Technique of a nonoperative Ponseti correction of clubfoot: (A) The thumb is positioned over the lateral aspect of the head of the talus and the fingers correct the forefoot. No counterpressure should be applied at the calcaneocuboid joint because the entire foot must be abducted under the talus. (B ) The cavus and adduction are corrected by slight supination of the forefoot in relation to the hindfoot. Why We Fix the Distal Donor site Fibula in Pediatric Patients? For those of us in pediatric orthopedics, a fibular harvest isn't just about the graft it’s about preserving the long-term integrity of the donor site. When we harvest a fibula in a growing child, we aren't just taking bone, we are removing a critical stabilizer. If the distal remnant is left "floating" or unsupported, we risk more than just post-op discomfort. Research highlights that pediatric patients are at a higher risk for progressive valgus ankle deformity and instability due to the loss of this lateral support. By performing a prophylactic distal tibiofibular synostosis, we bridge the gap. This simple but essential step: Anchors the distal remnant to the tibia. Provides the necessary lateral stability for the ankle joint. Helps prevent long-term valgus malalignment as the child grows. To the next generation of surgeons: Never underestimate the importance of donor site management. It’s not just about the procedure today it’s about the patient’s function for the next fifty years.
