For those of us in pediatric orthopedics, a fibular harvest isn't just about the graft it’s about preserving the long-term integrity of the donor site. When we harvest a fibula in a growing child, we aren't just taking bone, we are removing a critical stabilizer. If the distal remnant is left "floating" or unsupported, we risk more than just post-op discomfort. Research highlights that pediatric patients are at a higher risk for progressive valgus ankle deformity and instability due to the loss of this lateral support. By performing a prophylactic distal tibiofibular synostosis, we bridge the gap. This simple but essential step: Anchors the distal remnant to the tibia. Provides the necessary lateral stability for the ankle joint. Helps prevent long-term valgus malalignment as the child grows.
