وحده من أهم المشاكل الكمن الي نشوفها بالطوارئ هي Uncertain regular wide complex tachycardia WCT يعني WCT و ما تعرفه هل هو VT SVT with aberrancy (bundle branch block) Antidromic AVRT ركزت بالتخطيط و ما عرفته شنو نوعه، شلون راح نعالج. According to the 2020 update ESC guideline: اول ما نشوف البيشنت هل هو stable او unstable Unstable= any patient with signs and symptoms of hypoperfusion 1. Unstable 👉👉 check the pulse: A. Pulseless 👉👉 start with CPR (ACLS algorthim) B. Pulsatile 👉👉 check the R and T waves: .. If you can't differentiate bewteen the waves 👉👉 give asynchronized DC 120-200 J .. If you can differentiate bewteen them 👉👉 give synchronized DC (120-200 j) 2. Stable uncertain WCT 👉👉 check the morphology of QRS: A. Monomorphic 👉👉 start with vagal manruvers, if no response give adenosine 6 mg then 12 mg, and check the response: -- good response: SVT and Rx accordingly. -- no response 👉👉 VT B. Polymorphic 👉👉 think about SVT with accessory pathway and electrolyte disturbances Remember: irregular WCT 👉👉 always think about AF with aberrancy or with WPW شلون نفرق؟؟ شوف الـ QRS morphology A. Monomorphic 👉👉 AF with aberrancy, Rx with rate control drugs B. Polymorphic 👉👉 AF with WPW, give DC even if the pt was stable. C. U cannoy assess the morphology due to very fast rate (> 200 bpm) 👉👉 AF with WPW يمكن هذا أسهل diagram للـ WCT. اذا احد كدر يرتبه النه كـ Scheme او مخطط بعد أفصل. هذا اخر update نازل عن الـ Uncertain WCT #cardio

A nice case to be seen: فيميل hypertensive و جاي تاخذ lisinopril صار عدها New onset blisters in inter-mammary area and the oral cavity. Dx of ACEIs-induced pemphigus vulgaris was put in the list. Confirm dx by histological study + anti desmoglein (1 and 3) antibodies. Rx: 1. Stop ACEis 2. Give predinsolone 1 mg/kg/day 3. We may add azathioprine 2 mg/kg/day OR mycophenolate 1 g twice daily. #dermatology

الـ guideline كلش صريحة بيشنت عنده AVNRT و كان ستيبل نبدي وياه vagal maneuvers اذا ما استجاب، نرجع ننطي ادينوسين 6 mg then 12 mg then 12 mg (or 18 mg), between each other dose is 30 seconds to 1 min و الادينوسين ننطيه via proximal IV set، و الافضل هو left cubital line و من ننطيه ندفع وراه 20cc flush NS و نرفع ايد البيشنت ما تحسن ع الادينوسين، ننطي verapamil بجرعة 0.15mg/kg او 5-10 مغم، وريدي خلال دقيقتين. اذا ما تحسن نعيد الجرعه بعد 15 دقيقة، و ال max dose هي 20mg. اذا ماكو ريسبونس، ننطي betaloc (metoprolol) بدوز 5mg، و نكدر نعيدها كل 5 دقائق و المكزمم هو 15mg. اذا ماكو ريسبونس، الـ ESC guideline تكلك انطي DC shock. الـ AHA تكلك انطي IV amiodarone (و هذا يعتبر weak recommendation). الهدف من هذا البوست هو حتى اوضحلك ان الكوردارون مو good choice للـ AVNRT. #cardio @AASmedicalbot

Headache + Eye right pain + Vomiting + Red right eye with lacrimation + Decreased visual acuity 👉 Acute angle closure glaucoma is characterized by narrowing of anterior chamber angle leading to decreased aqueous outflow and elevated intra-ocular pressure.. يكون نسبته اكبر بالفيميل خصوصا اللي اكبر من 40 سنة اضافة ننتبهه للدركات اللي تعتبر predispose or trigger Anticholinergic + sympathomimetic 👉 pupillary dilation Sulfonamides 👉 swelling of structures (lens, retina, choroid) بعد مت ركزنا ع المضاد تبين هو trimethoprim اما Ddx 👉 Cluster headache يكون recurrent attacks خلال دقائق وكمون بالميل وخاصة young male اضافة 👉 Migraine it usually presents in adolescence or early adulthood and this patient red eye unlikely to be migraine 👉 Subarachnoid hemorrhage not commonly cause v red eye 👉 Giant cell arteritis vomiting is not typical of GA 👉 Optic neuriis seen as a manifestation of multiple sclerosis😁 اما التريتمنت topical drops 👉 pilocarpine one every 5 min for 15 min, then 30min or 0.5% timolol one Acetazolamide 👉 500 mg slowly IV then 250 mg IV analgesia 👉 morphine up to 2.5 mg IV antiemetic 👉 metoclopramide 10 mg IV

هلى ذكرى الـ phenytoin كذا مرة اشوفهم يخففونه بـ Glucose water 5%، لكن هذا الشيء مو صحيح. Phenytoin is compatible ONLY with 0.9% NS. Any other fluid isn’t compatible with phenytoin #pharma
