Pellagra is a nutritional deficiency disease caused by a lack of niacin (vitamin B₃) or its precursor tryptophan. It is classically described by the “3 Ds”:
1. Dermatitis
Symmetrical, rough, dark, and scaly rash.
Often occurs on sun-exposed areas (face, neck, arms, hands — “Casal’s necklace”).
2. Diarrhea
Inflammation of the digestive tract.
Can cause abdominal pain, nausea, vomiting, and chronic diarrhea.
3. Dementia
Neurological symptoms such as irritability, confusion, depression, memory loss, insomnia, hallucinations.
If untreated, a 4th D is added:

Alcoholism
Malabsorption syndromes (e.g., Crohn's disease)
Carcinoid syndrome
Hartnup disease
Certain medications (e.g., isoniazid)

Dietary history
Low blood niacin levels (if tested)

Nicotinamide 300–500 mg per day orally.
High-protein diet
Meat, eggs, fish, peanuts, dairy, legumen.
Treat underlying cause (e.g., alcoholism, malabsorption).
Prevention
Balanced diet rich in:
Meat, fish, poultry
Dairy products
Legumes
Whole grains Nuts and seeds.
