Notes on cornea pt 2 Exposure keratitis: •corneal ulcer with lagophthalmos •lower third of the cornea is affected with upper horizontal line ---------------------------------------------- Keratomalaci: •the eye is quite (no pain) & white (melting) •give topical vitamin A --------------------------------------------- Neurotrophic keratopathy: • CN5 lesion ➡️ no pain • specific c/p: epithelium exfoliation - large central ulcer - hypopion - perforation - red eye (angry look) - painless • specific TTT: temporary tarsorrhaphy (lateral or paramedian) but without anaesthesia (ماهو مش حاسس أصلا 😒) ---------------------------------------------------- Arcus senilis: •mainly bilateral, may be unilateral in case of unilateral carotid stenosis (مش هيحصل في الناحية اللي فيها ضيق للكاروتيد) •mainly in old age but may be in young in some cases like DM ----------------------------------------------- Keratoconus: • bilateral progressive corneal thickness associated with apical protrusion & irregular myogenic astigmatism • aqueous enters the corneal from tearing of the descemet membrane (acute hydrops) ➡️ diminished vision •corneal topography is specific diagnostic tool ➡️ important in pre-clinical cases ➡️ use it if suspect keratoconus in pt of astigmatism before doing the LASIK (LASIK is contraindicated) •TTT: rigid contact lenses - corneal collagen cross linking - ring segment implants - keratoplasty (deep anterior lamellar keratoplasty= DALK) - 5% hypertonic saline for hydrops

Notes on cornea (pt 1) •أهم موضوعين في الشابتر هما الcorneal ulcer والkeratoconus •cornea is the most sensitive organ in the body •it is avascular ➡️ transparent & less immune cells •central thickness is 0.52 mm •Bowman layer can't regenerate but the descemet membrane can •corneal endothelium works as a pump of water to make the stroma dehydrated •causes of loss of corneal sensation: (herpetic or leprotic keratitis - neurotrophic keratitis or cerebellopontine angle lesion - keratomalacia) •slit lamp microscopy is important for corneal examination • specular microscopy on slit lamp is important for corneal endothelium examination --------------------------------------------- bacterial Keratitis & Corneal ulcer: •main corneal ulcer symptoms: pain - redness - edema - lacrimation - blephrospams - photosensitivity •the stroma can absorb the orange fluorescence and becomes green = ulcer •chronic dacryocystitis is the comments PDF of pneumococcal keratitis •perforation is common •pneumococci ➡️ toxins ➡️ iris ➡️ exudate , cells , fluids ➡️ pseudo - pus (hypopion or sterile pus) ➡️ 2ry glucoma •pneumococci itself can't invade the cornea as well as the pseudomonas..... diphtheria, gonococci & listeria can invade •the triple specific TTT: penicillin for pneumococcus- pure (100%) carbolic acid - treat the chronic dacryocystitis -------------------------------------------------- Viral keratitis (HSV): •typical dentritic Corneal ulcer (called beautiful ulcer) •it is superficial and never perforate except if you give corticosteroids (Contraindicated here) or 2ry infection happened....it never vascularize and has high recurrence •specific diagnosis is double staining test.... Corneal hypothaesia is diagnostic & prognostic •HZV ulcer is atypical dentritic (pseudo) •specific TTT: debridement - acyclovir ----------------------------------------------- Fungal keratitis: •typical Hx is: فلاح دخل في عينه عود حطب أو نبات •specific c/p: elevated plaques - feathering borders - satellite bodies - hypopion --------------------------------------------- Acanthamoeba keratitis: •typical Hx: واحدة ست بعدسات لاصقة مبتنضفهاش كويس •commenat PDF: contact lens •specific c/p: severe pain out of sudden with no early signs - radial kerato-neuritis - confocal microscopy to detect the cyst •specific TTT: topical chlorhexidine 0.02% ---------------------------------------------- Complications: •Nebula affects the vision more if it's paracentral & it's scattered •leucoma affects the vision more if it's central •do surgical perforation (paracentesis) before pathological perforation happens •corneal abrasion = loss of the epithelium....... Corneal ulcer = loss of epi. & Other layers ➡️ necrosis & suppuration •signs of fistula: soft tension - absent ant chamber - green river sign with seidel test positive •in perforation with iris prolapse: leave it alone! Don't reposit nor excise ------------------------------------------- Definitions: • lagophthalmos= incomplete closure of the palpebral fissure when eyelids are gently shut but not during swallowing saliva •leucoma adherent = dense white scar in cornea in which iris is incarcerated •leucoma non-adherent: as previous but iris isn't incarcerated •staphyloma= as leucoma adherent but ectatic (protrusion)
