Mr. Omar 45y male with free past medical history presents with fever (38.7) on the 4th postoperative day following an uncomplicated laparoscopic appendectomy performed for acute appendicitis.
Associated symptoms: mild right lower quadrant abdominal discomfort, malaise, and decreased appetite.
Denies cough, dysuria, chest pain, or calf swelling.
No rigors or chills.
Examination Findings
General: Alert, mildly ill, febrile (38.7), pulse 98 , BP 118/72 , RR 18.
Surgical site: Slight erythema and tenderness at the umbilical port site, minimal serous discharge. No purulent drainage.
Chest: Clear vesicular breath sounds bilaterally.
Abdomen: Mild tenderness in right iliac fossa, no rebound, no guarding.
Legs: No edema or tenderness.
WBC count 13,200
CRP 62
Urinalysis Normal
Chest X-ray Normal
Ultrasound abdomen Small localized collection (2×3 cm) near appendectomy site
Progress
After 48 hours of antibiotic therapy, the patient became afebrile.
