A Man Without a Spleen Needs Those Vaccines. That's Not the Same Patient as Everyone Else.
Asplenia leaves someone genuinely defenseless against encapsulated bacteria — pneumococcus, meningococcus, Hib — and for that patient the case is overwhelming. Individual risk assessment is what medicine is supposed to be. What it has drifted toward is a single recommendation applied to every body regardless of age, history, or exposure, in a system where uptake is the metric everyone is graded on. Risk stratification isn't a rejection of vaccines. It's the difference between a physician and a protocol.
