HAIRY TONGUE β€’ Hairy tongue, also known as black hairy tongue is an unusual, harmless condition characterised by a 'Hairy' enlargement and discolouration of the filiform papillae. β€’ It most often occurs in adults over 40 years of age and is rare in infants. β€’ Hairy tongue is usually asymptomatic and the main problem is its unsightly appearance. β€’ Occasionally patients may complain of a burning or tickling sensation on the tongue, nausea, or halitosis. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

LEUKOPLAKIA β€’ Leukoplakia is a White Plaque and a Clinical term Excluding other Diagnoses to which this Plaque Cannot be Attributed. β€’ It is by far the most common oral precancer lesion that the risk of its malignant transformation depends on patient habits, histopathologic and clinical features. β€’ Lesions on the tongue, floor of mouth and lip vermilion are more susceptible to dysplastic changes and biopsy should be considered. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

WHITE SPONGE NEVUS β€’ White sponge nevus is a rare, autosomal dominant hereditary genodermatosis exhibiting marked leukokeratosis. β€’ De novo mutations occur in type 4 and type 13 keratin genes. β€’ Intra oral Lesions are most common in buccal mucosa and present as bilateral asymptomatic white plaques with a rough, spongy appearance which do not detach on scraping. β€’ Because this is a benign condition, no treatment is necessary and disease progression usually stops after puberty. β€Ž #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

β€’ An aphthous ulcer is the most common ulcerative condition of the oral mucosa, and presents as a painful punched-out sore on oral mucous membrane. β€’ 20% of the population have one or more, at least occasionally. They usually first appear in childhood or adolescence, and more commonly affect females than males. β€’ Aphthous ulcers can be an early manifestation of a systemic disease such as BehΓ§et disease, or gastrointestinal disorders including celiac disease, Crohn disease, and ulcerative colitis. β€’ Interestingly, smoking may be protective against aphthae, even though smoking makes many oral and skin conditions worse. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Lupus erythematosus (LE) is a group of diverse, persistent autoimmune inflammatory diseases. Systemic lupus erythematosus (SLE) affects several organs (such as skin, joints and kidneys) and blood tests reveal circulating autoantibodies. Skin involvement or cutaneous lupus (CLE) affects 80% of patients with SLE and it is the first sign of SLE in about one-quarter of them. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Schematic vision of surface zone of enamel in a longitudinally sectioned tooth, and buccal aspect of upper molar. Perikymata are clearly visible across the entire crown. Perikymata are incremental growth lines that appear on the surface of tooth enamel as a series of linear grooves. In anatomically modern humans, each perikyma takes approximately 6–12 days to form. Thus, the count of perikymata may be used to assess how long a tooth crown took to form. Wikipedia

Miller's Classification of Gingival Recession: β€’ In Miller Class I and Class II recession, no loss of interdental bone or soft tissue, and complete root coverage could be predictably achieved. β€’ The difference between a Class I and Class II recession is based on whether or not the recession extended to or beyond the mucogingival junction (MGJ). β€’ Miller Class III recession pointed out two scenarios in which complete root coverage could not be achieved. One was an extruded tooth with no interdental bone or soft-tissue loss and the other was associated with loss of some interdental bone and soft tissue that limited the amount of root coverage that could be achieved. Newer surgical techniques now allow the clinician to sometimes attain complete root coverage when interdental bone loss exists. β€’ In Miller Class lV recession, the interdental recession is described as being severe and the papilla absent, which meant that root coverage was unachievable. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Target lesions typically occur in Erythema Multiforme. They can arise on any body’s site, including Face, Upper Chest, Back, Arms, Legs, Hands, Feet and Mucous Membranes (such as the Lips). A Target Lesion is also called a Bulls-Eye Lesion or a Cockade (a rosette pattern of Concentric Rings). Target lesions appear within the first 3 days of an episode of Erythema Multiforme and once one lesion has appeared, it stays in the same location for 7 days or more until the Skin heals. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

His sister was selected as a donor with 95% of HLA matching an went underwent successful Allogenic Hematopoetic Stem Cell Transplantation (alloHCT). Early symptoms of choronic GVHD (Graft versus Host Disease) started from the beginning of the third year after alloHCT. This autoimmune response was manifested in eyes, joints, skin, and liver. Involvement of salivary and lacrimal glands led to Hyposecretion of Saliva and Tears. Other symptoms: Dyspigmentation, Alopecia (Hairloss), Poikiloderma, Joint Stiffness, Nail Dystrophy, Xeropthalmia, Entropion Eyelid resulting in Trichiasis and Coronal Perforation. Following 6 years of alloHCT, the patient is in remission of cGVHD. Dx: Sclerotic-Phenotype Chronic Graft Versus Host Disease #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Cancer of the upper jaw is a malignant tumor that affects the maxillary bone, characterized by an infiltrative type of growth and regional metastasis. The main symptoms of neoplasia are nasal congestion, purulent bloody discharge, constant aching pain, pathological mobility of intact teeth, and deformation of the alveolar process. The diagnosis of cancer of the upper jaw is made based on the patient's complaints, physical examination data, the results of radiography, cytological and histopathological studies. The treatment of cancer of the upper jaw is combined, resection of the maxillary bone is preceded by a course of chemotherapy or radiation therapy. Sialolithiasis is a common salivary gland disorder mostly in ductal structure of submandibular gland (Wharton duct) which is formed by accumulation of calcium salts around central focus of bacteria, inspissated mucous or foreign material. Symptoms may include episodes of pain and swelling, usually associated with eating. Calculi may be physically detectable if of sufficient size and close to ductal orifice. Treatment β€’ For Small Stones: By manual manipulation (moist heat, sialogogues, increased fluid intake) β€’ For Larger Stones: It may require surgical excision, If it’s sufficiently calcified, radiographic studies may demonstrate radiopaque mass. In panoramic, it may be superimposed over mandibular body. Dx: Sialolithiasis β€Ž#case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Cribriform Adenocarcinoma of the Tongue and Minor Salivary Glands (CATMSG) is the tumor originally described under the name cribriform adenocarcinoma of the tongue (CAT). This distinct tumor entity differs from polymorphous adenocarcinoma by location, cytology, histological architecture, and behavior, with lymph node metastases at the time of presentation. CATMSG is a unique low-grade salivary gland neoplasm and surgical excision with lymph node dissection is the treatment of choice in most patients. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Carcinosarcoma is a true malignant mixed tumor. Parotid gland is the most common location and the mean survival for this rare and highly aggressive tumor is only 29.3 months. Histologically, the neoplasm comprises frankly malignant epithelial and mesanchymal elements. The epithelial component is most often a squamous cell carcinoma or adenocarcinoma, and the most common malignant mesanchymal component is chondrosarcoma followed by fibrosarcoma, leiomyosarcoma, osteosarcoma, liposarcoma and rhabdomyosarcoma. Discovered on GIST-1 (DOG1) is a calcium-activated chloride channel initially described in gastrointestinal stromal tumors (GIST), but now known to be expressed in a variety of normal and tumor tissues including salivary gland tissue. DOG1 staining is a marker of salivary acinar and to a lesser extent intercalated duct differentiation. Strong apical/luminal DOG1 staining is seen in immunohistochemistry (IHC) and it can be used to support the diagnosis of tumors with acinar cell differentiation particularly Acinic cell carcinomas (ACC). #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š
