✅ مدیریت و درمان سوختگی‌های مینور ☑️ معیار سوختگی‌های مینور (قابل درمان سرپایی) ▫️عمق Superficial یا Partial Thickness ▫️سایز کوچک و Non-circumferential ▫️عدم آسیب استنشاقی یا الکتریکی ولتاژ بالا ▫️عدم نیاز به جراحی ▫️دست و پا: معمولاً مینور نیستند اما اگر کوچک باشند با دبریدمان و آموزش و فالوآپ دقیق می‌توان سرپایی درمان کرد. ▫️ هر درگیری مفصلی الزاماً سوختگی شدید محسوب نمی‌شود. ☑️ اقدامات اولیه ▪️حذف عوامل: ▫️خارج کردن فوری لباس، جواهرات (حلقه) و دبری‌ها ▪️خنک‌سازی: ▫️شستشو با آب لوله‌کشی یا سالین خنک (حدود ۱۲ درجه) به مدت ۳۰ دقیقه (موثر بر کاهش عمق سوختگی). ▫️روش جایگزین (برای جلوگیری از Maceration): استفاده از گاز خیس‌کرده با آب/سالین خنک روی زخم. ⚠️ استفاده از یخ یا آب‌-یخ ممنوع (افزایش درد و آسیب بافتی) ⚠️ در اطفال ریسک هیپوترمی مانیتور شود. ▪️شستشو: ▫️استفاده از آب لوله‌کشی و شوینده ملایم ▫️کلرهگزیدین بدون الکل نیز مجاز است. ⚠️ استفاده از بتادین ممنوع ☑️ کنترل درد: ▪️دارودرمانی: ▫️استامینوفن و NSAIDها (کافی برای اکثر موارد) ± اپیوئید (مثل اکسی‌کدون هر ۴-۶ ساعت). ▫️تجویز به صورت «سر ساعت» (نه PRN) در ابتدا. ▫️دوز Rescue: تجویز مسکن ۳۰ دقیقه قبل از تعویض پانسمان. ▪️اقدامات موضعی: ▫️کرم لیدوکائین/پیلوکارپین (برای دبریدمان). ▫️بالا نگه داشتن عضو (Elevation) برای کاهش تورم و درد. ☑️ مدیریت بافت مرده و تاول‌ها ▪️تاول‌های پاره شده: ▫️دبریدمان کامل پوست‌های نکروزه و شل تا حذف کامل اپیدرم مرده (کانون عفونت). ▪️تاول‌های سالم: ▫️کوچک (< ۲ سانتی‌متر): دستکاری نشود. ▫️بزرگ: مورد اختلاف؛ توصیه آپتودیت به عدم آسپیریشن به دلیل ریسک عفونت ▫️مقاوم (> چند هفته): ارجاع (احتمال Deep Partial/Full Thickness). ☑️ پانسمان و داروهای موضعی ▪️سوختگی‌های Superficial ▫️نیاز به آنتی‌بیوتیک موضعی یا پانسمان ندارد. ▫️تنها درمان: مرطوب‌کننده بدون عطر. ▪️سوختگی‌های Partial Thickness     ▫️نانوکریستال‌های نقره یا هیدروکلوئیدها     ▫️عسل مدیکال     ▫️آلوئه‌ورا: فاقد اویدنس برای اثربخشی     ▫️ سیلور سولفادیازین: تاخیر در ترمیم زخم، دردناک بودن تعویض     ⚠️ استروئید موضعی: ممنوع     ▫️پروفیلاکسی آنتی‌بیوتیک سیستمیک: جایگاهی ندارد (مگر عفونت واضح).     ▫️واکسیناسیون کزاز: در صورت سوختگی فراتر از سطحی مدنظر باشد ▫️تکنیک پانسمان: ۱. پماد آنتی‌بیوتیک/موضعی ۲. گاز غیرچسبنده (Non-adherent مثل گاز وازلینه) ۳. گاز خشک Fluffed جهت جذب ترشحات و محافظت (باز کردن تای گازهای استریل بسته‌بندی و مچاله کردن آن‌ها جهت ایجاد حجم) ۴. باند کشی ▫️نکات تعویض:     ▫️روزانه یک‌بار یا در صورت اشباع شدن زودتر.     ▫️قبل از برداشتن، پانسمان قبلی با آب کاملاً خیس شود تا درد کاهش یابد.     ▫️شستشوی ملایم دلمه و پماد قبلی (بدون ساییدن خشن). ▪️نواحی خاص ▫️صورت و دست (کوچک): روش باز (بدون پانسمان) + شستشو و پماد ۳-۲ بار در روز. ▫️انگشتان: حتماً پانسمان جداگانه هر انگشت (جلوگیری از چسبندگی و Maceration). ▫️ بیماران کاندید ارجاع: پانسمان صرفاً با گاز وازلینه و بدون پماد زیاد. ☑️ مراقبت‌های پس از اپی‌تلیالیزیشن (بسته شدن زخم) ▪️اقدامات: ▫️قطع پانسمان. ▫️استفاده از مرطوب‌کننده بدون عطر (وازلین/اوسرین) جهت بازگشت چربی پوست. ⚠️ محصولات حاوی لانولین و موم‌های سنگین ممنوع. ▪️کنترل خارش: ▫️آنتی‌هیستامین سیستمیک (دیفن‌هیدرامین)، لوسیون مرطوب‌کننده، لباس نخی گشاد. ☑️ سوختگی‌های دهان: ▫️لب و گوشه دهان: آنتی‌بیوتیک موضعی + وازلین مکرر (جلوگیری از چسبندگی). ▫️مخاط دهان: شستشو با سالین و رعایت بهداشت دهان. ⚠️ دهان‌شویه الکلی ممنوع. ☑️ فالوآپ و هشدارها ▪️زمان ویزیت: روز بعد از سوختگی، سپس هفته‌ای یک‌بار. ▪️علائم بهبود: نقاط سفید مرواریدی در بستر زخم (Epithelial buds). ⚠️ عدم مشاهده این نقاط بعد از 10-7 روز: احتمال سوختگی عمیق‌تر از ارزیابی اولیه و لزوم ارجاع ▪️اسکار: ▫️بهبود < ۳ هفته: ریسک اسکار پایین. ▫️بهبود > ۳ هفته: ریسک اسکار بالا: استفاده از شیت سیلیکونی/پانسمان فشاری ▪️ عفونت: ▫️هایپرمی نرمال: حاشیه قرمزی دقیقاً از مرزهای زخم پیروی می‌کند. ▫️سلولیت: قرمزی بیش از ۲ سانتی‌متر فراتر از لبه زخم گسترش می‌یابد و حاشیه‌ها نامشخص و به‌هم‌پیوسته است. ▫️عفونت می‌تواند سوختگی «درجه ۲ سطحی» را به «درجه ۲ عمقی» یا حتی «تمام ضخامت» تبدیل کند. ▫️تمام عفونت‌های مشکوک در سوختگی‌های Partial یا Full thickness نیازمند بستری + آنتی‌بیوتیک تزریقی هستند. ▫️کشت سطحی: بی‌ارزش است. ▫️بیوپسی Full-thickness: در صورت شکست درمان یا شک به مقاومت

☑️ مرور داروها و پمادهای رایج ▪️پماد موپیروسین: ▫️کاربرد: در صورت شک به کلونیزیشن با MRSA، سوختگی‌های صورت ▪️پمادهای چشمی (اریترومایسین/تتراسایکلین): ▫️کاربرد: سوختگی‌های اطراف چشم ▪️سیلور سولفادیازین (SSD): ▫️ویژگی: تسکین‌دهنده، کاهش کلونیزیشن زخم ▫️معایب:     ▫️ایجاد Pseudo-eschar و ریسک عفونت (لزوم دبریدمان)     ▫️کُند کردن روند اپی‌تلیالیزیشن     ▫️اثربخشی کم در سوختگی‌های وسیع (زیر ۵۰٪) ⚠️ منع مصرف: بارداری، شیردهی، شیرخواران زیر ۲ ماه، نزدیک چشم ▪️پانسمان‌های نانوکریستال نقره (Acticoat, Aquacel Ag, Mepilex Ag): ▫️مزایا: آزادسازی مداوم نقره، اثر ضدمیکروبی قوی‌تر، کاهش دفعات تعویض (هفتگی) ▪️مافنید استات: ▫️ویژگی: نفوذ عالی به بافت مرده (Eschar) و غضروف. ▫️کاربرد: سوختگی‌های گوش و بینی و سوختگی‌های تمام‌ضخامت. ▫️پوشش عالی روی ارگانیسم‌های MDR ⚠️ عوارض: دردناک بودن، ریسک اسیدوز متابولیک ▪️گاز پترولیوم آغشته به بیسموت (Xeroform): ▫️کاربرد: Donor site، سوختگی‌های سطحی کوچک اطفال ▫️مزیت: نچسب، بوگیر، نیاز به تعویض ندارد کلرهگزیدین: ▫️کاربرد: معمولاً همراه با گاز برای سوختگی‌های Superficial Partial Thickness ▫️مزیت: برخلاف SSD، با پروسه اپی‌تلیالیزیشن تداخل ایجاد نمی‌کند. عسل مدیکال: ▫️سرعت ترمیم بالاتر (حدود ۵ روز سریع‌تر از SSD) ▫️افزایش احتمال استریل شدن زخم‌های عفونی ⚠️ اثربخشی و ایمنی آن در سوختگی‌های وسیع یا شدید اثبات نشده است پوویدون آیوداین (Betadine): ▫️سمیت سلولی و تاخیر در ترمیم زخم ▫️نیاز به تعویض مکرر (۴ بار در روز) محلول داکین: ▫️طیف آنتی‌میکروبیال بسیار وسیع. ▫️سایتو پروتکتیو ▫️کاربرد: زخم‌های مقاوم

کلیات سوختگی و درمان‌های موضعی ☑️ طبقه‌بندی سوختگی‌ها ▪️سوختگی Superficial یا درجه ۱: ▫️درگیری: صرفاً اپیدرم ▫️علائم: قرمز، خشک، دردناک، سفید شدن با فشار (Blanching)، بدون تاول ▫️سیر بهبود: ۳ تا ۶ روز، بدون اسکار ▪️سوختگی Superficial Partial Thickness یا درجه ۲ سطحی: ▫️درگیری: اپیدرم + بخشی از درم ▫️علائم: ایجاد تاول (طی ۲۴ ساعت)، قرمز، مرطوب/ترشح‌دار، دردناک، سفید شدن با فشار ▫️سیر بهبود: ۷ تا ۲۱ روز، معمولاً بدون اسکار (احتمال تغییر پیگمانته) ▪️سوختگی Deep Partial Thickness یا درجه ۲ عمقی: ▫️علائم: درد فقط با فشار عمقی، ظاهر خشک/مومی (سفید تا قرمز)، تاول، سفید شدن کُند با فشار ▫️سیر بهبود: ۲ تا ۹ هفته، ریسک بالای اسکار هایپرتروفیک و اختلال عملکرد مفاصل ⚠️ اگر طی ۲ هفته ترمیم نشود، عملاً معادل تمام‌ضخامت تلقی می‌شود. ▪️سوختگی Full Thickness یا درجه ۳: ▫️درگیری: تمام لایه‌های پوست + بافت زیرجلدی ▫️علائم: بی‌حس/کم‌حس، ظاهر چرم‌مانند/زغالی/سفید مومی، خشک و غیرارتجاعی، عدم سفید شدن با فشار، کنده شدن آسان موها ⚠️ هرگز خودبه‌خود ترمیم نمی‌شود و نیاز به گرافت دارد. ▪️سوختگی درجه ۴: درگیری استخوان و عضلات و ... ☑️ تخمین وسعت سوختگی (TBSA) ▫️سوختگی‌های سطحی (درجه ۱) محاسبه نمی‌شوند. ▫️در موارد مشکوک بین Deep Partial و Full Thickness، جهت احتیاط تمام‌ضخامت در نظر گرفته شود. ▪️روش‌های محاسبه: ▫️چارت Lund-Browder: دقیق‌ترین روش (به‌ویژه برای اطفال) ▫️قانون نُه‌ها (Rule of Nines): مناسب بزرگسالان (سر ۹٪، هر دست ۹٪، هر پا ۱۸٪، تنه قدامی ۱۸٪، تنه خلفی ۱۸٪) ▫️روش کف دست (Palm Method): مناسب سوختگی‌های پراکنده/نامنظم     ▫️فقط کف دست بیمار = ۰.۵٪     ▫️کف دست + انگشتان = ۱٪ ☑️ کرایتریاهای ارجاع فوری ▪️آسیب‌های استنشاقی ▪️تمام سوختگی‌های Full Thickness ▪️سوختگی‌های Partial Thickness بالای ۱۰٪ TBSA ▪️درگیری نواحی خاص: صورت، دست‌ها، پاها، ژنیتالیا، پرینه و روی مفاصل اصلی ▪️سوختگی‌های الکتریکی، شیمیایی و رعدوبرق ▪️همراهی با تروما یا بیماری‌های زمینه‌ای ▪️اطفال زیر ۱۴ سال (یا زیر ۳۰ کیلوگرم) ▪️عدم کنترل درد بیمار ☑️ جایگاه آنتی‌بیوتیک سیستمیک ▪️در سوختگی‌های Superficial و بدون عفونت: ▫️هیچ نیازی به آنتی‌بیوتیک پروفیلاکسی سیستمیک نیست. ▪️در صورت بروز عفونت: ▫️نیاز به آنتی‌بیوتیک سیستمیک + احتمال بستری ☑️ انواع پانسمان و نکات کلیدی ۱. گاز خشک (Gauze): ⚠️ هرگز نباید به‌تنهایی استفاده شود. ▫️علت: باعث تشکیل دلمه می‌شود، جدا کردن آن سخت و بسیار دردناک است. ▫️روش صحیح: حتماً باید همراه با یک پماد یا عامل مرطوب‌کننده باشد. ۲. پانسمان‌های رایج (مشابه گاز وازلینه): ▫️شامل فیلم‌های نچسب (Non-adherent films) یا گازهای با مش ریز (Fine mesh gauze). ▫️معمولاً همراه با یک آنتی‌میکروبیال موضعی استفاده می‌شوند. ۳. پانسمان‌های نوین: ▫️شامل فوم‌ها، آلژینات‌ها، هیدروکلوئیدها و هیدروژل‌ها. ▫️انتخاب نوع آن بستگی به ویژگی‌های خاص زخم (مثل میزان ترشح) دارد. ☑️ توالی تعویض پانسمان ▪️فرکانس تعویض: ▫️بازه: از «۲ بار در روز» تا «هفته‌ای ۱ بار». ▫️معیار: باید به اندازه‌ای باشد که ترشحات کنترل شود، اما آن‌قدر زیاد نباشد که در روند Re-epithelialization اختلال ایجاد کند. ▪️نکته هنگام تعویض: ▫️در هر بار تعویض، بقایای آنتی‌میکروبیال موضعی قبلی باید به‌آرامی از روی پوست پاک شود و سپس لایه جدید استفاده گردد. ▫️سابیدن بیش‌از‌حد و دبریدمان با تیغ ضروری نیست.

Diagnosis of iron deficiency in adults

🔹Expected response 🔸Typical response ~ • resolution of symptoms(first few days of treatment), • a modest reticulocytosis (peaking in 7 to 10 days), • normalization of the hemoglobin level in six to eight weeks 🔸 Monitoring and hemoglobin/iron targets ~ 1. For Oral iron, we often re-evaluate the patient two weeks after starting. We check the hemoglobin and reticulocyte count and review tolerability of the oral iron. 2. For IV iron, we generally see patients four to eight weeks after the iron has been administered. 🔸Approaches to lack of response • nonadherence to oral iron, • ongoing blood loss, • incorrect initial diagnosis, • additional causes of anemia or iron deficiency. 🔺H. pylori, gastritis, celiac disease • These conditions are common causes of iron deficiency in individuals without a source of blood loss, and they can interfere with oral iron absorption (incomplete response to oral iron therapy). و نکته آخر! All clinicians administering IV iron should have adequate personnel, training, and equipment to manage extremely rare but potentially life-threatening adverse events, including medications to treat anaphylaxis, resuscitation equipment, and provisions for intensive care management. 🔸چند برند قرص آهن در ایران (دارویاب) ~ 1. فریرون (Feriron) که ارزانتره و برای کسی که مشکل گوارشی نداره بهتره. 2. فروفورت دئودنال (Ferro-Fort Duodenal) با عارضه گوارشی کمتر. 3. ففول (Fefol) که ترکیبی با فولیک اسیده 🔺درباره تزریقی هم ونوفر داریم و فرینجکت(کمتر توی بازار هست)، که توی متن توضیح دادیم.

🔹IV iron therapy (not IM)
1️⃣ Indications ~ • Unable to tolerate GI side effects of oral iron (eg. older adults), • Ongoing blood loss (eg. heavy uterine bleeding) • anemia is severe (eg, Hb <7 g/dL) or causing significant fatigue or other severe symptoms, • IBD, • CKD, • gastric surgery or other that interferes with absorption, (eg. celiac disease, Whipple's disease, bacterial overgrowth) • second and third trimesters of pregnancy, • other settings where more rapid correction of anemia is indicated, • Prefer to replete iron stores in one or two visits, • Poor adherence or gastrointestinal side effects of oral iron. 🔺We avoid IV iron in patients with active infections. 2️⃣ Dose and formulation • We often give a fixed dose of approximately 1000 mg 💉Iron sucrose ~ (IS; Venofer) ~ Amp 20mg/ml; 5ml equal 100mg elemental iron ~ max dose: to 200 to 300 mg ~ rate: 200mg in 200 cc N.S over 2 to 15 minutes~ a total of five doses in 2 weeks is common. 🔺A test dose (1.25 mL [25 mg] by slow IV push) is recommended if the patient has a history of drug allergies; otherwise, a test dose is not required. 💉Ferric carboxymaltose ~ FCM (Ferinject, Injectafer)~ expensive and less avilable in Iran~ Amp 50mg/1ml, 10ml equal 500mg elemental iron~ 🔺As with all formulations, we start slowly, observe for signs of allergy or infusion reaction, and if not seen, administer the balance over 15 minutes. 🔺Due to the risk of hypophosphatemia, we do not use FCM in IBD, bariatric surgery, excess alcohol, trauma or sepsis, diabetes, and impaired oral intake. 3️⃣ Adverse effects of IV ironAnaphylaxis is exceedingly rare. Nevertheless, clinicians should be prepared to treat anaphylaxis if it occurs.Infusion reactions are characterized by self-limiting fever, urticaria, palpitations, neck or back spasm, or arthralgias (in <1 percent). 4️⃣ Use of premedication~ not routinely 🔸For individuals with asthma, inflammatory rheumatic conditions, or multiple drug allergies: • We routinely premedication with 125 mg of methylprednisolone and an H2 blocker (eg, 20 mg of famotidine). • For patients with a history of inflammatory arthritis, we administer methylprednisolone, 125 mg intravenously, and prescribe a short course of prednisone (1 mg/kg per day orally for four days) 🔺We avoid diphenhydramine, as this may cause hypotension or symptoms that mimic a more severe allergic reaction. 🔺We do not use H1 blockers (antihistamines) to prevent (or treat) infusion reaction. 🔺We administer every IV iron product slowly at first and observe patients for infusion reactions. 🔺We warn patients (and infusion personnel) in advance of the infusion that they may experience selflimiting fever, arthralgias, and myalgias, either during the infusion or within 24 hours, and that this does not represent an allergic reaction or cause for panic. 🔺Misperceptions about risks • In a typical scenario, myalgias, including chest and back discomfort, may be mistakenly ascribed to anaphylaxis, prompting administration of antihistamines and/or epinephrine, which in turn causes tachycardia and/or hypotension, converting the minor reaction to a more serious event. 🔺While it is prudent to take all reactions seriously, we believe most reactions are due to infusional symptoms (rather than a true allergy) and should be managed as such. 5️⃣ Treatment of allergic and infusion reactions 🔸Transient fever, arthralgias, myalgias, or flushing without associated hypotension, tachypnea, tachycardia, wheezing, stridor, or periorbital edema: 1. We temporarily hold the infusion and observe the patient. 🔺We do not administer antihistamines or other medications, as these may worsen rather than improve symptoms. 2. If symptoms resolve, we complete the infusion (most common outcome). 2. If symptoms worsen, we treat as a more serious reaction. 2. If symptoms persist unchanged, we may administer a dose of methylprednisolone, wait 30 minutes, and reinitiate the iron infusion. 3. Patients can use NSAIDs after leaving the infusion facility if these symptoms progress.

✅ Treatment of iron deficiency and iron deficiency anemia in adults 🔺Whom to treat ~ • All patients to raise the ferritin to at least 50 ng/mL; (Regardless of whether symptoms are present). • An exception is when iron depletion is used therapeutically (eg, hereditary hemochromatosis, porphyria cutanea tarda, polycythemia vera) or in patients with mild, asymptomatic iron deficiency without anemia in whom the cause has been addressed. 🔺Role of RBC transfusion • for severe anemia, severe symptoms (myocardial ischemia), or hemodynamic compromise (not routinely). 🔹Oral iron therapy
1️⃣ Indications ~• For patient with uncomplicated iron deficiency who tolerate oral iron. 2️⃣ Dose and formulation (oral iron) • 60–110 mg elemental iron daily. • Efficacy is similar with daily or (or Monday, Wednesday, and Friday)Daily dosing (one dose per day). May be preferable for those who find it easier to remember taking a daily dose. • Every other day dosing May be preferable for those who wish to reduce gastrointestinal side effects. 🔺There is no value to taking >1 dose per day. 🔺We do not use enteric-coated or sustained-release capsules due to poor absorption. 💊Ferrous sulfate 🔸Daily dosing OR Every other day dosing (one dose on Monday, Wednesday, and Friday) • 325 mg tablet (contains 65 mg elemental iron per tablet). • 75 mg/mL oral solution (contains 15 mg elemental iron per mL). • 220 mg/5 mL oral elixir (contains 44 mg elemental iron per 5 mL). 3️⃣ Duration of therapy • the total duration of oral iron therapy for iron deficiency anemia is generally 3 to 6 months 4️⃣ Interventions to enhance absorption • preferred to taking iron supplements on an empty stomach (eg. in the morning); unless gastrointestinal side effects necessitate taking them with food. ⏬ Food ~ Iron should especially be taken separately (2 hours) from calcium-containing foods and beverages (milk), calcium supplements, cereals, dietary fiber, tea, coffee, and eggs. ⏬ Antacids – Iron should be given two hours before, or four hours after, ingestion of antacids. • Vitamin C ~ the effects may be small. Thus, the decision to use vitamin C should be individualized. 5️⃣ Side effects (oral iron) • Gastrointestinal side effects are extremely common • Patients may also be bothered by itching and by black/green or tarry stools that stain clothing or cause anxiety about bleeding. 6️⃣ Strategies to improve tolerability • Increasing the interval to every other day if not done already • Making dietary modifications (taking iron with food or milk), although this may reduce absorption. • Switching to a formulation with a lower amount of elemental iron. • Switching from a tablet to a liquid, for which it is easier to titrate the dose. • Use of a stool softener or bulk-forming laxative

▪️Serum ferritin~ • body iron stores • positive acute phase reactant that can increase independently of iron status in disorders associated with inflammation, infection, liver disease, heart failure, and malignancy or obesity. • A ferritin level below 30 to 50 ng/mL is considered diagnostic of iron deficiency regardless of the patient's underlying condition or hemoglobin concentration. • cutoff of <30 ng/mL for individuals who are asymptomatic (eg, identified by screening) and <50 ng/mL for individuals with any symptoms of iron deficiency. • Increasing CRP levels were associated with increasing ferritin levels. ▪️Serum iron~ • Serum iron alone is not a test for iron deficiency, • its utility is in calculating (TSAT), • fluctuate with iron-containing supplements and dietary, • Serum iron is low in iron deficiency and (ACD/AI), • low serum iron is not diagnostic of any condition ▪️Serum transferrin and total iron binding capacity (TIBC)~ • Transferrin is a circulating transport protein for iron. • increased in iron deficiency • negative acute phase reactant so decreased in ACD/AI ▪️Transferrin saturation (TSAT)~ • TSAT = (serum iron ÷ TIBC) x 100 • cutoff of <20 percent is generally used to screen for iron deficiency • Because the TSAT is a ratio,an increase in the serum iron (eg, due to hemolysis or recent ingestion of an iron tablet) can raise the value, even in an individual who has iron deficiency and an increased TIBC. ▪️Hepcidin~ • as a "lock-hormone" in the context of iron homeostasis. • reduces dietary iron absorption and iron release from stores, leading to decreased plasma iron levels. • decreased in iron deficiency and increased in ACD/AI 🔺Progressive iron depletion • Initially, iron stores can be totally depleted without causing anemia. This is reflected by low ferritin and TSAT, and elevated TIBC. 🔺Test interferenceRecent intake of iron-rich foods or of oral iron supplements (even in multiple vitamins) may affect serum iron and therefore TSAT. For this reason, fasting state is the most reliable for serum iron.Oral iron~ test after an overnight fast or at least five to nine hours after the last ingestion of iron containing foods or pills. Intravenous iron~ we typically wait and retest iron studies at the time of repeat CBC (eg, after approximately four weeks). * Ferritin concentration is not affected by food.
▪️Indications for referral to
▫️Hematologist (not indicated in the majority) if: • iron studies are inconclusive, • the diagnosis is unclear, or • the administration of intravenous iron is under consideration. ▫️Gastroenterologist if • source of gastrointestinal blood loss or malabsorption is suspected • there is suspicion for autoimmune gastritis or celiac disease 🔺Search for source of blood and iron loss — Iron deficiency almost always requires treatment, which includes iron administration and identification of the underlying cause, regardless of the severity of the deficiency and/or the presence of anemia. A history of antithrombotic medication (aspirin, anticoagulation) cannot be used as a rationale to forgo this evaluation.
▪️Screening (asymptomatic individuals)~ CBC plus iron studies method~ Annual screening for high risk patient
* all of the following (high risk), regardless of symptoms or the presence or absence of anemia: • All people who menstruate • pregnant • Athletes • Vegetarians • Regular blood donors • bleeding disorders or receiving an anticoagulant • Medications (anticoagulants, anti-inflammatory drugs, antiplatelet drugs, or proton pump inhibitors) • Individuals with a history of gastric surgery • Patients with chronic infections • Any patients undergoing major surgery • Patients at risk for reduced access to medical care (socioeconomically) • Older adults (eg, >65 years), esp with CKD or HF • Adolescents, due to increased iron requirements during growth spurts • pediatric populations ▫️Individuals at low risk or those who prefer not to be screened • males and postmenopausal females who are not in any at-risk category

✅Iron deficiency
🔺Clues in History and P/E:most of cases are asymptomatic(or non-specific symptoms) • Symptoms of anemia (fatigue, pica, restless leg syndrome, pagophagia) • fingernail change, brittle skin • Autoimmune gastritis or celiac disease • Heavy menses or prior pregnancy • GI bleeding or frequent blood donation 🔺lab: • low RBC count • mcv (nl or low), low mchc • high rdw • low retic count • high plt (esp in severe or longstanding cases) 🔺PBS: • hypochromia, poikilocytosis,microcytosis, anisocytosis and target cells. 🔺Diagnosis — *any one of the following findings in the appropriate clinical sitting: 1️⃣ Serum ferritin <30 ng/mL. Some individuals may be diagnosed with iron deficiency at higher ferritin levels, especially if symptomatic or having a chronic inflammatory state. 2️⃣ Transferrin saturation <20 percent, mostly used in patients for whom the ferritin is thought to be unreliable due to an inflammatory state.; (TSAT= Serum Iron/TIBC) 3️⃣ Anemia that resolves upon iron administration. ☝🏻administration of iron to an individual with thalassemia will worsen the existing iron overload commonly seen in this condition. Thus, it may be prudent to obtain iron studies to confirm the diagnosis even in cases where iron deficiency is considered extremely likely. 4️⃣ Absence of stainable iron in the bone marrow (providing that adequate staining controls are performed). 🔺Tips: 1. Mentzer index: Calculated as MCV/RBC. A value >13 suggests IDA, while <13 favors thalassemia trait. 2. Rule of 3: Hb/RBC ratio < 3 suggests thalassemia trait, while a ratio ≈ 3 is more consistent with IDA. 3. Low RBC count is characteristic of iron deficiency and increased RBC count is characteristic of thalassemia. A typical RBC count for a patient with a hemoglobin of 9 g/dL is approximately 3 million/microL 4. All patients with microcytic anemia– should have iron studies. [serum iron, (TIBC)/transferrin, serum ferritin(iron stores), with calculated (TSAT)]; even when the diagnosis of thalassemia or ACD/AI is likely (such as in an individual with a known family history of thalassemia). 5. CRP and ESR do not distinguish between IDA and anemia of chronic disease, but their elevation should prompt caution in interpreting ferritin and iron studies. 6. Low MCV and low MCHC may be absent in early or mild iron deficiency, and can be masked by coexisting macrocytic processes or combined nutrient deficiencies. ❗️ACD/AI Mild microcytosis(eg. 75-80) with low iron, low TIBC; high-normal to high ferritin in the appropriate clinical context (eg, chronic inflammatory condition with normal MCV prior to its development) is consistent with ACD/AI.

❗️دلیل علمی: کاهش تولید ترانسفرین (TIBC) به عنوان واکنش‌گر فاز حاد منفی/ افزایش فریتین به عنوان واکنش‌گر فاز حاد مثبت در شرایط التهاب و بیماری مزمن.
اگر فرض کنیم آهن سرم «مسافران» (که در خیابان در حال جابه‌جایی‌اند)، ترانسفرین/TIBC: «تاکسی‌ها» (ظرفیت ناوگان حمل‌ونقل برای سوار کردن مسافر)، فریتین: «هتل‌ها» (محل ذخیره و استراحت مسافران) و درصد اشباع (Saturation):«شلوغی داخل تاکسی‌ها» (چقدر از صندلی‌ها پر شده است) باشد. - در فقر آهن (Iron Deficiency): وضعیت شهر «قحطی مسافر» است. هتل‌ها (فریتین) کاملاً خالی شده‌اند. سازمان تاکسیرانی برای جبران، با ناامیدی تمام تاکسی‌های موجود را به خیابان می‌فرستد (TIBC بالا)، اما چون مسافری نیست، اکثر تاکسی‌ها خالی می‌چرخند (اشباع بسیار پایین). - در آنمی التهاب/مزمن (ACD): ووضعیت شهر: «حکومت نظامی» در اینجا آهن وجود دارد، اما به خاطر شرایط جنگی (عفونت، سرطان یا بیماری خودایمنی)، بدن احساس خطر کرده و سیستم را قفل می‌کند (نقش هپسیدین).. مسافران هستند اما در هتل‌ها حبس شده‌اند تا آسیب نبینند (فریتین نرمال یا بالا). سازمان تاکسیرانی هم به خاطر شرایط امنیتی، تعداد تاکسی‌ها را کم کرده و به پارکینگ برگردانده است (TIBC پایین). هم مسافر در خیابان کم است (آهن سرم پایین) و هم تعداد تاکسی‌ها کم شده است (TIBC پایین).

Summary of diagnosis

سلام. قبل از اینکه مبحث بعدی رو بذاریم، میخواستم درباره‌ی این نکته که " آیا متن آپتودیت طولانیه؟ " صحبت کنیم (سوال و بحث تکراری). اول اینکه این چیزی که میگم اصلا دلیل بر این نمیشه که متن اصلی رو نخونیم و فقط سامری رو بخونیم؛ اما خب راهکار برای مواقعی که حجم یه مطلبی زیاده و یا میخوایم سریعتر به جواب برسیمه. قبلا هم توضیح دادیم؛ اما این‌بار با مثال جلو میریم. مبحث بعدی آنمی فقر آهن دوتا تاپیک درنظر داریم که با خروجی pdf حدودا ۶۰ص میشه. یکی دیگر از معجزات آپتودیت، بخش" Summary and recommendations و graphics " هست. توی مثال بعدی قراره فقط همین دو مورد (بخشی از تصاویر) رو مرور کنیم، ببینیم آیا به هدفی که داشتیم میرسیم؟(غربالگری- تشخیص- درمان- فالوآپ بیماران آنمی فقر آهن). پس پلن میشه خواندن summary و بعدش مرور تصاویر. در ادامه‌ش اگر وقت کردیم هم متن اصلی. اما این‌بار سرعت خواندن متن اصلی بیشتر میشه چون insight داریم بر مبحث.
