1. I am deeply saddened and troubled to learn that the government is wrongfully accusing health professionals—such as Dr. Samuel Hailu, a brilliant and dedicated physician who was my junior at TASH, and Dr. Mahlet Guush, a nursing mom—of inciting unrest and rioting. 2. This action sets a dangerous precedent. If we do not unite and respond now, it could affect every one of us in due time. 3. I strongly urge all senior physicians across the country to urgently convene and consider halting work until a solution is found. 4. The persecution of these professionals is also an attack on all of us. How can it be that someone who simply demands their rights in their own country is punished as if they’ve committed a crime? 5. I encourage TEAM DEBOL and similar groups to document these unlawful actions against professionals and promptly report them to international human rights bodies such as Amnesty International—before it’s too late. 6. I also call on His Excellency Dr. Abiy Ahmed to engage in dialogue with health professionals to find a way out of this crisis before the country reaches an irreversible point. Perhaps lower-level cadres are providing misleading information regarding the health sector. 7. I strongly advise all healthcare professionals not to be coerced into silence or submission, and not to allow their movement to be derailed. 8. Hello again. One of our colleagues, Dr. Biniyam Dadi (a medical intern at SPHMMC), was detained by the police at his home. He was detained because he appeared in a viral TikTok video related to the pre-strike demonstration.

"I have resigned from my Medical Director position" To: Team Debol I am a senior physician, and I feel a deep sense of shame—not only in myself but in all who carry the title “senior.” We have failed our heroic interns, residents, general practitioners, other health professionals, and the public due to our selfishness, egos, and narcissistic behavior. We missed a critical opportunity to reform and build a responsible, efficient health system. Today, I officially resigned from my position as medical director at the hospital where I work. This decision stems from my failure to stand by my fellow physicians and health workers when they were unjustly arrested and detained. In this moment, I wish I were an intern or a resident. I no longer have the interest or willingness to listen to moral teachings or pursue fellowship training in this country, particularly from senior figures who have repeatedly allowed this system to collapse. Medical Ethics ከ20 ዓመት በላይ አስተምሬለሁ! (Professor Wondimagegn Embiale) These days, the dominant narrative from Ministry of Health (MOH) officials seems to be: “Health professionals have a duty to be compassionate to their patients.” Today, I listened to an interview with the State Minister of Health, who cited various professional codes of ethics and concluded emphatically: “Health professionals have an obligation to their patients, and stopping work—even for a minute—is a crime.” As someone who has taught medical ethics for the past 20 years, I found this statement both deeply concerning and overly simplistic. I served as the course chair for medical ethics at Bahir Dar University’s College of Medicine and Health Sciences for a decade, and for five years, I was also involved in teaching medical ethics and professional conduct at the national level under the MOH. These roles have given me a front-row seat to the moral dilemmas and systemic pressures that health professionals face daily. While the ministry has acknowledged that the concerns of health professionals have been dragging on unresolved for years, it remains silent on a more urgent issue: the living conditions and economic realities of the workforce. The current salary structure and incentive system in the civil service are not just inadequate—they are dehumanizing. Expecting a medical doctor to work for the equivalent of $2.50 USD per day—below the World Bank’s international poverty line—is, in effect, a form of modern slavery. Does the ministry realize that the monthly salary of a first-degree holder does not even cover the cost of a single romantic dinner in Addis Ababa, let alone the needs of a family? The silence on this injustice is deafening. I had hoped that ministers—particularly those who are health professionals themselves and fully understand the bleeding state of the system—would have taken a stand and resigned in solidarity with the call for genuine health system reform. Yes, compassion is undeniably a cornerstone of medical professionalism. But to frame any service interruption as a “crime” is to ignore the structural injustices and burnout that plague the civil service. Ethics is not about blind obedience. It is about thoughtful, principled decision-making—even when the circumstances are difficult. Compassion must go both ways. If we expect health professionals to serve with dignity, we must also ensure they are treated with dignity. Without fairness and justice, ethical principles risk becoming tools of control rather than instruments of care. It’s time we stop using the language of ethics to silence those who are simply asking to be treated as human beings.

Dr. Dereje, with all due respect, We won’t let you flip the narrative. The One Who Failed the Oath Is You! … You went on national television and threatened Ethiopian healthcare workers, your own colleagues, for going on strike. You claimed it is “inexcusable” to leave emergency rooms and labor wards. You spoke of revoking licenses. You wrapped yourself in the language of responsibility, ethics, and professional duty. But let’s turn the table, Dr. Dereje. You’re not defending the health system. You are the face of its failure. You are not the victim here. You are not the authority with moral ground to stand on. You are the one who took an oath, not just as a physician, but as a senior public official, to build a functional, fair, and dignified health system.. And you broke that oath. …. Where is the system you promised? Where are the safe hospitals, the basic equipment, oxygen, the life saving drugs? Where are the fair wages, the functioning wards, the human dignity? …. You know the truth better than anyone, because you trained in this very system. You witnessed its suffering. You lived through the same hellish hospital conditions, the lack of gloves, broken monitors, mothers screaming through childbirth without help. when you finally had the power to change it, you chose the path of denial, comfort, and self-preservation. Now, when doctors, nurses and other healthcare professionals many of whom are literally going hungry, rise up to say “enough is enough,” you respond not with action, not with listening, but with threats? No. We will not let you flip the narrative. How can a broken system deliver care when even its caregivers are starving? Dr. Dereje, let’s not sugarcoat it. It’s not the workers who abandoned the health system. It’s you. It’s not the strike that is killing the system. It’s your years of inaction, excuses, and betrayal. This strike didn’t come out of nowhere. It is the final cry of a workforce that has been ignored, starved, and disrespected for far too long. And your response only proves their point, those in power would rather punish than fix. …. You’re not defending patients. You’re defending your position. You’ve become the shield for a broken, corrupt structure that grinds down its most vital workers. You’ve turned from healer to enforcer. From physician to politician. You will be remembered as the doctor who chose power over people. So don’t talk to us about oaths, ethics and moral, Dr. Dereje. You broke yours long before any healthcare worker walked off the job. And if Ethiopia were a country where accountability truly mattered and law enforcement functioned as it should, it wouldn’t be the healthcare workers losing their licenses. It would be you. የአክሽን ኤድ ጥናት ስለ ጤና ባለሙያዎች ምን ይላል ? አክሽን ኤድ በአፍሪካ ስድስት ሃገራት በጤና እና ትምህርት ዘርፉ ላይ አካሂዶ ይፋ ባደረገው ጥናት 32 በመቶ (የሀገራቱ አማካኝ) የሚሆኑት የጤና ባለሞያዎች በክፍያ ማነስ ምክንያት ሞያውን ለመልቀቅ በማሰብ ላይ ይገኛሉ ሲል ገልጿል። ጥናቱን ፦ 🇪🇹በኢትዮጵያ 🇬🇭በጋና  🇰🇪በኬንያ 🇱🇷በላይቤሪያ 🇲🇼በማላዊ 🇳🇬በናይጄሪያ 600 የጤና ባለሙያዎች፣ መምህራንና የማህበረሰብ ክፍሎችን በማሳተፍ ነው ያካሄደው። በጥናቱ የተሳተፉ ኢትዮጵያውያን የጤና ባለሙያዎች ምላሽ በሪፖርቱ የሰፈረ ሲሆን የቀረቡት ጥያቄዎችም እ.አ.አ 2020 ጋር በማነጻጸር የቀረቡ ስለመሆናቸው ጠቅሷል። የኢትዮጵያውያኑ የጤና ባለሞያዎች ምላሽ ምን ይመስላል ? - ስልሳ አምስት በመቶ (65%) የሚሆኑት የጤና ባለሙያዎች በክፍያ ማነስ ምክንያት ሞያውን ለመልቀቅ በማሰብ ላይ ይገኛሉ። (በጥናቱ ከተሳተፉ 6 ሀገራት ትልቁ ነው) - መቶ በመቶ (100%) የሚሆኑ የጤና ባለሞያዎች የጤና መገልገያ ቁሶችና መድኃኒት እጥረት መኖሩን ጠቁመዋል። (በጥናቱ ከተሳተፉ 6 ሀገራት ትልቁ ነው) - ሰባ አምስት በመቶ (75%) የሚሆኑት ለቤተሰቦቻቸው የሚያቀርቡት የምግብ መጠን መቀነሱን አንስተዋል። - መቶ በመቶ (100%) የሚከፈላቸው ክፍያ አነስተኛ መሆኑን ገልጸዋል። - ዘጠና በመቶ (90%) የሚሆኑት የበጀት ቅነሳ እናቶችና ህጻናት ጤና ላይ ከፍተኛ ጫና ፈጥሯል ብለው ያስባሉ። - ዘጠና አምስት በመቶ (95%) የሚሆኑት ለጤናው ዘርፍ በቂ በጀት አልተመደበም ብለው ያምናሉ። - ሀምሳ ሶስት በመቶ (53%) የሚሆኑት የታካሚዎች ቁጥር ጨምሯል ብለው ያምናሉ። - መቶ በመቶ (100%) የሥልጠና ዕድሎች ቀንሰዋል ብለው ያምናሉ። - ሰባ አምስት በመቶ (75%) የሚሆኑት ሞያቸው የግል ህይወታቸው ላይ (Work-life Balance) ጫና መፍጠሩን ገልጸዋል። - መቶ በመቶ (100%) የሚሆኑ የጥናቱ ተሳታፊዎች ለጭንቀት የመጋለጥ እድላቸው እየሰፋ መምጣቱን አንስተዋል። ጥናቱ መንግስታት ውጤት አልባ ከሆነ የIMF የበጀት መቀነስና ዕዳ መክፈል ፖሊሲ ወጥተው ትኩረታቸውን ዜጎቻቸው ላይ ማድረግ አለባቸው ሲል ምክረ-ኃሳብ ያስቀምጣል። በአቡጃ ስምምነት መሰረት የአፍሪካ ኅብረት አባል ሀገራት ከአጠቃላይ በጀታቸው 15 በመቶ የሚሆነውን ለጤናው ዘርፍ እንዲያውሉ ይጠበቃል። ቲክቫህ ኢትዮጵያ አዲስ አበባ #TikvahEthiopiaFamilyAA @tikvahethiopia

" መንግስታት ውጤት አልባ ከሆነ የIMF የበጀት መቀነስና ዕዳ መክፈል ፖሊሲ ወጥተው ትኩረታቸውን ዜጎቻቸው ላይ ማድረግ አለባቸው " - አክሽን ኤድ በአፍሪካ ስድስት ሃገራት በጤና እና ትምህርት ዘርፉ በጀት በመቀነሱ ሰራተኞች የከፋ ችግር ውስጥ መውደቃቸውን አዲስ ጥናት አመላከተ። ጥናቱን አክሽን ኤድ የተሰኘ ግብረሰናይ ድርጅት የሰራው ሲሆን ፦ - በኢትዮጵያ - በጋና - በኬንያ - በላይቤሪያ - በማላዊ - በናይጄሪያ 600 የጤና ባለሙያዎች፣ መምህራንና የማህበረሰብ ክፍሎችን ያሳተፈ ነው ተብሏል። በጥናቱ ከተሳተፉት መሃል 97 በመቶ የሚሆኑት የጤና ባለሙያዎች የሚያገኙት ደመወዝ መሰረታዊ የሚባለውን ወጪያቸውን ለመሸፈን እንደማያስችላቸው ተናግረዋል። 97 በመቶ የሚሆኑት የጤና ባለሙያዎች በደመወዛቸው የቤት ኪራይ መክፈል፣ የምግብ እና ሌሎች ወጪዎችን መሸፈን እንዳልቻሉ አንስተዋል። በጥናቱ በ6ቱ ሀገራት የሚገኙ የጤና ተቋማት ተገልጋዮች በሃኪሞች እጥረት እና በዋጋ መጨመር ደስተኛ አይደሉም ሲል ያስቀመጠ ሲሆን መንግስታት ውጤት አልባ ከሆነ የIMF የበጀት መቀነስና ዕዳ መክፈል ፖሊሲ ወጥተው ትኩረታቸውን ዜጎቻቸው ላይ ማድረግ አለባቸው ብሏል። ህመምተኞች ወደ ጤና ተቋማት ለመሄድ ረጅም መንገዶችን እንደሚጓዙ፤ የወባ መድኃኒትም በ10 እጥፍ ዋጋው መጨመሩ በጥናቱ የተካተተ ነው። ለአብነትም በኢትዮጵያ ሙያከላ ቀበሌ የምትኖር ሴትን በመጥቀስ ከ5 አመት በፊት የወባ መድሃኒት በ50 ብር ትገዛ እንደነበር እና አሁን ግን ከግል ተቋማት በ500 ብር እንደሚገዛ መናገሯን ገልጿል። በሌላ በኩል ፥ በጥናቱ ከተሳተፉት 87 በመቶ የሚሆኑት መምህራን የክፍል ውስጥ ቁሳቁሶች /የማስተማሪያ ቁሳቁሶች እንዳልተሟሉላቸው ሲገልፁ ፤ 73 በመቶ ያህሉ የትምህርት ማስተማሪያ ቁሳቁሶችን ከኪሳቸው አውጥተው እንደሚገዙ ተናግረዋል። 84 በመቶ መምህራን ደመወዛቸው ባለፉት 5 አመታት ከ10 እስከ 50 በመቶ በሚሆን መጠን የቀነሰ መሆኑ ተመላክቷል። ለዚህ ሁሉ ቀውስ አክሽን ኤድ መንግስታት በትምህርት እና ጤና ላይ የሚያወጡት ወጪ መቀነሱ ነው ሲል ያስቀመጠ ሲሆን ዓለም አቀፉ የገንዘብ ተቋምም (IMF) መንግስታት የመከረበት መንገድ መኮነን አለበት ሲል ገልጿል። አይ ኤም ኤፍ መንግስታት እንደ ትምህርት እና ጤና ባሉ ልማቶች ላይ የሚያወጡትን ወጪ በመቀነስ ዕዳቸውን እንዲከፍሉ ይመክራል ያለው አጥኚው ተቋም፥ በማሳያነት በናይጄሪያ በ2024 ከሃገራዊ ገቢዋ 4 በመቶ ያህሉን ለጤና ስታውል 20 በመቶ ያህሉን ዕዳ ለመክፈል አውላለች ነው ያለው። በተለምዶ አንድ ሃገር የበጀቷን አንድ አምስተኛ ለትምህርት ማዋል አለባት ቢባልም በጥናቱ ከተካተቱት ስድስት ሃገራት ውስጥ አራቱ የትምህርት በጀታቸው ከዚህ መጠን በታች ነው። አክሽን ኤድ መንግስታት እና IMF እየሄዱ ያሉበትን መንገድ በመቀየር ለነዚህ ዘርፎች ትኩረት በመስጠት የሰራተኞቹን ኑሮ እንዲያሻሽሉ እና ዜጎችም ጥራት ያለው ትምህርት እና የጤና አገልግሎት እንዲያገኙ እንዲያደርጉ ምክረ ሃሳብ አቅርቧል። ቲክቫህ ኢትዮጵያ አዲስ አበባ #TikvahEthiopiaFamilyAA Via @tikvahethmagazine @tikvahethiopia
