Oral manifestation of Primary syphilis Syphilis is an infectious disease, transmitted sexually and vertically during pregnancy. It is characterized by periods of activity and latency; due to widespread systemic involvement and the progression to serious complications in some of the patients who were not treated or who were treated inadequately. Oral manifestations of syphilis can present as gray plaques, ulcers with irregular and whitish edges, mucous plaques, nodules, spots and erosion. The presence of condylomata lata, snail-shaped ulcerations, red papular macules and split papules may be noted. The diverse characteristics of oral syphilis lesions can increase the complexity of its diagnosis. Therefore, it is essential that the dentist knows the possible differential diagnoses. ❓️Do you know what Test used to confirm this disease? And what is the appropriate treatment? Alveologingival Group : These fibers originate from the crest of the alveolar bone and inserts into lamina propria of the free gingiva. Circular Group : This fiber group forms a band around the neck of the tooth. Together with the gingival group of fibers, these fibers bind the gingiva to the neck of the tooth. Dentogingival Group : The dentogingival group of fibers they originate from the cervical cementum (near cementoenamel junction) and inserts into the connective tissue of the free gingiva.Among the gingival group of fibers, these are the most frequent fiber bundles. Dentoperiosteal Group : These group of fibers they originate from the cementum slightly below the dentogingival fibers and inserts into the outer cortical plate of the alveolar bone. Alveolar crest Group : The alveolar crest fibers originate from the cementum just below the cementoenamel junction (CEJ) and insert at the crest of the alveolar bone. Horizontal Group : The horizontal group of fibers presents below the alveolar crest group and they originate from the cementum and at a right angle to the long axis of the tooth inserts into the alveolar bone. Oblique Group : The oblique group of fibers originates from the cementum and in an oblique direction inserts into the alveolar bone lining the socket coronally. The oblique group of fibers is more in number as compared to other principal fiber groups. 🔎🦷✨ @dental_case_study

Gingival fibers : are connective tissue fibers that inhabit the gingival tissue adjacent to teeth and help hold the tissue firmly against the teeth. They are primarily composed of type I collagen, although type III fibers are also involved.

A 10-year-old patient was referred for evaluation of increased volume on the dorsum of the tongue, which caused mild discomfort when eating, with no history of bleeding and unknown time of evolution. Due to the appearance and location of the lesion, the hypothesis of lymphangioma was raised. The diagnosis was confirmed after histopathological analysis of a fragment removed in an incisional biopsy. How would you describe this injury? #case_study 🔎🦷✨ @dental_case_study 🔬 @dent_tech_for_u 📚

Exostoses are localized bony protuberances that arise from the cortical bone. Vestibular exostoses occur as bilateral bone volume increases along the buccal surface of the alveolar ridges of the maxilla and/or mandible. They are generally asymptomatic, unless the thin covering mucosa becomes ulcerated due to trauma. Is treatment necessary? 🤔 #case_study 🔎🦷✨ @dental_case_study 🔬 @dent_tech_for_u 📚

Woman, 64 years old, reports smoking 1 pack of cigarettes a day for over 30 years and drinking alcohol daily. It evolves with a lesion on the floor of the mouth, biopsied with a report of squamous cell carcinoma. The tumor was staged at T4N1M0 and referred for oncological treatment. What are the most commonly used treatments for this type of tumor? #case_study 🔎🦷✨ @dental_case_study 🔬 @dent_tech_for_u 📚

Woman, 36 years old, with main complaint of burning sensation in the oral cavity for the last 3 months, worsened mainly when eating hot and spicy foods. Clinically, she presented with desquamative gingivitis with a positive Nikolsky sign. After incisional biopsy, histopathological examination showed acantholysis in addition to the presence of Tzank cells. The basal cells of the epithelium remained attached to the basement membrane. Direct immunofluorescence showed intercellular deposition of C3, IgG and IgM in the stratum spinosum. What is the most likely diagnosis in this case? #case_study 🔎🦷✨ @dental_case_study 🔬 @dent_tech_for_u 📚

CONGENITAL EPULIS: A rare benign tumor in the newborn Congenital epulis (CE) of the newborn is a rare benign soft tissue tumor that presents at birth. It occurs usually as a single mass with various sizes, although some multiple lesions have also been reported.

🤰🏽Pregnancy Granuloma Pyogenic Granuloma is one of the entities most frequently associated with the enlargement of soft tissues in the oral cavity. Similar to a tumor, non-neoplastic in nature, it is considered to represent an exuberant tissue response to local irritation or trauma. However, the pathogenesis of pyogenic granuloma has not yet been completely established. There is a predilection for females, and the most affected area is the gums, representing 75% to 85% of cases. The lips, tongue and buccal mucosa are the other most common locations. The majority of the lesions are described as nodules with a pedunculated base, with a soft consistency and a reddish surface. In pregnant women, this pathology is commonly called granuloma gravidarum  or pregnancy tumor, and may begin or be exacerbated during pregnancy, and is normally only monitored, as it regresses after birth, but sometimes it interferes with the development of masticatory or aesthetic function, requiring intervention. Pyogenic granuloma may be associated with the ingestion of oral contraceptives, indicating that female sex hormones play an important role in the pathogenesis of this lesion. Treatment involves total removal of the lesion and sending the material for histopathological analysis and diagnostic confirmation. In the photos we have the case of a patient who had given birth 2 months ago, but with the lesion progressing over 6 months. We can even notice an increase in extraoral volume due to the extent of the lesion. The lesion was completely removed and the patient remains under follow-up without signs of recurrence. #case_study 🔎🦷✨ @dental_case_study 🔬 @dent_tech_for_u 📚

LIPOMA • Lipoma is a mesenchymal neoplasm that represents 5% of benign tumors in the oral cavity. • This tumor, when present intraorally, generally shows an increase in volume with a soft consistency on palpation, yellowish in color, asymptomatic, slow growing and with a base that can be sessile or pedunculated. • Frequently found on the buccal mucosa, tongue and floor of the mouth. • Treatment consists of conservative total excision and recurrences are rare. • An interesting fact is that, after the biopsy, when storing the lipoma in a 10% formaldehyde solution, the lesion floats in the vial. 🤔 What would be the differential diagnoses in this case? Have you ever seen a lipoma in this location? Tell me in the comments! #lipoma #lipomaoral 🔎🦷✨ @dental_case_study
