#شذرات في Acute severe ulcerative colitis شلون نعرف حالة Ulcerative colitis is an acute severe one الجواب عن طريق Truelove and Witts criteria: Bloody stool frequency of ≥ 6 per day plus any of • Tachycardia (>90 bpm) • Temperature (>37.8°C) • haemoglobin <10.5g/dL • Elevated ESR (or CRP) >30 .... شنو هي Definitive Rx الجواب Either methylPRD 60 to 80 mg once daily or hydrocortisone 100 mg three to four times daily for at least 3 days .... شلون نعرف المريض استجاب للعلاج؟ The Oxford day 3 criteria predicts failure to respond to intravenous corticosteroids by assessing stool frequency and/ or CRP at day three. • Stool frequency ≥ 9 or • CRP ≥ 45 mg/ L and a stool frequency of 3– 8 per 24 hours .... شنو هي الخيارات في حالة عدم الاستجابة؟ Ciclosporin and Infliximab are both recognized rescue therapies with placebo- controlled evidence supporting their use and RCTs demonstrating similar efficacy .... يا هو نختار Infliximab vs Ciclosporin Factors favouring infliximab • hypocholesterolaemia (<3 mmol/L) or hypomagnesaemia (<0.5 mmol/L) (increased risk of neurotoxicity with ciclosporin) • hyperkalaemia or hypertension • Epilepsy • Renal/hepatic impairment Factors favouring ciclosporin • Shorter half-life — if more likely to need colectomy; will clear the system quicker • Azathioprine-naïve patients • history of tuberculosis, hepatitis B, multiple sclerosis or heart failure (NYhA III or IV) .... في حال ما استجاب على rescue therapy بعد ٣ الى ٥ ايام شنو هي next step Colectomy is the next step

السلام عليكم … كلنا نعرف ال NAC (N acytel cystine) هو الانتي دوت مال براستول توكسستي ! وممكن نستعمله ك mucolytic agent ! وكذلك ك anti-oxidant agent ! هاي اشياء نعرفها .. ….. بس اكو استعمال جديد ب (زرع نخاع العظم )الي هو NAC treatment was associated with a reduced incidence of ( transplantation-associated thrombotic microangiopathy) … And it was associated with a more event free survival in bone marrow transplant pts as compared to placebo ! … NAC prophylaxis was safe and effective in preventing the occurrence of Poor hematopoitic function by repairing the impaired bone marrow EC..
