+7Dx: Periapical Cyst
Periapical cyst (radicular cyst) is an Inflammatory odontogenic cyst lined by epithelial cells derived from rests of Malassez.
It may be asymptomatic and incidentally found with radiographs but may be painful if infected. Swelling (occurs slowly) is common as the cyst enlarges.
The radiographic pattern is identical to that of a periapical granuloma and a loss of lamina dura is seen.
The cyst is lined by stratified squamous epithelium, which may demonstrate exocytosis, spongiosis, or hyperplasia.
Dystrophic calcification, cholesterol clefts with multinucleated giant cells, and areas of hemosiderin pigmentation may be present in the lumen, wall, or both.

+6Dx: Periapical Granuloma
PA granuloma is an Inflamed fibrous or granulation tissue at the periapical region of a necrotic or infected tooth.
Most PA granulomas are asymptomatic, unless acute exacerbation occurs. Typically, the involved tooth does not demonstrate mobility or significant sensitivity to percussion. The soft tissue overlying the apex may or may not be tender. The tooth does not respond to pulp tests unless the pulpal necrosis is limited to a single canal in a multirooted tooth.
In radiographic view, affected teeth typically reveal loss of the apical lamina dura. The radiolucent lesion may be circumscribed or ill-defined. Root resorption is not uncommon.

+6β’ Nasopalatine duct cyst (incisive canal cyst) is the most common among non-odontogenic developmental cysts and arises from embryogenic remnants of nasopalatine duct.
β’ Most of these cysts develop in the midline of anterior maxilla near the incisive foramen.
β’ Most patients are asymptomatic. When symptomatic, they present as swelling of anterior hard palate, sometimes associated with pain and drainage.
β’ They are seen as a solitary well-defined, oval or round unilocular radiolucency, between central incisors, >0.6 cm in diameter.

β’ Root resorption and tooth displacement may be present.

superior displacement of the tongue on a background of six months of progressive submental swelling.
β’ Dermoid cysts are cysts filled with sebum like material with evidence of specialized skin derivatives.
β’ Floor of mouth dermoid cysts account for 1.6% of all dermoid cysts and they usually present as a midline symmetrical slowly enlarging lesion.
β’ Cysts superficial to geniohyoid may cause posterosuperior displacement of the tongue, dysphonia, dysphagia or airway obstruction. While cysts inferior to geniohyoid can cause submental swelling.

+9Ewing's sarcoma (ES) is a rare malignant tumor, primarily affects the skeletal system. 90% occur in the first three decades of life. Nearly 50% of all cases reported occur in the femur and pelvic bones. Less than 3% of all cases originate in the jaws.
Rapid growth and propensity for metastasis are dominant features of this primary malignant bone tumor. Symptoms such as swelling, pain and sensory disturbances are rather unspecific and can sometimes be misleading.
ES is a poorly differentiated neuroectodermal tumor with small, round and blue cells. Most cases show a characteristic translocation t(11;22)(q24;q12).
Histopathological examination of the resected bone, showed infiltration of both mandibular resection margins necessitating re-excision. Two weeks postoperatively the patient underwent chemotherapy.

+3HODGKIN LYMPHOMA (Owl-Eye-Nuclei)
β’ Classic Reed-Sternberg (RS) cells are quite large measuring up to 100 microns in size with abundant pale eosinophilic cytoplasm.
β’ The nucleus is bilobed or multilobed making the cell appear binucleated or multinucleated in sections.
β’ Some RS cells may actually have two or more separate nuclei. The nuclei have a distinct, thick nuclear membrane and vesicular chromatin.
β’ There is a single, large, round, eosinophilic, central nucleolus surrounded by a halo.
β’ In the most classic examples of RS cells, the two nuclei, placed side-by-side created an "owl eye" appearance. RS cells may be surrounded by a rosette of T-cells.

+5A 73 yrs old man presenting with metastatic carcinoma in the mandible.
He complained of swelling and pain in the right mandible and paresthesia of the right lower lip for the past 3 weeks.
His medical history included ascending colon adenocarcinoma that was treated with surgery 9 months previously with liver metastasis after surgery and subsequent chemotherapy.
Histopathological findings of the right mandible showed moderately differentiated adenocarcinoma similar to the primary tumor.

+169-year-old male patient who was referred by his dentist for detecting a lesion in the oral cavity.
Non-smoker, non-drinker patient but has been a coca chewer for more than 15 years.
Six years ago, a prosthesis was made that hurt the mucosa and he stopped using it due to the chronic trauma it produced.
Two biopsies were taken and a verrucous carcinoma and a squamous cell carcinoma were detected.
This case remind us that patients should visit a dentist if they notice any lesions in the oral mucosa, as early as possible.

+8A 51-year-old male with a history of prostate cancer, surgically treated 3 years earlier, complaining of a 4-month mandibular swelling and paresthesia. The intra-oral evaluation showed an extensive swelling with teeth mobility in the right mandible body.
The panoramic radiography and CBCT images showed an extensive osteolytic lesion.
PET-scan showed confined lesion in mandibular bone and ruled out metastasis to other areas.
An incisional biopsy was performed and the histopathological and immunohistochemical analysis established the diagnosis of diffuse large B-cell lymphoma.
The treatment included six cycles of chemotherapy with complete remission. The patient was under the seventh month of follow-up with no evidence of relapse.

Along with mast cells and basophils, they also control mechanisms associated with allergy and asthma. They are granulocytes that develop during hematopoiesis in the bone marrow before migrating into blood.They form about 2 to 3% of WBCs.
Eosinophil infiltrate in association with the head and neck squamous cell carcinoma (SCC) have been reviewed in literature and the significance of such an association has been variably thought to be either a potential diagnostic tool for stromal invasion or as a prognostic indicator.

Panoramic view shows multiple βScooped Out" radiolucencies in posterior part of mandible and maxilla. Extensive superficial alveolar bone involvement caused the βFloating in Airβ tooth appearance.
Microscopic examination reveals a diffuse infiltration of mononuclear cells resembeling histiocytes with an abundance of eosinophils, lymphocytes and neutrophils.
Immunohistochemical procedures, CD-1a or langerin(CD207), can be used to confirm the presence of langerhans cells and diagnosis of langerhans cell histiocytosis.
The patient underwent excision and curettage for these lesions.Adjuvant therapy such as chemotherapy and radiotherapy may be considered after surgery.
Prognosis is excellent if the disease is confined to jaws.

+7GEOGRAPHIC TONGUE
β’ Geographic tongue is a benign chronic relapsing recurring inflammatory condition of the oral cavity with unknown etiology.
β’ It usually presents as asymptomatic erythematous migratory atrophic serpiginous patches with loss of filiform papillae surrounded by white serpiginous borders that involve the lateral and dorsal aspects of the tongue, with varying shapes and sizes, giving it its migrating map-like pattern.
β’ Extra-lingual lesions can be seen on lips, labial and buccal mucosa and floor of the mouth.
β’ The outcome of geographic tongue is good. In most cases, it resolves over time without treatment.
