#كيسات_الردهة ⚫️ الـ chemotherapy بي انواع خير من الله كل نوع لازم نعرف شلون نسويله monitoring وشنو يسوي adverse effects وشلون نتابعها وشلون نسويلها management ⚫️وحده من كيسات الردهه هي مريضه تشخصت على انها Molar pregnancy فحسبنا السكور مالتها فصار اللاين مال تريتمنت هو Methotrexate ⚫️ الـ Methotrexate يسويلنا هوايه مشاكل و وحده من المشاكل هي Mucositis فلازم دائما نتابع البيشنت لا يصير عدها هل شي لان ممكن نوصل انو نوقف ال Methotrexate عن البيشنت وهاي تصير بال Grade 4 Mucositis فأثناء متابعتنا لكيت البيشنت بده يصير عدها sloughing and mouth ulcer in the buccal .. وبدت تشتكي متكدر تاكل بس تشرب 🔴 فأهنا واحد من اللاين مال تريتمنت هو Magic Solution هو خبطه من عده مواد نحضرها بتراكيز معينة و نخلي البيشنت يتغرر بيها ( وفي بعض الحالات يتم بلعها حسب الـ Severity ) ونتابع وهكذا الحمدلله البيشنت كتبتلها Magic Solution مع المتابعه والاجراءات الاخرى تماثلت بالشفاء التام ( الصور مرفقة ) #Mohammed_Khader

مازال الاختلاف قائم على جماعه الـ acute Pancreatitis  انو نبلش وياهم feeding  لو نبقيم فاستنك ؟! وليش الـ new recommendation  صارت تمشي ويه الـ feeding  عكس ماكان سابقا ! 🤔 ⚫️ اول شي سابقا كان مريض الـ Pancreatitis نبقي فاستنك ومنبلش feeding  لحد ما جتي اخر الـ recommendation انو نبلش راسا feeding  طول ما البيشنت can tolerate  .. والسبب ؟ ↙️↙️ ⚫️ Prevention of Intestinal Barrier Dysfunction & Bacterial Translocation - Fasting leads to mucosal atrophy and increased intestinal permeability, allowing bacteria to translocate into the bloodstream, increasing the risk of infections and sepsis. So, Early enteral nutrition (EN) helps maintain gut integrity and reduces these complications. ⚫️ Better Clinical Outcomes & Reduced Need for Parenteral Nutrition ( shorter hospital stay ) - Early feeding decreases the need for total parenteral nutrition (TPN), which is associated with infection risks and metabolic complications (e.g., hyperglycemia). So, Studies show that patients receiving early oral or enteral feeding have shorter hospital stays and improved recovery rates. ⚫️ Reduction of Systemic Inflammatory Response Syndrome (SIRS) - Early enteral nutrition helps modulate the inflammatory response, reducing the risk of multi-organ failure. ⚫️ Minimal Stimulation of the Pancreas - Contrary to old beliefs, research indicates that enteral feeding does not significantly stimulate pancreatic enzyme secretion in a way that worsens inflammation, particularly when using nasojejunal or nasoduodenal feeding tubes. 🔴 Practical Recommendations - Oral feeding: Initiate as soon as tolerated by the patient. - Enteral feeding: If oral intake is not feasible, nasoenteric feeding (preferably nasojejunal) should be considered over parenteral nutrition. 🟣 These recommendations are supported by guidelines, including (  AGA, ASPEN, and IAP/APA ) #Mohammed_Khader

الـ endocrine عالم بحد ذاته ومن نلكي فد disease معين لابد نسرج على الاشياء الاخرى او الـ diseases الاخرى الي تكون related بهل المرض الي اكتشفنا .. ⚫️ مريض عنده signs & symptoms مال hypothyroidism و فعلا تشخص مؤخرا بالـ primary hypothyroidism وبده علاج على ال Levothyroxine طبيعي - بعد مرور فتره بسيطه البيشنت صار deteriorated وبدت اوضاعه تلاص هنا فكروا بعدم الاستجابه للعلاج وبدوا زيدون الجرعه وكذلك بدت حالته تسوء اكثر واكثر شنو السبب يا ترى ؟! ⚫️ اكو معلومه حلوه تكول ↙️↙️ - Any patient recently started on thyroxine whose symptoms worsen, needs exclusion of Addison’s disease .. - والسبب انو ↙️↙️ Levothyroxine ( thyroxine )) enhances Metabolic clearance of Cortisol which leads to exacerbation of the symptoms .. لان الـ Cortisol هو بلا شي قليل ويجي الـ Thyroxine ويقلله اكثر فتبدي تلاص عنده .. 🔴 وهنا تسويلة lab test وفعلا يطلع Addison’s disease مجرد تحتاج بالاول تبلش Corticosteroid ومن بعدها يلا تبلش Thyroxine وكلشي ينحل .. شيش كص سيندرم .. 28 سنه شاب اجاني خايف ايده هيج صارت فجاه (( مثل الصوره )) .. فال ddx الي اجه ابالي Erythema ab igne livedo reticularis اثناء اخذ الهستري من الشاب تبين انه يعمل في مطعم وبالاخص واكف على (( شيش الكص )) .. مع الهستري تبين انها erythema ab igne والي يكون واحد من اهم اسبابها هو repeated exposure to the heat .. وهذا ما يتوافق مع الهستري مال مريض وعمله .. سويت reassurance للشاب ونبهته على ضروره تقليل نسبه تعرضه للحراره .. ولازم يحاول يقلل هل شي او يلبس شي ويحمي ايده .. 🔴 بما انو حاليا احنا بالشتاء فأحيانا تصير بسبب الكعده الطويلة يم الصوبه فأسميها ( صوبه سندروم😂 ) #Mohammed_Khader

خلي نسولف اليوم شويه عن الـ Antibiotics  وشنو الـ Update الي صاير بيها .. كلامي اليوم حيكون من ↙️↙️ - Infectious Diseases Society of America 2024 Guidance ( 15 / 11 / 2024 ) ⚫️ مثل متعرفون سابقا كان الـ Aminoclycosides تستخدم للـ Hospital Acquired infection وبالاخص الـ Pseudomonas وايضا بالـ MDR Pathogens ⚫️ ومن ناحيه الـ Priority بالعائله نفسها كـ AntiPseudomonas فهي ↙️↙️ - Tobramycin > Amikacin > Gentamicin  ( طبعا مامن الممكن نستخدمها وحدها وانما تعتبر add on therapy بالاخص بالـ Chest infection ) 🔴 حاليا بأخر ابديت الـ Gentamicin  منكدر نسخدمه كـ AntiPseudomonas صار كله Resistance  ونشال من الكايدلاين بالاخص بالـ Hospital Acquired pneumonia  🔴 وبالنسبه للبقيه من نفس العائلة فصار الاتي انو الـ Tobramycin  مازال نكدر نستخدمه Regardless to the source of pathogens 🔴 اما بالنسبه للـ Amikacin  فصار only available for infections originating from urinary sources (susceptible ≤16 µg/mL) .. 🟤 إن شاء الله احاول اكمل بقية ال Guideline ونشوف شنو ال Update الي صاير بي .. #Mohammed_Khader

واخيرا وصلنا الـ IDSA Guideline 2024 شنو رأيكم اسوي ملخص عليه صايره بي تغييرات لطيفة ومهمه ..

نسولف اليوم عن شي مثير للجدل شويه والي هو الـ Vitamin D3 واخر الـ Update بخصوصه .. الي راح اذكره هو من - Endocrine Society Guideline 2024 .. فهسه نبلش .. ⚫️ السؤال الاول هل يحتاج نسوي سكرين او ندز تحليل للـ Normal people ؟! الكايدلاين يكول↙️↙️ - In the general population of adults (age range, 19–74), neither routine vitamin D supplementation nor routine testing of 25(OH)D levels are recommended ⚫️ طيب السؤال الثاني في حال دزينا تحليل وجتي النتيجه مثلا 20 راح نعتمد على التقسيم المتعارف عليه القديم ؟ والي هو ( sufficiency, insufficiency, deficiency ) لو شلون ؟! فالكايدلاين يكول ↙️↙️ - No longer endorses specific 25(OH)D levels to define vitamin D sufficiency, insufficiency, and deficiency .. بما معناه انو ال Deficiency الي نكولها للبيشنت ممكن اصلا هي نورمل بالحقيقه اله !! .. ⚫️طيب السؤال الثالث واحد مثلا Osteomalacia ودزيناه تحليل طلع الـ D3 جوه الـ 12 ( يعني يحتاج تريتمنت ) يا دوز راح انطي وشنفضل ؟! الكايدلاين يكول ↙️↙️ - When supplementation is given, low daily doses are recommended (rather than high doses given at intervals such as weekly or monthly), Because evidence suggests some adverse effects with intermittent high dosing. - Cholecalciferol 300000 iu, 100000 iu and 500000 iu should no longer be available in the market any more .. بما معناه من تريد تنطي D3 لا تروح وتنطي High dose ( بحجه عود الD3 حتى يصعد بشكل اسرع ، انطي daily dose راح تشتغل مثل ذيج وبطريقه امن .. ) #Mohammed_Khader
