🛑مدرات البول من نوع Thiazide diuretics مثل: Hydrochlorothiazide Chlorthalidone قد تكشف وجود فرط نشاط جارات الدرق الأولي (Primary Hyperparathyroidism - PHPT) غير المشخّص. كيف يحدث ذلك؟ في الحالة الطبيعية: الكالسيوم يُرشّح عبر الكلية ثم يُعاد امتصاص جزء منه. جزء من الكالسيوم يخرج مع البول. عند استخدام الـ Thiazides: تزيد إعادة امتصاص الكالسيوم في الأنبوب البعيد (Distal convoluted tubule) في الكلية. يقل فقدان الكالسيوم في البول (↓ urinary calcium excretion). نتيجة لذلك يرتفع مستوى الكالسيوم في الدم قليلًا (mild hypercalcaemia). إذا كان لدى المريض PHPT خفي: تكون الغدد جارات الدرق تفرز كمية زائدة من PTH. الـ PTH يزيد إعادة امتصاص الكالسيوم ويحرّك الكالسيوم من العظام ويزيد امتصاصه من الأمعاء. عند إضافة Thiazide يحدث ارتفاع إضافي في الكالسيوم، فيظهر المرض الذي كان غير واضح. الخلاصة: Thiazide لا يسبب عادةً فرط كالسيوم شديد، لكنه قد يكشف (unmask) فرط نشاط جارات الدرق الأولي الموجود مسبقًا بسبب تقليل طرح الكالسيوم في البول.
