Many people take painkillers like diclofenac, ibuprofen, naproxen, ketoprofen, piroxicam, and others almost every day without knowing they can cause serious harm if used wrongly.
What are NSAIDs?
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs) are medicines commonly used to relieve:
• Pain
• Fever
• Inflammation
• Joint and muscle aches
Examples include:
• Diclofenac
• Ibuprofen
• Naproxen
• Piroxicam
• Ketoprofen
• Indomethacin
• Meloxicam
While these medicines are effective, they are not meant to be taken carelessly.
Hidden dangers of NSAIDs
1. Stomach ulcers and bleeding
NSAIDs reduce the protective mucus that lines the stomach, increasing the risk of:
• Gastritis
• Stomach ulcers
• Vomiting blood
• Passing black, tarry stools
2. Kidney damage
Long-term or excessive use can reduce blood flow to the kidneys, leading to acute kidney injury or worsening existing kidney disease.
3. Increased blood pressure
NSAIDs can raise blood pressure and may reduce the effectiveness of blood pressure medicines.
4. Heart problems
Some NSAIDs, especially when used for long periods or in high doses, may increase the risk of heart attack and stroke.
5. Fluid retention
They can cause swelling of the legs and worsen heart failure in susceptible individuals.
6. Asthma attacks
In some people with asthma, NSAIDs can trigger severe breathing difficulty.
Who should be especially careful?
• People with stomach ulcers
• Patients with kidney disease
• People with heart failure
• People with uncontrolled hypertension
• Elderly individuals
• Pregnant women (especially in late pregnancy)
How can you reduce the risk?
• Use the lowest effective dose for the shortest possible time.
• Avoid taking more than one NSAID at the same time unless prescribed.
• Take them after food if advised by your healthcare provider.
• Do not mix them with alcohol.
• Inform your doctor if you have kidney disease, heart disease, or a history of ulcers.
#NB:NSAIDs are excellent medicines when used correctly, but they are not harmless. Frequent or unnecessary use can damage your stomach, kidneys, heart, and blood vessels. Always use them responsibly and seek medical advice if you need them regularly.

☑️ First-Line Therapy: Xanthine Oxidase Inhibitors (XOIs)
Allopurinol is King:
☑️ It is indeed the universally preferred first-line agent for urate-lowering therapy (ULT).
☑️ The CKD Paradigm Shift:
✔️ Historically, clinicians were hesitant to use allopurinol in severe chronic kidney disease (CKD) due to the risk of
Allopurinol Hypersensitivity Syndrome (AHS).
☑️ Current guidelines completely support its use as the first-line agent 👉even in Stage 3 or higher CKD👉 (eGFR < 60 mL/min).
✔️✔️ Start Low and Go Slow:
🔴 To mitigate AHS and prevent acute gout flares triggered by rapid uric acid shifts, the 😳👉standard mandate is to start at a 👉low dose (\le 100 mg/day,
or \le 50 mg/day in CKD)
👉 followed by gradual dose titration to reach the target serum urate (SU) level
(typically < 6\text{ mg/dL})
👉 rather than starting at a higher fixed dose.
🔴 Second-Line / Refractory Cases:

✔️When to Switch:
🟤 Febuxostat (starting at \le 40 mg/day) is reserved for patients who either cannot tolerate allopurinol 👉👉(e.g., due to severe hypersensitivity or rash)
👉or remain refractory to it despite maximum appropriate dose titration.
