Tips about Metoclopromide (Plasil) 1. Below 20 year 👉👉 give 0.1 mg/kg/dose. Avoid it below 1 year. Don't use it more than 5 days. 2. It increases serum aldosterone 👉👉 fluid retention + hypo K + metabolic alkalosis. Avoid it in patients with CKD, HF, and liver impairment. 3. In renal colic, plasil has analgesic effect similar to narcotics, beside its anti-emetic effect 4. It block dopamine receptors in pituitary 👉👉 no inhibitiry hormone to prolactin 👉👉 hyper prolactinaemia 👉👉 infertility, on long term use. 5. It decreases seizure threshold 👉👉 avoided in Epilipsy 6. It has prokinetic effects 👉👉 used in diabetic gastroparesis, 10 mg 30 min before meals. 7. Dose adjustment in CKD: If eGFR < 40% 👉👉 decreases the dose by 50%. If eGFR < 10 👉👉 decrease it by 75%. 8. The most common side effect is acute dystonic reaction ADR 👉👉 Rx by allermine. 9. Safe in pregnancy, better to be avoided in breast-feeding 10. Tips about PPIs 1. The fastest acting ppi is Rabeprazole (within 5-8 min), that is why is called rape-, from rapid. 2. The safest ppi Regarding drug drug interaction is Pantoprazole. 3. Pantoprazole, Rabeprazole, and nexium (form of esomperazole) can be taken Regardless the meal. 4. The highest affinity to H/K ATPase pump is Esomperazole 5. One daily PPIs dose better to be taken before breakfast 30-60 min, rather than other meal times. 6. All ppi particularly Rabeprazole can lower serum TSH level 7. Re-bound hyperacidity phenomenon is seen particularly with omperazole 8. Clopidogrel-PPIs interaction is no longer significant, you don't need to switch into Pantoprazole or seperate between these 2 drugs 8-10 hrs. 9. Extremely rare side effect of ppi is interstitial nephritis 10. Long term PPIs use + peripheral numbness 👉👉 think about PPIs - induced B12 deficiency. #pharma

Pediatric diarrhea: كلش كمن، أحنه فبل شارحين عن الـ severe diarrhea و شلون نبدي بـ IV fluid. هسه خل نشرح عن الـ mild to moderate diarrhea (بطلب من أحد الأصدقاء). علاجها يتكون من ٣ علاجات بصورة أساسية، و علاج واحد ممكن نضيفه او لا 1. Oral rehydration solution ORS 2. Zinc syrup 3. Probiotic 4. +/- Antibiotics (not essential, have special indications) طبعاً هذا الشيء ما جاي يُطبق بصورة صحيحة و عدهم العلاج الاساسي هو الانتيبايوتك. نجي للـ ORS : يجي على شكل sachet بتراكيز مختلفة 20.5 g, 10.25 g, and 4 gram نحل الساشيت ابو العشرين بـ ١٠٠٠ سيسي ماء، نحل أبو العشرة بـ ٥٠٠ سيسي، و نحل أبو الـ 4 بـ ٢٠٠ سيسي. يُحل فقط بالماء (لا عصير و لا حليب و لا شيء آخر)، الماء لازم يكون مو حار، يعني مو تغلي الماي و تجي تحل الساشيت بيه. من تحله، مباشرة تخليه بالفريزر مال الثلاجة حتى يطوّل ٢٤ ساعة عندك 👈👈 أذا ما تخليه بـ الفريزر، ساعة و ينتهي مفعولة . شلون تنطية: Mild dehydration: Deficit 50ml/kg/4 hr [12ml/kg/hr] + maintenance 100 ml/kg/day Moderate dehydration : 75 ml/kg/4hr + 100 ml/kg/24hr مثلا، طفل وزنة 20kg و عندة موديريت ديهايدريشن 👈👈 Deficit: 75 * 20 = 1500 ml/4 hr 👉🏽👉🏽 375 ml/hr Maintenance: 100 * 20 = 2000/ day 👉🏽👉🏽 83 ml/hr So each hr, the pt should take 375 + 83 = 458 ml/hr سهلة كلش، فهّم الام، و كللها تستمر ع العلاج حتى توقف الأسهال. نجي للـ zinc syrup يجي بتراكيز 10-20 مغم/5cc Dose of zinc: Less than 6 month 👉🏽👉🏽 10 mg/day More than 6 mn 👉🏽👉🏽 20mg/day و يستمر عليه لمدة أسبوعين سواء توقف او ما توقف الأسهال. نجي للـ probiotics : ألي موجود عدنا هو enterogerminia شراب. أكو هواي شرابات أحسن من عنده بس مو شائعه عدنا للأسف . و لا تستعمل انتي بايتك بكل الحالات إلا أذا كان اندكيتيد، رجااااااااااااااااااااااءاً #pediatric

هلو راجعت الـ FDA لگيتها أصلاً ساحبة الـ phenylpropanolamine من الأسواق. و ايضاً مسحوب من بريطانيا. بسبب الـ haemorrhagic stroke يعني كل علاج كحة يحتوي على هاي المادة، أنسحب من الصيدليات. الأيتم إلي موجودة عدنا و يجتوي على هاي المادة هو Dolo-cold، و إلي يُستعمل و بكثرة Dolo-cold contains : paracetamol + allermine + caffiene + pseudophedrine + phenylpropanolamine فـ خلص، بعد لحد يكتب العلاجات الي تحتوي على الـ phenylpropanolamine. أعيد و أكرر: أنا مو مندوب و لا هدفي أن أروّج لشركة و أسقط شركة ثانيه، بس أكو أشياء لازم الكل يعرفها. #pharma

Scientifically, all pts with MI should be send for CRP 👉👉 patients with high CRP, have poor prognosis on short and long terms. ليش الـ high CRP عنده Poor prognosis؟؟ Atherosclerosis is an Inflammatory process, just like all other Inflammatory diseases. فتتكون عندك atheroma و حتى هاي الـ atheroma يصير بيها destabilization نحتاج acute intensive inflammation الي راح يسوي rupture of atheroma و يحولى الـ chronic stable angina إلى ACS. فكل ما يكون الـ CRP مرتفع أكثر، كل ما يعني ان الانفلاميشن الي صار داخل الـ Plaque هو أقوى. زين اني شستفاد من هذا الكلام؟؟ Statins has intra-plaque anti-Inflammatory action و المريض مو ننطية ستاتن فقط علمود ننزل الـ lipids، بالعكس حتى لو كانت الدهون طبيعية لازم ننطي ستاتن، و السبب هو ألي ذكرته Statins improve plaque stability via exertion anti inflammatory action inside the plaque. و أكثر ستاتن تمتلك هاي الخاصية هي Atrovastatin and Rosuvastatin هذا ملخص بسيط عن معرفة أهمية الـ CRP في حالات الـ MI. #cardio

أريد أگول شي 😑😑😑 جاي تشوفون أن الـ haemorrhagic stroke كمن خلال هاي الفترة أو بشكل عام الستروك بأنواعها صحح؟؟؟ كـ تحليل من يمي (و الله العالم) هذا بسبب أستعمال أدوية النشلة و الكحة. هواي أدوية نستعملها للگحة تحتوي على Alpha agonist like ephidrine or pseudophedrine or phenylpropanolamine فهاي الأدوية تسوي vasoconstriction و بالتالي تسوي Hypertension and vessels rupture فيصير عدهم haemorrhagic stroke. أذا تريد تستعمل علاج للكحة لـ شخص عندة hypertension، فأتجنب هاي العلاجات الي تحتوي على alpha agonist. اكو هواي علاجات بيها combination لطيف و آمن. مبدئيا، كلهم ننطيهم Lukafen (مثل ما اتفقنا فوگ) أو ننطيهم Bronchorest الي هو Montelukast + levocitrizine (3rd generation anti histamine) بعدها تشوف الگحة مالته dry or wet؟؟؟ 1. Dry cough: use dextromethorphan + 1st generation anti-histamines (like allermine or triprolidine) و الأمثلة كثيرة جدا جدا عن علاجات بيها هذا الكومبنيشن، ما راح اذكرهم حتى ما يصير ترويج. 2. Wet cough: use drugs that contain Bromehxine + Guiafensin و الأمثلة أيضا كثيرة جدا جدا عن علاجات بهذا الكومبنيشن 3. Slightly wet (or slightly dry): use drugs that contain Guiafensin + dextromethorphan و أيضا أمثلة كثيرة عدنا بـ elderly حاول تتجنب أدوية الگحة ألي تحتوي على theophylline مثل Exidil syrup or Exvent syrup or bronchiuum بالأشخاص ألي عدهم قرحة معدة، حاول تتجنب الأدوية الي تحتوي على bromehxine مثل Solvodine syrup or tab, Bronchovet, bronchosolvin, Bronchomark و غيرها #pharma

خل نتفق، أن هذا المنشور مو ترويج لعلاج معين او شركة معينة، بس الكل يحتاج يعرف هاي المعلومات: الألتهابات بالجهاز التنفسي تختلف عن بقية الألتهابات لحدٍ ما، لأن تكون The main mediators of inflammation in respiratory system are leukotriens and histamines و بصورة أقل يكون prostoglandin، فلذلك anti prostoglandin medications ألي هن NSAIDs مثل البروفين و غيرها، مو كلش تفيدنا. بالعكس ممكن تضرنا. فمن يجيك شخص عندة مثلا Cough, allergic rhinitis, tracheitis يعني أي أعراض مال التهابات الجهاز التنفسي، فلازم ننطي Anti histamine + anti leukotriens أقوى انتي هستامين و بنفس الوقت يُعتَبر ءامن واحد هو Fexofenadine (Telfast) 120 mg or 180 mg و بالنسبة للـ leukotrien antagonist المشهور عدنا هو montelukast. أكو علاج أسمه Lukafen (montelukast + Fexofenadine) كلش effective للأشخاص الي عدهم Sore throat, allergic sinusitis, asthma, etc. موجود بأغلب الصيدليات و مو كلش غالي. هذا مو ترويج للـ Lukafen، اصلا ما اعرف الشركة المصنعه و الله. بس تحتاجون هذا العلاج على الأقل لأهلكم، للي عدهم حساسية عنيفة، أو أي أعراض مال كوفد #pharma
