اجاك مريض فاقد الوعي او وعيه مخربط يعني البرزنتيشن هو altered mental status زين هسه شنو يعني هذا الكيس CVA و اخابر ابو النيور مدسن كبل ؟؟؟ الجواب اكيد لا اهم شي اقيس random blood sugar لانو احتمال هذا الكيس هايبو كلاسيميا ... وراها بعدها اجي اخلي سستم ابالي ... هذا المريض فاقد الوعي لوحده من ذني ال 3 مشاكل .... ياما structural metabolic toxicology زين خلي نجي على وحده وحده اولا : structural شنو يعني .. معناها اكو اسكيميا او هيمورج او ماس بالبرين ... ثانيا : metabolic احتمال hypoglycemia او hyponatremia او uremia او Sepsis ثالثا : Toxicology يعني احتمال هاي حالة تسمم من الكاربون مونوكسايد الى الكحول و ادويه الامفيتامين او ادويه ال benzodiazepine و غيرها زين شسوي حتى اشخص ذني حسب تحاليل اوليه من ضمنها CBC. RFT , LFT , s. electrolyte , CRP titer , ABG و GUE و CXR حتى اكدر اعرف الميتابولك كوز و ندز وراها Brain CT scan حتى نستبعد الحالات ال structural و ندز وراها tox screen اذا جان اكو شك بالموضوع التوكس ( حسب الهستري ). ⭐| @medical_st7 .

شلون نعالج الـ Neonatal hypoglycemia شي عملي و بسيط 1. Symptomatic but no seizure: dextrose water 10% 2 cc/kg 2. With seizure: Dextrose water 10% 4 cc/kg و بعدها نمشي المريض على 5 cc/kg/hr of dextrose water 10% Note: avoid dextrose 50%
