| 13 | Local anesthesia:Classification of LA according chemistry:
1_Ester compound: hydrolyzed by liver and plasma esterases 👉🏻(cocaine, procaine, benzocaine, tetracaine)
2_Amide compound:are dealkylated in liver 👉🏻(lidocaine,prilocaine ,cinchocain,bupivacaine, mepivacaine,Ropivacaine)
1_prilocaine: It is amide compound and may causes methemoglobin during high doses and used to (dermal anesthesia+Venepuncture in children)👉🏻مهم
2_mepivacaine: is not used in obstetrics due to toxicity in neonates
3_Cocaine it is ester of benzoic acid (medium duration) it is too toxic to be injected therefore it is used topical only and it has effect on (CNS, CVS, Tolerance , abuse, hyperpyrexia)
4_Bupivacaine :may severe CVS toxicity including arrhythmia and hypotension
5_procaine (short duration) it is ester of diethyl amino ethanol and it is hydrolyzed by (PABA)
these drug lacks topical activity.
6_Lignocaine (zylocaine, lidocaine) (medium duration)
It is amide LA and it is choice for surface anesthesia+injection
It is standard drug against all other LA are compared.
📌ملاحظات مهمه
*Convulsions are treated by (Diazepam 0.1_0.2mg/kg)
*In alternative vasoconstrictor is used (felypressin )this drug does not have side effects on the heart and blood pressure
*Exception of cocaine all local anesthesia are Vasodilator (مهمه)
*Adrenaline and less commonly nor adrenaline are combined with LA to double its duration of action.
*effect of the vasoconstrictors is done by injection of phentolamine (alpha blocker) into the site where the operative job is done
*LA are weak bases and are usually water insoluble for this reason they are rsually prepared in an acidic salt solution (hydrochloride salt) which is water soluble and stable. | 307 |
