🔺اذا جاء مريض يشتكي من lower back pain اهم سؤالين ضروري ينسألوا 1.Radiating to lower limb or not 2.Is it more in morning or at the end of the day If more at morning the cause is mostly inflammatory as sacroiliitis If more at the end of the day the cause most probably is mechanical as prolapse

زملائنا الامتياز: ده ملخص سريع جدا لعلاج الضغط وإمتي تبدأ وإمتى تتابع ! حسب توصيات ال WHO 2021 بكل بساطة علشان يبقى سهل يثبت مع حضرتك على طول: . when to start pharmacological antihypertensive treatment ? WHO 2021 recommend that: 1- individuals with a confirmed diagnosis of hypertension and systolic blood pressure of ≥140 mmHg or diastolic blood pressure of ≥90 mmHg. (but no co-morbidity) . 2- antihypertensive treatment of individuals with existing cardiovascular disease and systolic blood pressure of 130–139 mmHg. . 3- antihypertensive treatment of individuals without cardiovascular disease but with high cardiovascular risk, diabetes mellitus, or chronic kidney disease, and systolic blood pressure of 130–139 mmHg. . which types of Rx to start ?! WHO 2021 recommends the use of drugs from any of the following three classes of pharmacological antihypertensive medications as an initial treatment: 1. thiazide and thiazide-like agents 2. angiotensin-converting enzyme inhibitors (ACEis)/angiotensin-receptor blockers (ARBs) 3. long-acting dihydropyridine calcium channel blockers (CCBs). . And If your pt. need combination, use also the same drug groups, preferably with a single-pill combination (to improve adherence and persistence) . Target of Rx: 1- <140/90 mmHg in all patients with hypertension without comorbidities. 2- <130 mmHg in patients with hypertension and known cardiovascular disease (CVD). 3- <130 mmHg in high-risk patients with hypertension (those with high CVD risk, diabetes mellitus, chronic kidney disease). . Follow up: 1- WHO suggests a monthly follow up after initiation or a change in antihypertensive medications until patients reach target. 2- WHO suggests a follow up every 3–6 months for patients whose blood pressure is under control. . بشكل عام فيه نقطة مهمة جدا لازم تاخد بالك منها: العلاج الجديد أو مع تغيير الجرعة، بياخد 4 أسابيع لحد ما يوصل لأعلى مستوى من شغل العلاج ! وبالتالي أي تغيير قبل كدة مش هيضيف للمريض أي حاجة !

Most important sign on ECG for pericarditis is👉 P-R depression. Sign of pericardial effusion by percussion is 👉 dullness below left scapula.
