Interesting case to see . شاب عمرة ٢٦ سنة اجة للطوارىء بسبب Sudden onset dyspnea and limb paresis with palpitation بالرغم هو قبل ما عندة اي امراض مزمنة . ال ECG show sinus tachycardia. ال chest auscultation reveals bilateral crackles. ال BP نورمل، و ال RR نورمل . Neuro ex : show signs of lower motor neuron lesions bilaterally (power 3/5, hyporeflexia) . Pupil is constricted bilaterally . بعد ربع ساعة تقريبا، اخوة الي هو ال relative، ظهرت عندة اعراض مشابهه لحد ما . و الحالتين بقن يصير بيهن progression و خاصة ال dyspnea. الكيس اجاوبه بعد شوية بالليل، بس خل نسمع آرآكم . @AhmedAbdSambot

كيس مهم و نكتبة بالعربي حتى الكل يقرأة : قبل ساعة تقريباً، اجانه شاب عمرة ٢٥ سنة بسبب syncope. ما عندة أمراض مزمنه بس عندة مشكلة بعينة و ياخذلها علاج (ما يعرف شنو اسمة). تخطية كشف sinus tachy, ضغطة كان 90/40 بحالة ال supine. حرارتة مرتفعة قليل،لا عندة اسهال و لا تقيؤ خلال هالفترة. انطية لتر نورمل سلاين و ضغطة ما ارتفع. ١) اكو أشياء بالهستري ما ذكرتها لأن اريدكم انتم تسألون عنها ٢) اريد ع الاقل ddx مو الا تشخيص واحد، و شنو الي خلاك تفكر هجي. ٣) هنا دزوا الجواب @AhmedAbdSam، لأن اهواي رسائل بالبوت بعدني ما قاريها

Neonatal Physiological Jaundice 1. Criteria .. Appear After 24 hrs ..mainly unconjugated hyperbilirubinaemia. .. Peak at 4th day then decline. .. Not increase more than 5mg/dl/day. .. Not exceeds 12mg/dl (in preterm 15mg/dl) . .. Disapear at 10th day. 2) etiology : .. Short life span of RBC (90 day) .. Defect ligandin (which uptake unconjugated bilirubin into the liver) .. Defect in conjugation enzymes .. Under developed gut flora (impair conversion of direct bilirubin into stercobilinogen >> increase re-absorption). 3. Mx .. Follow it at 10-14 day to confirm its dispearance. Note : any deviation from above features, it is not physiological jaundice.

كل عام و انتم و الأمة الأسلامية بألف خير 3 patients presented to ER 1. The 1st one complain from limb weakness after eating excessive amounts of candies 2. The 2nd one, complain from limb weakness after eating fried rice. All his family complian from the same condition. 3. Third patient complain from sudden lower limb weakness, he could not wake up from the sleep, not Preceded by provakative factor. 4. The 4th pt is female, has previous hx of unexplained dysphasia, presented with sudden weakness and dyspnea. اريد منكم تشخيص كل حالة أو ع الاقل ddx, كل كيس منطيكم بيه كلمة تعتبر هي ال key لل dx. @AhmedAbdSambot
