ليش المريض الي عنده acute exacerbation of COPD ما نكدر ننطيه high flow oxygen ؟ اذا نرجع للفسيولوجي عدنا two receptors تنطي فيدباك لل respiratory center (حتى يزود او يقلل الrespiratory work) 1. Central chemoreceptors : - it’s sensitive to Co2 level and PH of CSF (High CO2 , Low PH : hyperventilation Low CO2 , high PH : hypoventelation) طبعا هي تتحسس الPH اكثر شئ ، والCO2 هو يسوي تغيرات بالPH - it’s fast and strong but it’s accommodating with time يعني هي تتحسس لأقل التغييرات بالPH level و CO2 و تحفز الrespiratory center بس اذا بقت هاي التغيرات لفترة طويلة راح تتأقلم وبعد ما تحفز الrespiratory center 2. Peripheral chemoreceptors: - It’s sensitive to PO2 (oxygen) , and less sensitive to CO2 and PH - It’s weaker than central but not accommodating with time يعني ما دام البيشنت عنده هايبوكسيا فهي تحفز ال respiratory center ⛔ Patient with COPD had chronic hypoxia and hypercapnia و بما انه الcentral chemoteceptors تتأقلم وي الchronic high level of CO2 فهذا المريض ب الrespiratory drive معتمد عال peripheral (الي تتحسس الهايبوكسيا) لان السنترال تعودت وطفت من زمان What happens If give high flow oxygen? - Correcting hypoxia too quickly → Peripheral chemoreceptors shut down → Reduced respiratory drive. Leads to: ✅ Hypoventilation ✅ CO₂ retention → Respiratory acidosis ✅ Confusion, respiratory depression, even death
