هسه نجي ع Breast feeding Jaundice هذا جوندس بسبب نقص الحليب اثناء الرضاعة لذلك حت متنسوا خلو ابالكم هالاسم Poor breast feeding Jaundice لان هو ديصير لأن الطفل مديرضع بكميات كافية (لازم نورملي يترضع ٨ مرات باليوم يعني كل ٣ ساعات) اقل من هالعدد حيصيرلة جوندس لذلك مرات يسمو Breast feeding failure Jaundice زين ليش؟؟ لأن من تقل الرضاعة حيقل مصدر الطاقة Calories الي يعتمدها الطفل بالتالي حتقل حركة الامعاء bowel motion لان مصدر الطاقة قل وبالتالي البلروبين الجاي من الكبد والي ينزل بالامعاء والي المفروض يطلع بال feaces ما راح يطلع لان ماكو bowel motin تطلعه فيصير بي ركوووود ويبقة بالامعاء ويرجع للكبد عن طريق enterohepatic circulation هاذا عادة يصير بال 1st wk علاجة ايضا سهل وبسيط فقط تزيد عدد Breast feeding وتوصلها للنورمل رينج او شويه اعلى

(BFJ) VS. (BMJ) 1_Breast milk Jaundice هو أحد اسباب ال indirect hyperbilrubenemia واحنا نخاف من indirect bilrubin الي يعتبر نيورو توكسك لأن يعبر ال BBB ويسويلنا Kernictrus على عكس الدايركت الي ميگدر يعبر هذا BMJ شلون يصير وشنو اعراضة؟ يقال (Theory) انو بعض الامهات يحتوي حليبهن على glucurdinase اكثر من اللازم لظروف معينة... وهذا ال glucuurodinase يزيد ال enterohepatic circulation وهاي السيركليشن تمثل كمية البلروبين الي راح يرجع من الامعاء للكبد فمن تزيد هاي الدوره يعني راح يرجع بلروبين اكثر وبالتالي out of liver hand فيتراكم بالدم الخلاصه (حليب الام بي هوايه glucurdinase هذا يخلي البلروبين يرجع من الامعاء للكبد بكميات اكبر من المعتادة خلال دورة اسمها enterohepatic circulation وأيضاً هذا glucurdinase متهم بانه يساعد على deconjugation بالتالي يصير عدنا Unconj. Bilirubin اكثر هذا عادة يصير بالاسبوع الثاني من الولادة يعني الطفل عمرة (٧_١٠) ايام علاجة سهل وبسيط فقط توقف breast feeding تلث ايام اربعة بينما اللفر يصحصح وترجع الامور تمام وبعدين ترجعه للرضاعه الطبيعيه (ماراح يرجع جوندس) وما ننسة نسوي exclusion للهيمولتك كوزز

بالبداية، انا اسف لأن مجاي انشر هالفترة بس فعلا مشغول. ركزلي بهذا الكيس، جدا interesting جدا. 1 day old male newborn, delivered vaginally for diabetic mother on Insulin. المهم، اول ما جابوة صار عندة subtle seizure و تمت معالجتة ع انه hypoglycaemia (الأم الدايبتك البيبي مالها يصير عنده hypoglycaemia و راح نشرحها بفويس ليش)، بعد ٤ ساعات صارت عندة fit ثاني مرة (بالرغم ان مستوى السكر رجعوه للنورمل ب اول مرة)، هم رجعوا و انطوة هايبرتونك كلوكوز. بعد ساعتين صار عنده fit بسبب الهايبوكلاسيميا 😑😑. So, screening and seeking is started 😍😍. He has micro-penis and microcephaly. Abdominal examination revealed right hypochondrial mass. ف صار عدنا Hypoglycaemia, RUQ mass, micro penis, microcephaly. و انتبهوا ل رجلية، شوفوهن شبيهن، very characteristic. يمكن بعد ما تشوفون مثلها 😂😂.
