Anemia evaluation in outpatients (nonpregnant adults).

⏭️Normocytic (MCV; 80 to 100 fl)
🔺Clues • most common finding in anemic adult males and postmenopausal females. • Causes are more numerous and may be multifactorial. 🔺CBC, lab • Often associated with a slightly elevated RDW, and the reticulocyte count is not substantially increased (and may be decreased). • increased RDW may indicate a population of microcytic or macrocytic RBCs that is too small to shift the MCV out of the normal range, OR combined microcytic and macrocytic processes, such as iron deficiency plus vitamin B12 or folate deficiency. 🔺Causes 1. Nutrient deficiency – Any of the causes of acquired microcytic or macrocytic anemia, especially early stages of deficiency of iron, vitamin B12, folate, or copper. 2. Multiple causes – Combined microcytic plus macrocytic anemia. The most characteristic situation is simultaneous deficiency of vitamin B12 and iron in an individual with celiac disease or autoimmune gastritis. • Hemolytic anemia without marked reticulocytosis (due to concomitant bone marrow suppression from another cause). 3. ACD/AI 4. CKD – Anemia of chronic kidney disease (CKD). 5. HF – Anemia of heart failure (HF), including cardio-renal syndrome. 6. Endocrine – Anemia with endocrine deficiency, including hypothyroidism (most cases), androgen deficiency, or adrenal insufficiency (in adrenal insufficiency, anemia may be masked by volume contraction). 7. Cancer – Cancer-associated anemia, including monoclonal gammopathies. * Multiple myeloma (MM): Common presentations include anemia, bone pain, elevated creatinine or serum protein, fatigue, and hypercalcemia. 8. Clonal hematopoietic stem cell disorders – Anemia due to a clonal disorder of erythropoiesis (myelodysplastic syndrome, aplastic anemia, or clonal cytopenias of uncertain significance [CCUS]. 9. Early blood loss – Blood loss that has not yet caused iron deficiency. 10. PRCA – Pure red cell aplasia. 11. Partially treated anemia – Anemia in process of correction or following transfusion. A "dimorphic RBC population" (presence of two distinct populations of RBCs of different sizes) may be suspected when an increased RDW is present; it can be confirmed by examination of the peripheral blood smear, although the distinct populations may not always be recognized. it is more commonly seen during early treatment of iron deficiency or megaloblastic anemia, or following transfusion of a patient with microcytic or macrocytic anemia. 12. Copper deficiency/zinc poisoning. 🔺Evaluation 1️⃣Reticulocyte count and chemistry panel(to assess for renal or hepatic dysfunction) 2️⃣Iron studies and hemolysis labs 3️⃣Additional tests – (If hemolysis is absent and there are no other obvious diagnoses); including cancer, endocrine disorders, blood loss, and nutrient deficiencies. • Evaluation for disorders common in older adults is generally reasonable, including testing for monoclonal gammopathies, clonal cytopenias, androgen deficiency (in men), and bone marrow evaluation for myelodysplasia and pure red cell aplasia.

▪️Myelodysplastic syndromes (MDS)
🔺When to suspect • median age is 70 years old. • MDS should be considered in any patient with persistent (>4months) unexplained cytopenia(s) or clinical symptoms associated with anemia, infections(neutropenia), or bleeding/bruising(thrombocytopenia); morphologic dysplasia of blood or marrow; or unexplained bone marrow failure. 🔺CBC, lab 1. Isolated anemia is common, whereas isolated neutropenia, thrombocytopenia, or monocytosis without anemia is infrequent. 2. (MCV) is usually normal or macrocytic (>100 femtoL) 3. (RDW) is often increased 4. inappropriately low reticulocyte response 5. Approximately one-half of patients with MDS have a reduced total white blood cell (WBC) count, usually resulting from absolute neutropenia. 6. Variable levels of thrombocytopenia are present. 🔺PBS 1. RBC: Macro-ovalocytosis is the most common morphologic abnormality;but elliptocytes, teardrop cells, stomatocytes, or acanthocytes (spur cells) may be seen. 2. WBC: hypogranular and hypolobulated neutrophils (pseudo-Pelger–Huët anomaly). 3. Plt:Platelets are usually morphologically normal, Megakaryocytic fragments are not seen. 🔺Tips 1. the macrocytosis is usually mild to moderate (e.g., 100-115 fL). An MCV > 120 fL is rare in MDS. ▫️Copper deficiency 1. It is frequently misdiagnosed as MDS. Always check copper levels in a patient with unexplained cytopenias (especially anemia + neutropenia) before making an MDS diagnosis. 2. The anemia is refractory to iron therapy. 3. Look for a history of bariatric surgery (especially gastric bypass), malabsorption (e.g., celiac disease), or excess zinc supplementation. 4. The anemia can be microcytic, normocytic, or even macrocytic. The most important clue on the CBC is often not the MCV, but the presence of neutropenia (low neutrophil count). Thrombocytopenia (low platelets) can also occur.
✅General approach to the MCV>100, with or without anemia.
1️⃣features that need urgent attention on cbc or pbs; refer to a hematologist for bone marrow and/or molecular (DNA) studies on bone marrow or peripheral blood. • ANC <1000 • PLT <50.000 • immature wbc 2️⃣three most common causes 1. B12/ folate deficiency 🔺some clues: MCV >115, low wbc or plt, neurologic finding, strick vegan, multilobar pmn; then check B12. 🔺In patients without known nutritional/gastric issues who have MCV >110 fL and a normal vitamin B12 level, serum methylmalonic acid (MMA) and serum folate should be measured. 2. hemolysis 🔺some clues: polychromasia, high retic count, spherocytes; then check LDH, bilirubin, haptoglobin. 3. alcohol 3️⃣Individuals with normal vitamin B12 and folate; we check the following: • TSH • Serum copper level, especially if neutropenia and/or neuropathy are present or if the history reveals zinc ingestion or other risk factors. • liver synthetic tests; If the blood smear shows target cells, The MCV in liver disease typically is not >105 fL. Other morphologic abnormalities such as stomatocytosis should be evaluated, if present. • HIV infection • medications 4️⃣If the copper level is normal and the blood smear shows evidence of dysplasia such as bilobed or immature neutrophils or binucleate RBCs, or other cytopenias, refer to a hematologist for bone marrow and/or molecular (DNA) studies on bone marrow or peripheral blood.

⏫Macrocytosis (MCV >100 fL) ▪️Megaloblastic anemias (B12 or folate deficiency)
🔺When to suspect • Unexplained anemia, macrocytosis [MCV] >100 fL) , pancytopenia, or hypersegmented neutrophils. • Certain autoimmune disorders such as thyroiditis or vitiligo, or those taking chronic metformin therapy. • Strict vegan diet or conditions that may interfere with absorption • gastrointestinal features, IBD, celiac, gastritis. • Medications associated with lower vitamin B12 levels, including metformin and ppis or H2blokers(eg, cimetidine). • Gastrointestinal symptoms such as sore tongue, anorexia, or diarrhea. • Unexplained neurologic or psychiatric symptoms. 🔺CBC, lab • Anemia, [MCV] >100 fL • Mild leukopenia and/or thrombocytopenia • Low reticulocyte count 🔺PBSHypersegmented neutrophils and macro-ovalocytosison which are highly sensitive and specific findings. 🔺Tips 1. Severe macrocytosis (MCV >110–115 fL) is almost exclusively associated with megaloblastic anemia; While macrocytosis (MCV >100 fL) can be seen in a variety of conditions—including liver disease, alcoholism, hypothyroidism, mds, and reticulocytosis. 2. In the absence of these gastrointestinal abnormalities in individuals who are eating a healthy diet that includes food fortified with folic acid, and in the absence of other reasons to suspect folate deficiency, we only test for vitamin B12 deficiency. 3. normal MCV; when a megaloblastic anemia (which raises MCV) coexists with a microcytic anemia like iron deficiency or thalassemia trait (which lowers MCV); check RDW and PBS. Do not rule out B12/folate deficiency based on a normal MCV if the RDW is high or the smear is suspicious. 4. RDW can be used to distinguish between these conditions by recognizing that a very high RDW (often >18%) in the context of anemia suggests mixed red cell populations, which is a clue for dual deficiencies (e.g., B12 and iron), whereas a moderately elevated RDW (typically 15–18%) is more characteristic of isolated iron deficiency anemia

▪️Thalassemia
🔺When to suspect 1. Thalassemia may be suspected at any age with symptomatic or unexplained microcytic anemia, classical examination findings, positive family history, or positive newborn screen. 2. A negative family history does not eliminate the diagnosis. 3. mild microcytosis anemia or microcytosis without anemia. 🔺CBC and lab 1. Microcytic, hypochromic anemia is typical in all thalassemia syndromes except asymptomatic carriers. 2. High RBC count; mildly increased reticulocyte count. 3. Thalassemia causes non-immune hemolysis, with high LDH, high indirect bilirubin, low haptoglobin, and negative Coombs testing. 4. Nl wbc and plt 🔺PBS 1. The common findings of thalassemia in peripheral blood smear are microcytosis, hypochromia, anisocytosis, poikilocytosis (including teardrop and elongated cells), and target cells. 🔺Tips 1. Initial testing includes complete blood count (CBC), blood smear review, hemolysis testing, and iron studies (at minimum, ferritin). 2. Iron stores should be tested even when the diagnosis of thalassemia is likely, such as in an individual with a known family history of thalassemia and no evidence for dietary deficiency. 3. caveats for protein-based test: transfusion within 3-4 months ago, iron deficiency . 4. Mentzer index: Calculated as MCV/RBC. A value >13 suggests iron deficiency anemia, while <13 favors thalassemia trait. 5. rule of 3: Hb/RBC ratio < 3 suggests thalassemia trait, while a ratio ≈ 3 is more consistent with iron deficiency anemia.
▪️Other rare causes of Microcytosis (mcv<80):
🔺Severe microcytosis (MCV < 70 fL) is more typical of thalassemia or severe iron deficiency, not these rare causes. ▫️Lead poisoning 1. Manifestations of acute toxicity may include abdominal pain ("lead colic"), joint/muscle aches, fatigue, decreased libido, headaches, difficulty concentrating, short-term memory deficits, and irritability. 2. The most classic—though not always present—finding is coarse basophilic stippling of the red blood cells. 3. lead poisoning is a sideroblastic process, so iron studies will show a high serum iron, high ferritin, and high transferrin saturation. ▫️Copper deficiency ▫️Zinc deficiency ▫️Alcohol ▫️Certain drugs

▪️Chronic disease/anemia of inflammation [ACD/AI]
🔺CBC, lab 1. It is typically mild to moderate anemia (Hb of 10 to 11 g/dL or 8 to 10 g/dL respectively). 2. The RBCs are normocytic and normochromic in the majority of cases; nl MCV and MCHC. 3. Anemia of chronic disease(isolated) is unlikely to be present when hemoglobin is severely reduced (e.g., <8 g/dL) or when MCV is markedly abnormal(<70); it is unusual and should prompt consideration of alternative or coexisting etiologies, such as iron deficiency anemia or thalassemia. 3. Low reticulocyte count (or inappropriately low for the degree of anemia). 4. Elevated inflammatory markers such ESR, CRP. ‼️anemia can confound ESR interpretation, but marked ESR elevation (>100 mm/hr) should prompt evaluation for infection, autoimmune disease, and malignancy (regardless of anemia status). . 5. Low serum iron (generally <60 mcg/dL), Normal to low serum transferrin (generally <300 mcg/dL), Low transferrin saturation (TSAT; generally <20 percent), Normal to increased serum ferritin (>100 mcg/L). 5. serum ferritin is the most specific marker for iron deficiency, but it is an acute-phase reactant and can be falsely elevated in the presence of inflammation, infection, liver disease, or malignancy; TSAT <20% is the most reliable threshold for diagnosing iron deficiency anemia in chronic inflammatory conditions, with ferritin <100 μg/L as a supportive criterion in this setting. 🔺PBS: • The blood smear shows a relatively uniform population of normal-sized RBCs without evidence of hemolysis (ie, no polychromatophilia, spherocytosis, or RBC fragments). 🔺Tips 1. In most cases, We generally exclude the following causes of anemia with the following testing, prior to the consideration of ACD/AI as the only diagnosis; • Iron deficiency anemia • Hemolysis – Haptoglobin, lactate dehydrogenase (LDH), and/or bilirubin. • Decreased kidney function – Serum creatinine and estimated glomerular filtration rate (eGFR). • Liver disease – Serum transaminases and other liver function tests. • Vitamin deficiencies – Vitamin B12, vitamin D, and folate levels. • Endocrine disorders – Hormone levels (eg, thyroid-stimulating hormone [TSH] and cortisol) • Malignancy – Age-appropriate cancer screening; evaluation of abnormal findings on history, physical examination, or laboratory testing; if applicable, assessment of associated clonal or genetic markers when systemic hematologic malignancies including myelodysplastic syndrome (MDS) are suspected.

🔹Evaluation based on MCVMicrocytosis (MCV <80 fL) ▪️Iron deficiency
🔺Clues in History and P/E: • Symptoms of anemia(fatigue, pica, restless leg syndrome, pagophagia); most of cases are asymptomatic(or non-specific symptoms) • fingernail change, brittle skin • Autoimmune gastritis or celiac disease • Heavy menses or prior pregnancy • GI bleeding or frequent blood donation 🔺lab: • low RBC count • mcv(nl or low), low mchc • high rdw • low retic count • high plt(esp in severe or longstanding cases) 🔺PBS: microcytosis, hypochromia, anisocytosis, and poikilocytosis are common findings on pbs. - while fish-shaped red blood cells, elliptocytes, and tailed poikilocytes are less common and usually indicate more severe disease. 🔺Diagnosis — any one of the following findings in the appropriate clinical sitting: • Serum ferritin <30 ng/mL. Some individuals may be diagnosed with iron deficiency at higher ferritin levels, especially if symptomatic or having a chronic inflammatory state. • Transferrin saturation <20 percent, mostly used in patients for whom the ferritin is thought to be unreliable due to an inflammatory state. • Anemia that resolves upon iron administration. • Absence of stainable iron in the bone marrow (providing that adequate staining controls are performed). 🔺Tips: 1. Mentzer index: Calculated as MCV/RBC. A value >13 suggests iron deficiency anemia, while <13 favors thalassemia trait. 2. rule of 3: Hb/RBC ratio < 3 suggests thalassemia trait, while a ratio ≈ 3 is more consistent with iron deficiency anemia. 3. All patients – All patients with microcytic anemia should have serum iron, (TIBC)/transferrin, serum ferritin, with calculated (TSAT). 4. CRP and ESR do not distinguish between iron deficiency anemia and anemia of chronic disease, but their elevation should prompt caution in interpreting ferritin and iron studies. 5. Low MCV and low MCHC may be absent in early or mild iron deficiency, and can be masked by coexisting macrocytic processes or combined nutrient deficiencies. 6. serum ferritin is the most specific marker for iron deficiency, but it is an acute-phase reactant and can be falsely elevated in the presence of inflammation, infection, liver disease, or malignancy; TSAT <20% is the most reliable threshold for diagnosing iron deficiency anemia in chronic inflammatory conditions, with ferritin <100 μg/L as a supportive criterion in this setting. ❗️Mild microcytosis(eg. 75-80) with low iron, low TIBC; high-normal to high ferritin in the appropriate clinical context (eg, chronic inflammatory condition with normal MCV prior to its development) is consistent with ACD/AI. 7. Recent intake of iron-rich foods or of oral iron supplements (even in multiple vitamins) may affect serum iron and therefore TSAT. For this reason, testing obtained in a fasting state is the most reliable. Ferritin concentration is not affected by food.

In the steady state, reticulocytes generally spend three days in the bone marrow and one day in the circulation. In severe anemia, reticulocytes can leave the bone marrow earlier and can circulate for two to three days,

▪️When to refer to hematology
1️⃣Hematologic emergency • Severe pancytopenia • Blasts or immature myeloid/lymphoid forms • Pancytopenia with hemolysis, thrombosis, or bleeding • Microangiopathic hemolytic anemia with schistocytes on the blood smear. 2️⃣Diagnostic uncertainty • Anemia with normal or inconclusive laboratory testing • Pancytopenia or bicytopenia in an older individual with normal vitamin B12, folate, and copper levels. 3️⃣Need for specialized evaluation • Abnormal lymphocytes (hairy cells, large granular lymphocytes, prolymphocytes) • Leukoerythroblastosis with or without teardrop cells (dacrocytes) Anemia with leukocytosis not explained by infection or thrombocytosis not explained by iron deficiency or reactive process • Familial or hereditary anemia 4️⃣Need for specialized treatment • Iron deficiency that is severe, not responsive to oral iron, occurring in the late second or third trimester of pregnancy, or for which oral iron is not tolerated or would not be absorbed. • Hemolytic anemia, including microangiopathic hemolysis • Suspected leukemia or myelodysplastic syndrome.

7️⃣Pioglitazone (Thiazolidinediones) 🔸اشکال موجود در ایران • قرص پیوگلیتازون 15 ،30 و 45 میلی گرم • ترکیبی پیوگلیتازون + متفورمین
🔺دوز توصیه شده به صورت کلی در دیابت تایپ دو: شروع با دوز روزانه 15 میلی گرم، افزایش 15 میلی گرم به دوز قبلی هر 4تا12 هفته(در صورت نیاز)؛ ماکزیمم دوز: 45 میلی گرم روزانه./ با یا بدون غذا
🔸مکانیسم اثر: • افزایش حساسیت انسولینی، کاهش قند خون ناشتا و پس از غذا • توانایی کاهش A1C: حدود 1.4-0.5 درصد 🔸نکات دارویی • به عنوان درمان کمکی یا درمان جایگزین در بیمارانی که به متفورمین جواب ندادند(یا متفورمین کنترااندیکاسیون داشته). • عدم نیاز به دوز ادجاست در کلیوی • اغلب به عنوان خط دوم یا سوم به بقیه درمانها add میشه • موثر در MASH • موثر برای افرادی که در معرض خطر هایپوگلایسمی هستند • باعث افزایش وزن میشود 🔺منع مصرف در: • نارسایی قلبی NYHA class III–IV (به علت احتباس مایع) • ادم شدید یا سابقه CHF • سرطان مثانه‌ی فعال یا سابقهٔ آن • افزایش واضح آنزیم‌های کبدی (ALT > 2.5× ULN) • بارداری • هماچوری ماکروسکوپیک بررسی نشده(uninvestigated) • ادم ماکولار 🔸مواردی که باید در طول درمان پایش شوند: • قند خون ناشتا و HbA1c ( حداقل 2 بار در سال، و یا هر سه ماه برای کسانی که به هدف درمانی نرسیده باشند) • مانیتورینگ دقیق برای علایم نارسایی قلبی (وزن گیری و علائم ادم، تنگی نفس) بخصوص در شروع/تغییر دوز دارو و بیماران با NYHA I or II • عملکرد کبد (َAST, ALT, ALP, total billirubin) قبل از شروع(ادامه مانیتورنیگ در در بیماری که برای آسیب های کبدی high risk باشد). • پایش weight gain • پایش علایم ادراری و کنسر مثانه • معاینات چشم پزشکی دوره ای • پایش تراکم استخوان ( بخصوص در در خانم‌های یائسه و افراد high risk به‌دلیل افزایش خطر شکستگی). 8️⃣Gliclazide (Sulfonylurea) 🔸اشکال موجود در ایران • قرص گلی کلازید 30و 60 میلی گرمی(ER)/ دوز 30میلی گرم از ER تقریبا معادل با 80 میلی گرم از IR میباشد.
🔺دوز توصیه شده به صورت کلی در دیابت تایپ دو: شروع با دوز روزانه 30 میلی گرم(از نوع ER)، افزایش 15 میلی گرم به دوز قبلی هر 4-1 هفته(در صورت نیاز)؛ ماکزیمم دوز: 120 میلی گرم روزانه./ همراه با اولین وعده غذایی
🔸مکانیسم اثر: • تحریک ترشح انسولین (مستقل از سطح گلوکز خون/ کاهش قند خون ناشتا و پس از غذا • توانایی کاهش A1C: حدود 2-1 درصد 🔸نکات دارویی • به عنوان درمان کمکی یا درمان جایگزین در بیمارانی که به متفورمین جواب ندادند(یا متفورمین کنترااندیکاسیون داشته). • ریسک بیشتر برای هایپوگلایسمی نسبت به بقیه دسته داروها • ریسک افزایش وزن • به‌ندرت خط اول یا دوم محسوب می‌شود، مگر در شرایط خاص (محدودیت مالی یا عدم تحمل سایر داروها و عدم CVD). 🔺منع مصرف در: • حساسیت شدید • دیابت تایپ یک • در DKA • بارداری • شیردهی • شرایط استرس(تروما، عفونت شدید، جراحی) • منع مصرف در آسیب کبدی شدید • در eGFR< 15 توصیه نمیشود. 🔸مواردی که باید در طول درمان پایش شوند: • قند خون ناشتا و HbA1c ( حداقل 2 بار در سال، و یا هر سه ماه برای کسانی که به هدف درمانی نرسیده باشند) • مانیتورینگ کبدی و کلیوی قبل و حین درمان • مانیتور برای علایم هایپوگلایسمی • وزن بدن (ریسک افزایش وزن) 9️⃣Glibenclamide or Glyburide /(Sulfonylurea)
🔺اگر خواستیم از این دسته داروها (Sulfonylurea) بدهیم: • اولویت با داروهای کوتاه‌اثر یا با ریسک کمتر هیپوگلیسمی مثل گلیکلازید • اجتناب از گلیبوراید/گلیبنکلامید (ریسک هیپو بالا، مخصوصاً سالمندان)

3️⃣ Dapagliflozin (SGLT2 inhibitor) 🔸همان موارد امپاگلیفلوزین به علاوه نکات زیر: 🔸اشکال موجود در ایران • قرص داپاگلیفلوزین 5 و 10 میلی گرم. • قرص ترکیبی داپاگلیفلوزین + متفورمین
🔺دوز توصیه شده به صورت کلی در دیابت تایپ دو: شروع با دوز روزانه 5 میلی گرم، و افزایش تا 10 میلی گرم روزانه، بعد از 4-12هفته(درصورت نیاز)/ صبح ها با یا بدون غذا.
• در HF و CKD دوز روزانه 5 میلی گرم توصیه میشود. • در eGFR<45 توصیه نمیشود. 4️⃣ Linagliptin (DPP-4 inhibitors) 🔸اشکال موجود در ایران: • قرص لیناگلیپتین 5 میلی گرم • ترکیبی لیناگلیپتین + متفورمین • ترکیبی لیناگلیپتین + امپاگلیفلوزین • ترکیبی لیناگلیپتین + متفورمین + امپاگلیفلوزین
🔺دوز توصیه شده به صورت کلی در دیابت تایپ دو: روزانه 5میلی گرم/ با یا بدون غذا.
🔸مکانیسم اثر • کاهش قند خون ناشتا و پس از غذا • توانایی کاهش A1C: حدود 0.8-0.5 درصد 🔸نکات دارویی • به عنوان درمان کمکی یا درمان جایگزین در بیمارانی که به متفورمین جواب ندادند(یا متفورمین کنترااندیکاسیون داشته). • ارجح در بیمارانی که به هدف گلایسمیک نزدیک هستند(بخصوص برای پیشگیری از ریسک هایپوگلایسمی و چاقی) • عدم تحمل متفورمین یا نارسایی کلیه (CKD مرحله ۳ تا ۵) • عدم نیاز به دوز ادجاست در نارسایی کلیه • تاثیری در بهبود/بدتر شدن کاردیورنال ندارند. • توصیه به اضافه کردن آن به GLP-1 ها نمیشه. • تاثیر زیادی روی وزن ندارد. • داروهای انسولین و سولفونیل اوره، نیاز به کاهش دوز دارند.(در مصرف همزمان با لیناگلیپتین). 🔺اگر بیمار دیابت نوع ۲ دارد + هیپرگلایسمی خفیف + عملکرد کلیوی طبیعی یا مختل+ نگران هایپوگلایسمی بودیم+ نگران weight gain بودیم، Linagliptin به عنوان درمان کمکی انتخاب مناسبی است. 🔸مواردی که باید در طول درمان پایش شوند: • قند خون ناشتا و HbA1c ( حداقل 2 بار در سال، و یا هر سه ماه برای کسانی که به هدف درمانی نرسیده باشند) • علائم پانکراتیت (درد شکم منتشرشونده به پشت) • درد مفاصل یا علائم حساسیتی (راش، ورم صورت) • علائم نارسایی قلبی • علائم بولوس پمفیگویید( eg. blisters, erosions) 5️⃣ Sitagliptin (DPP-4 inhibitors) 🔸همان موارد لیناگلیپتین به علاوه نکات زیر: 🔸اشکال موجود در ایران: • قرص لیناگلیپتین 100 میلی گرم • ترکیبی Sitagliptin+Metformin
🔺دوز توصیه شده به صورت کلی در دیابت تایپ دو: روزانه 100میلی گرم/ با یا بدون غذا.
• در بیماران با نارسایی کلیه قابل استفاده است (با تنظیم دوز بر اساس GFR) نیاز به تنظیم دوز بر اساس GFR دارد:
GFR ≥ 45 → 100 mg/day GFR 30–45 → 50 mg/day GFR < 30 → 25 mg/day
• عملکرد کلیوی (Cr و eGFR)، باید قبل و حین درمان پایش گردد. 6️⃣Liraglutide (GLP-1RAs) 🔸اشکال موجود در ایران • قلم تزریقی 6mg/ml, 3ml(بصورت کلی میشن 18 میلی گرم، و براساس میلی گرم درجه بندی میشه). • به شکل Victoza (ویکتوزا) و معادل ایرانی آن Melitide (ملیتاید) برای دیابت.(Saxenda برای مدیریت وزن)
🔺دوز توصیه شده به صورت کلی در دیابت تایپ دو: شروع معمول 0.6 mg SC روزانه (هفتهٔ اول) سپس افزایش به 1.2 mg و در صورت نیاز تا 1.8 mg روزانه (دوز نگهدارنده معمول). / • روزانه و بدون توجه به وعده غذایی • با انسولین در یک سرنگ نباید میکس شود. • شکم، بازو و یا ران برای تزریق مناسب است. • هربار از سوزن جدید استفاده شود. • در یک محل تزریق نشود، مثلا امروز اطراف ناف بود، فردا در جای دیگر از اطراف ناف تزریق شود.(جلوگیری از لیپوهیپرتروفی)
🔸مکانیسم اثر: • افزایش ترشح انسولین وابسته به گلوکز/ کاهش تخلیه معده و به دنبال آن احساس سیری و کاهش گلوگاکون بعد از غذا/ • توانایی کاهش A1C: حدود 1.5-0.8 درصد 🔸نکات دارویی • به عنوان درمان کمکی یا درمان جایگزین در بیمارانی که به متفورمین جواب ندادند(یا متفورمین کنترااندیکاسیون داشته). • موثر در زمانی که از اهداف کنترل قند دور باشیم(eg HbA1c 9 to 10%) • موثر در افرادی که ریسک CVD دارند • موثر در افرادی که تمایل به کاهش وزن داریم برایشان • عدم استفاده همزمان با داروهای DPP4-I • عدم نیاز به دوز ادجاست در بیماری کلیوی- کبدی • 🔺منع مصرف در: • سابقهٔ خانوادگی یا شخصی سرطان سلول C تیروئید (MTC) یا MEN2، سابقهٔ پانکراتیت فعال، بارداری، یا بیمارانی که نسبت به GLP-1 حساسیت دارند. 🔸مواردی که باید در طول درمان پایش شوند: • قند خون ناشتا و HbA1c ( حداقل 2 بار در سال، و یا هر سه ماه برای کسانی که به هدف درمانی نرسیده باشند) • علائم پانکراتیت • ارزیابی TG • ارزیابی علایم مربوط به کیسه صفرا • ظهور علایم افسردگی/ افکار سویسایدال/ رفتاری • علائم تیروئید: تودهٔ گردن، اشکال در بلع، گرفتگی صدا پایدار — به‌خصوص در بیمارانی با سابقهٔ خانوادگی MTC/MEN2. • قلبی‌عروقی (افزایش ضربان قلب )

داروشناسی در دیابت تایپ دو (بجز انسولین!) *(با تشکر از کانال گایدلاین‌های درمانی؛ که برخی نکات از محتوای ایشون آورده شده‌است). 1️⃣Metformin 🔸اشکال موجود در ایران • قرص متفورمین (اغلب 500 یا 1000 میلی گرم) • ترکیبی دو دارویی: متفورمین + (سیتاگلیپتین/لیناگلیپتین/امپاگلیفلوزین/داپاگلیفلوزین/پیوگلیتازون) • ترکیبی سه دارویی: متفورمین + لیناگلیپتین + امپاگلیفلوزین
🔺دوز درمانی توصیه شده به صورت کلی در دیابت تایپ دو: شروع با ۵۰۰mg روزانه همراه غذا ، افزایش تدریجی تا ۲۰۰۰mg/day طی یک الی دو ماه(در صورت نیاز).
🔸مکانیسم اثر • کاهش تولید گلوکز در کبد از طریق مهار گلوکونئوژنز، افزایش حساسیت به انسولین در بافتهای محیطی. • توانایی کاهش A1C: حدود 2-1 درصد 🔸نکات دارویی • در بیماران Asymptomatic داروی انتخابی اول است، مگر در صورت وجود منع مصرف یا اندیکاسیون قوی برای داروی دیگر. • اررزان، شواهد علمی کافی، در دسترس و... . • اثر کمی برای کاهش وزن • عوارض گوارشی (اسهال، تهوع استفراغ، دیس پپسی، درد شکم و...). • افزایش ریسک vit B12 deficiency (توصیه به مصرف ب12 در افرادی که high risk هستند). 🔺درباره‌ی هولد متفورمین قبل از کنتراست وریدی یا شریانی: ۱. اگه به دلایل دیگری (غیر از متفورمین) در معرض خطر بالای لاکتیک اسیدوز باشه، (مانند بیمارانی که شوک، افت فشار خون، یا هایپوپرفیوژن دارند)، متفورمین باید هولد بشه. ۲. در eGFR<45 آپتودیت توصیه میکنه متفورمین هولد بشه. ۳. گایدلاین رادیولوژی آمریکا میگه اگه eGFR>30 و شواهد AKI نداشته باشیم نیاز نیست متفورمین هولد بشه. ▫️برای شروع مجدد متفورمین: زمانی که eGFR ری‌تست بشه(معمولا ۲ تا ۵ روز) و eGFR>30 تایید بشه. 🔺منع مصرف در: • بیماری کلیوی eGFR< 30 • بیماری شدید کبدی • الکل (active alcohol abuse) • بیماری قلبی (unstable or acute HF at risk of hypoperfusion) • سابقه اسیدوز لاکتیک در مصرف با متفورمین • کاهش پرفیوژن بافتی به علت انفارکتوس یا سایر موارد 🔸مواردی که باید در طول درمان پایش شوند: • قند خون ناشتا و HbA1c ( حداقل 2 بار در سال، و یا هر سه ماه برای کسانی که به هدف درمانی نرسیده باشند). • آزمایش u/a (بخصوص برای قند و کتون) • عملکرد کلیه (eGFR, Cr) قبل از شروع و سپس حین درمان هر 3-6 ماه • مانیتورینگ CBC اولیه و سپس سالانه(چک فولات و ویتامین ب12 درصورت شک به آنمی مگالوبلاستیک) • مانیتور vit B12 هر 1 الی 2 سال(بویژه در نوروپاتی، مصرف بیشتر از 4 سال متفورمین، سایر علایم و رییسک فاکتورهای vit B12 deficiency). • آنزیم های کبدی قبل و حین درمان 2️⃣ Empagliflozin (SGLT2 inhibitor) 🔸اشکال موجود در ایران • قرص امپاگلیفلوزین 10 و 25 میلی گرم. • ترکیبی امپاگلیفلوزین + متفورمین • ترکیبی امپاگلیفلوزین + لیناگلیپتین • ترکیبی امپاگلیفلوزین + متفورمین + لیناگلیپتین
🔺دوز توصیه شده به صورت کلی در دیابت تایپ دو: شروع با دوز روزانه 10 میلی گرم، و افزایش تا 25 میلی گرم روزانه، بعد از 4-12هفته(درصورت نیاز)/ صبح ها با یا بدون غذا.
🔸مکانیسم اثر • افزایش دفع گلوکز در ادرار و کاهش گلوکز خون بدون تحریک مستقیم انسولین • توانایی کاهش A1C: حدود 0.7-0.5 درصد؛ که این مقدار در eGFR<45 کمتر هم میشود. 🔸نکات دارویی • به عنوان درمان کمکی یا درمان جایگزین در بیمارانی که به متفورمین جواب ندادند(یا متفورمین کنترااندیکاسیون داشته). • مناسب در مشکلات قلبی-عروقی و کلیوی • داروهای انسولین و سولفونیل اوره، نیاز به کاهش دوز دارند.(در مصرف همزمان با امپاگلیفلوزین). • در eGFR ≥20-30 نیازی به دوز ادجاست نیست و در eGFR<20 امپاگلیفلوزین نمیدیم. • در HF و CKD دوز روزانه 10 میلی گرم توصیه میشود. • موثر در کاهش وزن، کاهش فشارخون سیستولیک، و مشکلات قلبی-کلیوی • داروی سوم در بیماری که با دو داروی خوراکی دیگر قندش کنترل نشده. 🔺نامناسب برای بیماران با هیپوولمی، عفونت مکرر ادراری یا سابقه DKA و یا در دیابت تایپ1 🔺حداقل به مدت 3روز قبل از پروسیجر(مثل کولونوسکوپی یا جراحی)، به صورت موقت قطع شود. 🔸مواردی که باید در طول درمان پایش شوند: • قند خون ناشتا و HbA1c ( حداقل 2 بار در سال، و یا هر سه ماه برای کسانی که به هدف درمانی نرسیده باشند) • عملکرد کلیه (eGFR, Cr) قبل از شروع و سپس حین درمان هر 3-6 ماه • وضعیت حجم بدن: فشار خون، علائم دهیدراسیون، الکترولیت ها • علائم عفونت‌های ادراری یا تناسلی • علایم diabetic foot • در بیماران پرخطر: سطح کتون (در شک به DKA)
