73-year-old man, complaining of multiple painful oral ulcers, noticed about 1.5 months ago. He reported a gradual increase in size, specifically an ulcer on the right lateral tongue, associated with pain and difficulty in eating. Blistering preceded the development of these ulcers. He denied ulcers or bullous lesions on the skin or other mucosal areas of the body. His medical history was significant for acute myeloid leukemia, diagnosed a month earlier. The patient underwent a biopsy. After histopathological examination and immunohistochemistry, the final diagnosis of benign non-keratinized mucosa with soft tissue showing myelomonocytic infiltrate was made. This diagnosis was supported by the recent diagnosis of acute myelogenous leukemia. The atypical mononuclear infiltrate was suggestive of myeloid sarcoma, also known as granulocytic sarcoma: an extramedullary myeloid tumor. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Rare case of Osteonecrosis of the Jaw in a patient with Myeloproliferative Neoplasia who was receiving Imatinib, the first representative of the class of Tyrosine Kinase Inhibitors 😱 Drugs responsible for the development of Drug-induced Osteonecrosis are Antiresorptive Drugs, Antiangiogenic factors, Human Monoclonal Antibodies, and Tyrosine Kinase Inhibitors (TKIs). TKIs are one of the categories of drugs commonly used in Renal Cell Carcinoma, Gastrointestinal Carcinoma, Chronic Myelogenous Leukemia, and Lymphoblastic Leukemias. Recently, Imatinib has been reported to cause Osteonecrosis. However, occurrence of the condition with this particular drug is very rare. Potentially malignant oral lesions have an increased risk of progressing to cancer, but the risk varies with a number of patient- and lesion-related factors. It is difficult to predict the risk of progression individually. The Pathologist must make a judgment based on the evaluation of each case. However, factors associated with an increased risk of malignant transformation include gender, site and type of lesion, habits, and the presence of epithelial dysplasia on histopathological examination. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Myiasis is defined as an infection of living human tissues by eggs or larvae of dipterous flies. Its occurrence in the oral cavity is rare, and it affects more people with disabilities or mental limitations, on the streets and heavy drinkers. Cochliomyia hominivorax and Dermatobia larvae are the most common causative agents of human myiasis in America. These flies usually lay their eggs in pre-existing wounds and the lack of correct hygiene facilitates their spread. How can you manage this situation? #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

The initial manifestations of pemphigus vulgaris often involve the buccal mucosa, and the disease usually occurs in adults. The average age at diagnosis is 50 years, although rare cases can be seen in childhood. Usually, they complain of pain in the buccal mucosa, and the clinical examination shows superficial and irregular erosions, with ulcerations randomly distributed in the buccal mucosa. These lesions can affect virtually any site on the buccal mucosa, although the palate, labial mucosa, buccal mucosa, belly of the tongue, and gingiva are most frequently involved. Patients rarely report the formation of intraoral vesicles or blisters, and these lesions are rarely identified by the clinician during the examination, with the reason probably being early rupture of the thin, friable roof of the blisters. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

The Tyndall Effect is a phenomenon that consists of the scattering of light by colloidal particles or particles suspended in the atmosphere resulting in the spectrum of a light beam. One of the most interesting examples of the Tyndall effect is the blue color of the sky. It is the result of the passage of white sunlight through the atmosphere. When this occurs, sunlight is scattered by atmospheric particles that are suspended in the air, resulting in the blue color, since it suffers greater scattering. Salivary gland lesions, such as this ranula, can be used as an example. As they present in their content not only water, but several suspended particles, the Tyndall effect can be noticed. Saliva is a heterogeneous mixture that can be considered as a liquid colloid. When we examine a patient with a ranula or mucocele, depending on the depth and amount of accumulated colloidal solution, the blue coloration may be noticed. #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Patients with oral squamous cell carcinoma are mostly men, often older, who were aware of a change four to eight months before seeking professional help (eight to 24 months among lower socioeconomic groups). During the initial phase of growth, pain sensitivity is minimal, which may explain the delay in seeking professional care. If the healthcare professional does not have a high index of suspicion, weeks or months may pass before a biopsy is performed. Gingival carcinoma can mimic periodontal disease, which can lead to a delay in diagnosis and treatment. Delayed diagnosis (>1.5 months) and diagnosis at advanced stages (primary T4 tumor) were reported in 25% and 67% of gingival cancers, respectively. The patient underwent extraction and placement of implants in the anteroinferior region due to periodontal disease. During all follow-up appointments after implant placement, the patient continued to report pain and tenderness. Gingival inflammation was noted, but no signs of purulence were present. Five weeks postoperatively, the surgical site revealed wound dehiscence covered by scaling and necrotic tissue. The existing implants were removed and a biopsy was obtained and submitted for histopathological evaluation, which confirmed the presence of squamous cell carcinoma. During a dental consultation, a 20-year-old woman presented with an asymptomatic sessile nodule of 1.5 cm of unknown duration in the left buccal mucosa. The underside was erythematous, slightly lobular, with intact surface epithelium, while the top was slightly cyanotic, with prominent overlying superficial vessels. After immunohistochemical analysis, positive immunoreactivity was observed in tumor cells for pancytokeratin antibodies, S-100 protein and mammaglobin and negative for p63, factor XIIIa and CD31. Fluorescent in situ hybridization analysis revealed a rearrangement involving the ETV6 gene in 73 of 100 cells examined, consistent with the genetic abnormality found in analogous secretory carcinoma of the breast. A 66-year-old woman presented with a smooth, slightly reddish nodular lesion measuring 45 mm Γ— 40 mm Γ— 20 mm on the buccal and lingual gingiva around the dental implant of the lower left second molar. Horizontal alveolar bone loss around the dental implant was observed on imaging studies. A biopsy was performed and a histopathological diagnosis of pyogenic granuloma was made. Complete excision of the lesion and curettage of the implant surface were performed under local anesthesia. The patient had no recurrence of the lesion after one year of follow-up. Have you ever seen a Pyogenic Granuloma to this extent? Ref: 10.1016/j.ajoms.2021.10.003 #case_study πŸ”ŽπŸ¦·βœ¨ @dental_case_study πŸ”¬ @dent_tech_for_u πŸ“š

Lichen planus is a relatively common chronic dermatological disease that frequently affects the buccal mucosa. The peculiar name of this condition was given by the British physician Erasmus Wilson, who was the first to describe it, in 1869. Lichens are primitive plants composed of symbiotic algae and fungi. The term planus, in Latin, means plant. Wilson probably thought that the skin lesions resembled lichens growing on rocks to deserve the name. Although the term lichen planus suggests a flat fungal condition, current evidence indicates that this is an immune-mediated mucocutaneous disease. The ranula appears as a bluish, floating, rounded volume increase on the floor of the mouth, although deep lesions may present a normal mucosal color. Ranulas are most often seen in children and young adults. Generally, these lesions develop as large masses that fill the floor of the mouth and elevate the tongue, tending to be larger than mucoceles in other intraoral locations. In general, the ranula is located laterally to the midline, a feature that may aid in the differential diagnosis of midline dermoid cyst. Like other mucoceles, ranulae can rupture and release their mucinous contents and then form again. Female patient, 47 years old, presented with a slow-growing and asymptomatic nodule on the hard palate with 6 years of evolution. After biopsy, the diagnosis was Myoepithelioma. Myoepithelioma is a benign tumor of the salivary glands composed almost exclusively of cords, islands or layers of myoepithelial cells that may have epitheiloid, plasmacytoid or spiny forms with clear cytoplasm. It represents 1.5% of all major and minor salivary gland tumors, more common in middle-aged adults, with 40% occurring in the parotid gland, followed by the palate.
