Compared with other sisters who died, she had relatively high counts of neurofibrillary tangles and senile plaques. Sister Mary also had many health problems, from cancer to heart disease, and had progressively lost weight during the last few years of her life. Such weight loss, numerous chronic diseases, and abundant Alzheimer's disease lesions would be expected to affect the cognition of most people. Sister Mary, however, appeared to be cognitively intact, with good short-term memory and excellent orientation to her surroundings and current events, in spite of being an apparently frail centenarian.

The sisters were asked to write autobiographical essays when they were, on average, 22 years old. Upon review, it was found that an essay's lack of linguistic density (e.g., complexity, vivacity, fluency) functioned as a significant predictor of its author's risk for developing Alzheimer's disease in old age. However, the study also found that the Sisters who wrote positively in their personal journals were more likely to live longer than their counterparts.
