🟢Intravenous antibiotics are indicated in sickle cell disease patients with fever, even when a specific focus of infection such as otitis media or UTI is identified. The presence of a localized infection does not eliminate the need for parenteral antibiotics in febrile patients with sickle cell disease. 
Rationale for IV antibiotics in febrile SCD patients:
Patients with sickle cell disease develop splenic dysfunction as early as 3 months of age, placing them at high risk for septicemia and meningitis from encapsulated bacteria, particularlyStreptococcus pneumoniae, Haemophilus influenzae, and Neisseria meningitidis.  This risk persists despite penicillin prophylaxis and vaccination, as adherence to prophylaxis is often suboptimal and nonvaccine bacterial serotypes remain a threat.
