⚠️ Why should we avoid combining Topiramate with Valproate?

🔬 Mechanism: Both valproate and topiramate can contribute to the development of hyperammonemia. Their combination may increase the risk of elevated blood ammonia levels, which can lead to hyperammonemic encephalopathy.
🧠 Clinical manifestations may include: 🔹 Altered mental status or confusion
🔹 Lethargy and excessive drowsiness
🔹 Vomiting
🔹 Cognitive changes
🔹 In severe cases, encephalopathy or reduced consciousness
📌 Clinical Pearl: The combination of valproate and topiramate is associated with an increased risk of hyperammonemia, sometimes even when liver function tests are normal.
✅ Clinical Recommendation: Avoid this combination when possible, particularly in patients at increased risk. If concomitant use is necessary, monitor for symptoms of hyperammonemia and evaluate ammonia levels when clinically indicated.
🚨 Important: Unexplained lethargy, vomiting, or altered mental status in a patient receiving this combination should raise suspicion for hyperammonemic encephalopathy.

هل يُعدّ الحديد الوريدي IV iron أفضل من الحديد الفموي Oral iron أثناء الحمل؟
كلاهما فعّال في تعويض نقص الحديد واستعادة مخازنه، لكن يتميز
Intravenous (IV) iron
بقدرته على تعويض Iron stores ورفع مستوى Hemoglobin (Hb) بصورة أسرع.
وفقًا لتحليل (Meta-analysis) شملت 29
Randomized Controlled Trials, RCTs
وأكثر من 11,000 امرأة حامل، ارتبط استخدام IV iron بـ:
🔹 ارتفاع مستويات Maternal Hemoglobin (Hb) وتحسّن مخزون الحديد لدى الأم والوليد، ممثلًا بمستويات Ferritin.
🔹 انخفاض احتمالية الحاجة إلى Blood Transfusion خلال فترة التنويم المرتبطة بالولادة.
🔹 آثار جانبية أقل وأخف مقارنةً بالحديد الفموي، وفقًا لنتائج التحليل.

متى نفضّل IV iron أثناء الحمل؟
يُؤخذ الحديد الوريدي بعين الاعتبار بعد انتهاء First Trimester في الحالات التالية:
- عدم تحمّل Oral iron أو حدوث آثار جانبية تمنع الاستمرار عليه.
- وجود Severe Iron-Deficiency Anemia، خاصةً في المراحل المتأخرة من الحمل، عندما تكون الحاجة إلى تصحيح الأنيميا أسرع.
- عدم تحقيق استجابة كافية من العلاج الفموي، مع استمرار انخفاض Hb و/أو Ferritin رغم الاستخدام المناسب.

لا تتوفر بيانات كافية عن سلامة

خلال First Trimester؛ لذلك لا يُوصى باستخدامه روتينيًا خلال هذه الفترة. ويجب أن يستند اختيار العلاج إلى شدة الأنيميا، وعمر الحمل، وسبب نقص الحديد، والاستجابة للعلاج، مع مراعاة المستحضر المستخدم والبروتوكول المعتمد.
