Salter, Pemberton and Steel:
Salter innominate osteotomy is a redirectional osteotomy in which the acetabular fragment is rotated anterolaterally to improve femoral-head coverage, mainly for DDH in young children.
Pemberton osteotomy is an acetabuloplasty hinging on the triradiate cartilage that reshapes the acetabular roof and increases anterolateral coverage.
Steel triple osteotomy involves osteotomies of the ilium, ischium and pubis, allowing greater reorientation of the acetabulum and is useful in older children/adolescents.
Sutherland, Chiari and Dial:
Sutherland double osteotomy involves the ilium and pubis and allows acetabular reorientation without an ischial cut.
Chiari osteotomy is a salvage procedure in which the distal acetabular fragment is shifted medially to provide a bony buttress and improve coverage, mainly for older patients with dysplasia/subluxation.
Dial osteotomy is a curved acetabular redirectional osteotomy that rotates the acetabular fragment to improve anterolateral femoral-head coverage.

| 10 | Important points on Hip spica cast :
It encompasses the trunk and one or both lower limbs and is commonly used for pediatric femoral shaft fractures and after pediatric hip surgery.
For fracture treatment, the usual position is hip 45° flexion with slight abduction (to clear the perineum and facilitate hygiene), knee 45° flexion (to help maintain fracture reduction), and ankle in neutral.
The cast extends proximally to the nipple/rib cage for bony support. Distally, it may be single spica (one leg to foot), one-and-a-half spica (one leg to foot, other to knee), or double spica (both legs to foot).
For immediate spica application, a common indication is femoral shaft fracture in children <5–6 years with <2 cm shortening.
Important complications are loss of reduction, angulation, shortening, compartment syndrome, skin problems, urinary retention, and plaster cast syndrome.
Plaster cast syndrome results from duodenal obstruction between the aorta and SMA, causing vomiting, abdominal pain and distension; management includes immediate cast removal, NPO, IV fluids, antiemetics, NG decompression and electrolyte monitoring.
In DDH, the hip is placed in the human position—about 95° flexion and 40–45° abduction, which provides good stability while reducing the risk of femoral head osteonecrosis.

| 14 | Technique of a nonoperative Ponseti correction of clubfoot:
(A) The thumb is positioned over the lateral aspect of the head of the talus and the fingers correct the forefoot. No counterpressure should be applied at the calcaneocuboid joint because the entire foot must be abducted under the talus.
(B ) The cavus and adduction are corrected by slight supination of the forefoot in relation to the hindfoot.
The forefoot is never everted; rather, it is displaced as a unit.
