨𝐨𝐩 𝐝𝐢𝐮𝐫𝐞𝐭𝐢𝐜
-Diuretic-naïve → approximately furosemide 40 mg IV.
-Already taking chronic loop diuretic → start around double the daily oral dose according to the ESC 2026
𝐆𝐨𝐨𝐝 𝐫𝐞𝐬𝐩𝐨𝐧𝐬𝐞 → 𝐜𝐨𝐧𝐭𝐢𝐧𝐮𝐞 𝐮𝐧𝐭𝐢𝐥 𝐞𝐮𝐯𝐨𝐥𝐚𝐞𝐦𝐢𝐚
-If natriuresis/diuresis is adequate, repeat the effective diuretic regimen, generally every 12 hours, until complete decongestion.

Do not continue the same ineffective dose.
Double the loop-diuretic dose, or consider sequential nephron blockade with another diuretic.
Options include IV acetazolamide or an appropriate thiazide/thiazide-like diuretic, depending on renal function and previous loop-diuretic use.
𝐏𝐞𝐫𝐬𝐢𝐬𝐭𝐞𝐧𝐭 𝐝𝐢𝐮𝐫𝐞𝐭𝐢𝐜 𝐫𝐞𝐬𝐢𝐬𝐭𝐚𝐧𝐜𝐞
—Progressively escalate loop diuretics toward the maximally appropriate IV dose.
—Add another non-loop diuretic when necessary.
—Ultrafiltration is a rescue/selective strategy
ESC 2026: ultrafiltration may be considered for refractory volume overload unresponsive to escalating diuretic therapy — Class IIb, Level C.
