For those of us in pediatric orthopedics, a fibular harvest isn't just about the graft it’s about preserving the long-term integrity of the donor site.
When we harvest a fibula in a growing child, we aren't just taking bone, we are removing a critical stabilizer. If the distal remnant is left "floating" or unsupported, we risk more than just post-op discomfort. Research highlights that pediatric patients are at a higher risk for progressive valgus ankle deformity and instability due to the loss of this lateral support.
By performing a prophylactic distal tibiofibular synostosis, we bridge the gap. This simple but essential step:
Anchors the distal remnant to the tibia.
Provides the necessary lateral stability for the ankle joint.
Helps prevent long-term valgus malalignment as the child grows.

| 20 | Pes Anserinus: The Goose Foot and Pes Anserine Bursitis
The pes anserinus means “goose foot.” It is the combined insertion of 3 tendons on the upper medial side of the tibia:
Sartorius – Gracilis – Semitendinosus
From front to back, the tendon order is:
Sartorius → Gracilis → Semitendinosus
Think: S-G-S
The tendons spread out near their tibial insertion and resemble a goose foot, which explains the name.
A useful memory aid is:
“Should Say Grace Before Tea”
This helps remember the local structures:
Sartorius – Saphenous nerve – Gracilis – Bursa – Semitendinosus
The saphenous nerve lies between the sartorius and gracilis.
The pes anserine bursa lies deep to the tendons near the medial proximal tibia.
The gracilis is important because it is the only adductor muscle that does not insert on the femur. Instead, it inserts on the tibia as part of the pes anserinus.
Pes anserine bursitis occurs when the small bursa between the tibia and the pes anserine tendons becomes inflamed. The patient usually has pain and tenderness below the medial joint line on the proximal medial tibia.
This location is important:
Pain at the medial joint line suggests medial meniscus tear or arthritis.
Pain below the medial joint line suggests pes anserine bursitis. | 838 |
