Female patient, 84 years old, attended the emergency room complaining of severe pain and difficulty eating due to ulceration of the lower lip. The patient suffered from dementia, arthritis and osteoporosis, for which amitriptyline, alendronic acid, calcium and folic acid are prescribed. The patient had erythema and irregular shallow ulcerations affecting the upper and lower labial mucosa and the dorsum of the tongue. Hematological investigation and indirect immunofluorescence were performed, with all results within normal limits. The patient reported difficulty in swallowing alendronic acid and, therefore, the medication was crushed and placed in water to facilitate ingestion. After examinations, the diagnosis was of chemically induced ulceration due to prolonged contact with alendronic acid. Treatment included corticosteroids and discontinuation of alendronic acid. An 81-year-old patient presented with a pedunculated mass in the upper left maxilla, present for a short period (<1 month) with rapid progression in size over this period. There was no ulceration or discharge associated with the swelling; however, bleeding on palpation was present. A biopsy was performed and histopathological examination revealed a dense plasma cell infiltrate composed of cytologically atypical cells. Scattered binucleate and multinucleate forms were observed. An immunohistochemical analysis was performed which revealed that the atypical cells were strongly positive for CD138, BCL2, MUM1 and kappa light chain. Male, 37 years old, presented with a pink, non-pulsatile and partially compressible nodule on the left ventral surface of the tongue measuring 2 cm, lasting 8 years. He complained that the swelling was pushing up on his tongue and was now affecting his speech and swallowing. Excision of the lesion was performed. Histopathological analysis revealed a cyst lined partially by histiocytes and partially by gastric body mucosa. The cyst was surrounded by smooth muscle, recapitulating the gastric wall and contrasting with the skeletal muscle of the tongue. The diagnosis was oral heterotopic gastrointestinal cyst. Vitamin C deficiency is known as scurvy and its occurrence is usually limited to individuals whose diets are devoid of fresh fruits and vegetables. Although prevalent in the 19th century, its ease of absorption and availability in the general diet has greatly reduced its incidence. Affected groups include infants from poorer areas and edentulous older men. Clinical signs of scurvy are related to inadequate collagen synthesis. Oral manifestations are well documented and include generalized gingival edema with spontaneous bleeding, ulceration, tooth mobility and increased severity of periodontal infection and bone loss. Male patient, 22 years old, HIV+, sought care due to a multilobulated, sessile nodule, measuring approximately 2 cm in diameter and of varied colors, with discrete red, brown and black foci on the hard palate. The surface of the lesion was irregular and had some areas of ulceration and bleeding during manipulation. Panoramic radiography was requested and no alterations suggestive of bone involvement were observed. An incisional biopsy was performed. Morphologically, the tumor was non-encapsulated and composed of a diffuse neoplastic proliferation of fusiform and ovoid cells organized in a lobular arrangement. In some areas, chondroid differentiation was evident, with focal hemosiderin deposition. By immunohistochemistry, tumor cells were positive for S-100 protein, glial fibrillar acidic protein and D2-40, with low proliferative index evidenced by Ki67. Female patient, 64 years old, presented desquamative gingivitis that had been present for more than two years. In addition, she had itchy post-bullous skin lesions all over her body. Male patient, 52 years old, diabetic, complained of pain in the oral mucosa, progressive tooth mobility and spontaneous gingival bleeding. He was diagnosed with periodontal disease, for which he underwent periodontal treatment over the next five years, without significant improvement. After Biopsy, the diagnosis was Langerhans Cell Histiocytosis, confirmed by immunohistochemical staining, in which the patient presented a positive response against CD1a and negative for CD68, CD15 and CD30 antibodies. Ever heard of oral ulcers caused by Nicorandil? 😱 Nicorandil is a coronary vasodilator, prescribed for the prophylactic treatment of angina. It has been used since 1984 and is usually indicated as a third-line treatment in patients who do not respond to calcium inhibitors or beta-blockers. The opening of potassium channels, in combination with abnormally high levels of nicotinamide and nicotinic acid (nicorandil metabolites) can generate ulcers in the oral cavity. The frequency of occurrence of ulcers in patients treated with nicorandil has been estimated at 5%. The ulcerative lesions that develop are painful; they can be single or multiple (1 to 10 lesions) and are often deep and large in size (0.5–3 cm). Its edges are well defined and its base clean. These ulcers are preferentially located on the tongue and buccal mucosa. Progression is favorable within 1 to 12 weeks after discontinuing nicorandil treatment.
