Presented with epigastric pain + vomiting + dizziness of 3 hrs duration
ECG: inferior STEMI + CHB
PPCI done + temporarly pacemake inserted
post intervention, the patient developed severe SOB + clear chest
pH 7.1, HCO3 9, lactate 10
serum for beta hydroxybutaric acid: -ve
so, the ABG is a lactic acidosis.. not DKA
but why the patient is so dyspnic??

Inferior STEMI + Right ventricular infarct RVMI + CHB
this pt has BP 195/115 .... shall we lower BP or not???
answer is:
1) exclude airtic dissection which can extends to right coronary artery and cause such features.
2) if aortic dissection was excluded, then no anti hypertensive should be given, since hypotension is very common, and even small doses of anti-HTN medication can result in shok.
shock state will further result in coronary hypoperfusion 👉👉 more infarction 👉👉 worsening of shock
this is very practical point, commonly missed

hx of hypertension, hypothyroidism, and fatty liver.
all are controlled and on regular visit.
presented with nausea and 2 episodes of vomiting
and was associated with neck pain and some sort of dizziness. All the hx of 2 days duration.
no abdominal pain, no change in bowel habit, no ENT symptoms apart from dizziness, no ocular symptoms.
but she reported hx of posterior neck pain and occipital pain, no neurological deficit.
كانت داخله طوارىء، و دازيها وورك اب كامل
ECG, CBC, LFT, RFT, s electrolytes, abd US, and chest X ray
all were normal
فبعدها الأهل طالعين ع مسؤوليتهم. المهم تم تقييم المريضة.
و هنا انت لازم تعرف ترتيب هاي الأحداث، شنو اول شي صار، و شنو اخر شي صار عند المريضة

عندما تسود الفوضى تصبح مثل هكذا تصرفات مستساغة عند الشعب.
عندما يقوم الأعلام #المنحط بتشويه صورة الطبيب و تصويرهُ كـعدوا للمريض، لا نتفاجىء بمثل هكذا تصرفات.
عندما لا يُعاقَب المُتّهم #قانونيا و أمام الملىء، عندها لا يخاف المواطن من السلوك السيء و الحيواني، فما من رادع له.
عندما يأمن الشخص بأن هناك #عشيرة، ستقف معهُ أمظلوماً كان أم ظالماً، عندها لن يقوم هذا الشخص الهمجي بالتفكير بما سيفعله، و سيستهل كل عمل قبيح يقوم به (فهناك من يدافع عنهُ).
عندما ترى عامة الناس مؤيدين لهذهِ الأعتداءات، عندها سيقوم المعتدي بالإكثار من مثلها، حتى يُصوَّر بعينِ الحمقى إنه #البطل.
عندما تغيب الدولة، فعليها إن تُعدَّ نفسها لمستقبل مليء بالحيوانات، لمستقبل خالي من الطبقة المثقفة، لمستقبل أشبه ما يكون بالغابة.

البيشنت الي يجيك و عنده chest pain
قبل لا تكمل الهستري
خلي المريض يتمدد على سدية، لازم تفرغله سرير. (مو تخليه يشرحلك و تاخذ detailed hx و هو واقف, غريب يعني).. لا و فوگاها تاخذ من عنده drug allergy و تسأل عن البنسلين اليرجي !!!
أخوان يعني احنه حافظين حفظ لو فاهمين، لو أهم شي نكتب بالكارت حتى لو كلام مو مهم !!!
بعد ما تخليه ع السدية، مباشرة تحصل IV line، و الأفضل اثنين، كانولا زرقاء و فوك.
و انت تاخذ هستري، تكمل فتح الكارت + تبلغ الممرض يجي يسوي تخطيط ع السريع
بهذا الوقت، اي شي طارىء يصير، فأنت مستعدله.
و على ضوء التخطيط، راح تكمل شغلك وياه.
و أعتقد هاي بديهات، بس نشرت هالشي لأن موقفين صارن گدامي، كلش صعب واحد يتقبلهن ع طبيب. | 7 438 |

she with post prandial pain + vomiting for more than 7 mn ... which make her to avoid oral intake .... and lost significant weight.
extensive medical workup was -ve including: CBC, LFT, RFT, TSH, S cortisol, s electrolytes, lipase, amylase, abd US, multiple OGDs, and many trials of PPI.
brain MRI + ophthalmological review were normal.
pw weakness .
inx showed hypokalemia and hypo Mg

كتبتهم كلهم رير، لأن ما معقولة هيج هستري و الف مراجعة و اني اكتبله مثلا

و هءا كلش يفيد ابو الاشعة، حتى اخليه يعرف بشنو جاي افكر انا.

presented w abd pain and vomiting. preceded by hx of sore throat .
one of them, yesterday visited another teaching hospital, and treated as simple flu.
they are otherwise, healthy and active.
as part of workup, their RBS was higher than 450 !!!
VBG showed high anion gap metabolic acidosis HAGMA.
Both of them presented as DKA as the first manifestation of DM.

hx of DM, HTN, IHD, old non-disabiling ischemic stroke
13 days ago, she underwent emergent lapratomy for sigmoid volvolus.
After that, she srarted to develop decrease oral intake ... till 2 days, she has anuria.
she readmitted again.
her renal indices are tripled from baseline.
considered to be pre-renal AKI over CKD.... Thus, the team started to rehydrate the pt with NS + GS.
Despite good hydration, the pt has developed oliguria with new onset dyspnia and chest crept.

the team consult nephrology unit for opinion about hemodialysis HD.
when I assessed the pt, she was in respiratory distress, bilateral chest crept upto mid zones.
POCUS: IVC ≈ 2, and collapsible.
فهنا انتةالمفروض تستحضر ال ddx. ممكن من الوهلة الأولى، و التيم يقرأ براسك، تكولةالمريضة تحتاج ديالزز و ultrafiltration، و خلص.
بس مو هيج الوضع ...
wholistic approach for this pt is
acute in hospital dyspnia with chest crepts.
ddx:
1) fluid overload (hypervolemic lungs) .... which is less likely since IVC is collapsible
2) pulmonary embolism
3) new aspiration pneumonia
4) new cardiac events
هاي الي تجي ع البال
لكن من تصفن اكثر و تدقق بالهستري ... خليت احتمالات اخرى
5) thiamine deficiency
6) refeeding syndrome precipitated by glucose infusion (hypophosphatemia)

basic workup

s PO4 level ... and it was 1.33 ... very low
we stopped GW + we gave phosphate IV + empirical thiamine given + calori intake is adjusted to 1/3rd.

heavy smoker .... otherwise -ve hx.
presented with sudden onset thunderckap upper and lower back pain, when he is lying flat. Associated with diaphoresis ... then the pain started to fade ... at time of ER admission, the pain severity decreased by 80%, as the pt said.
his ECG was normal as well as his troponin.

ساعة ما وصلتله، الحجي زين، و يگول بس اگعد الألم يخف ... بس انام يزداد ... و بكل الأحوال، حالتي جدا زينه حاليا و أريد أطلع.
لحسن الحظ انه بنته كانت طبيبة و #متفهمة و ما تتدخل بقرارت الطبيب.
