باختصار شديد، هاي حالة مرضية تشبه بالضبط المشي أثناء النوم (بس مختلفة عنها) تصير أثناء الـ REM وعادةً بالأشخاص فوك الـ 50 سنة. غالبًا يكون بيها المريض منفعل بسبب الحلم فيركض أو يضرب أو يصيح وهو نايم. لما تكعّد المريض غالبًا رح يتذكر اللي صار ورح يكون واعي بمحيطه. المريض المصاب بـ iRBD ممكن يعرض نفسه أو اللي بصفه لإصابات، وهاي الحالة كلش خطرة مو بسبب الإصابات، بس لأن أغلب المرضى المصابين بـ iRBD رح يصير عدهم synucleinopathy ورة كم سنة. رغم أنّ هالمرض يشبه المشي أثناء النوم بس هو مختلف عنه تمامًا. المشي أثناء النوم هو حالة حميدة غالبًا مالها علاقة بمشاكل الدماغ أو الـ synucleinopathy. وهي تصير بالـ non-REM والمريض غالبًا لما يكعد رح يكون مرتبك وغير واعي بمحيطه.

هي مجموعة أمراض عصبية مثل: • الشلل الرعاشي Parkinson's Disease (PD) • الخرف المصاحِب لأجسام ليوي Dementia with Lewy Bodies (DLB) • الضمور الجهازي المتعدد Multiple-System Atrophy (MSA) كلهن يُعتقَد أنّ سببهن هو بروتين واحد إسمه α-synuclein. هذا البروتين موجود بكثرة وبشكل طبيعي بالدماغ ويؤدي بعض الوظائف العصبية. بس هو بروتين غريب لأن باقي البروتينات أثناء تكوينها تنطوي وتلتوي بطريقة معينة وبعدها تحافظ على شكل ثلاثي الأبعاد بصورة ثابتة (tertiary structure) طول فترة وجودها. بالمقابل، الـ α-synuclein "مَرِن" وما عنده شكل ثلاثي الأبعاد ثابت. شكله يتغيّر دائمًا ولهذا يسموه «بروتين فوضوي بطبيعته intrinsically disordered protein». الفوضى بتركيبه رغم أنها تساعده بتأدية وظائف مختلفة بالجهاز العصبي إلا أنّها تعرّضه بنسبة أكبر من باقي البروتينات لأن يُطوى بشكل خاطئ misfolding بمعنى أنّ البروتين α-synuclein رح ياخذ شكل خاطئ يجعله يفشل بأداء وظيفته وفوكاها يخليه يلتصق بباقي البروتينات المشابهة إلة ويترسب بالخلايا العصبية بكميات جبيرة ويتسبب بقتلها. لما الـ α-synuclein يترسب بمنطقة إسمها المادة السوداء substansia negra فالمريض رح تظهر عنده أعراض الشلل الرعاشي. ولما البروتين يتجمع بأماكن ثانية مثل القشرة الدماغية وجذع الدماغ، فالمريض رح يُصاب بنوع مميز من الخرف يسموه الخرف المُصاحِب لأجسام ليوي (DLB). أمّا أجسام ليوي فهي تكتّلات جبيرة من نفس البروتين α-synuclein داخل الخلايا العصبية. آخر شي، إذا ترسّبت البروتينات بجذع الدماغ والمُخيخ والحبل الشوكي وأماكن ثانية، فالمريض رح يُصاب بمرض نادر هو الضمور الجهازي المتعدد (MSA) اللي معناه ببساطة أنّ عدّة أجهزة أو أجزاء بالدماغ رح تضمر، بالتحديد رح يتعطل الجهاز العصبي الذاتي autonomic NS والمُخيخ بالإضافة لأعراض مشابهة للشلل الرعاشي وأعراض ثانية مختلفة.
