Diffuse hyperpigmentation mainly in sun-exposed area.
Hyperpigmentation, which is itchy + dysphagia (mostly not related to Hyperpigmentation)
1. Photo allergic reaction
هاي ممكن يصير بسبب
prolonged exposure to sunlight in predisposed person.
و ممكن تصير بسبب
Drugs like amiodarone, hydroquinone, fluoroquinolone
و كلكم تعرفون ان ال hydroquinone هو اصلا يُستعمل حتى يزيل ال dark areas بالوجة مثل النمش و الكلف، بس الاستعمال الطويل إله يسبب ochronosis الي هو hyperpigmentation.
2. Itchy hyperpigmentation in perimenopausal females >>> primary biliary cirrhosis PBC
و سبب ال Hyperpigmentation هو بسبب ال pruritus، يعني 2ndry change مو primary، هذا التشخيص الأقرب لهذة الحالة برأيي انا.
3. Haemochromatosis : combination of hepatic insuffuciency + DM + heart failure + joint pain + hyperpigmentation
4. Addison disease
يتميز بشغلة و هي أن ال hyperpigmentation موجود أكثر شي
In buccal mucosa, palmer creases, and scar sites
خلي ببالك ال scar الي تصير قبل ال اديسون ما يصير بيها hyperpigmentation، إنما الي تصير بعده هي الي راح يصير بيها hyperpigmentation (هاي دائما ننسأل بيها بالامتحانات 🙃)
5. Thyrotoxicosis كلش مهمة

📗 Epigastric pain that wakes Pt at night is characteristic of Peptic ulcer since the basal gastric acid secretion peak at night (between 10 pm & 2 am).
📗Epigastric pain that wakes Pt at morning; it's not peptic ulcer pain since gastric acid secretion is at lowest point at morning.

+351 yo F presents with increasing pigmentation over the face , arms, axilla & it started 2yo ago as itchy dark lesion on both sides of her mouth & itch subsided subsequently & complains of mild dysphagia..
No history of contact eczema , used topical steroids & no use of topical creams

The 2nd most important risk factor for MI is smoking.
يعني من يجي شاب عمره ٢٥ سنة و هو heavy smoker و عنده chest pain
تهتم بيه أكثر من رجال عمره ٦٠ سنة و ما عنده أمراض و هم عنده chest pain.
سالفة ال aging مو كلش مهمة مقارنة بال smoking
ال age تجي أهميته من خلال co-factors، يعني تلكاه عنده سكر و ضغط.
اما بس age، ف اوك مهم لكن مو اهم من شاب و heavy smoker.
لا تخلي هجي شاب يطلع بدون ecg.
قصص يابو قصص صايرة عند الشباب و راحوا ب sudden death، لأن ما عدنا هجي معلومة عن ال smoking.
ما بية حيل اكتب هواي 🙂🙂

كلش صعب مريض non diabetic و يصير عنده marked hypoglycaemia بحيث يفقد الوعي، لأن ال compensatiry mechanism شغاله عنده.
و رجع الوعي للطبيعي بعد إعطاء الكلوكوز، معناها صار عندي Whipple triad الي شارحها الكم بموضوع ال hypoglycaemia.
فنجي لل ddx
1. Addison crisis
هاي تفسر ال long hx of vomiting و أيضا تفسر ال hypoglycaemia، و ممكن توفى بسبب ال marked hypotension الي سبب cardiac ischaemia و cardiac arrhythmia, أو ممكن بسبب ال associated days-electrolytemia مثل ال hyperkalaemia!!
2. Pituitary macroadenoma
ممكن كان ال increase ICP هو الي مسبب hx of vomiting و أيضا مسببا 2ndry adrenal insuficiency، و هسه صار عنده pituitary apoplexy لسبب ما، و توفى.
3. Thiamine deficiency
ممكن هو كان alcoholic، و عنده نقص ثايمين و انطيه كلكوز قلا الثايمين و سبب عندي Werinkes encephalopathy و بنفس الوقت صار عندي acute cardiac failure، لأن أيضا ممكن يصير عند جماعة ال beriberi (نقص الثايمين).
4. Ondasteron side effect
ممكن هو عنده long hx of vomiting، و من راجع الصيدلية انطوة زوفران و الي سبب long QT syndrome، و sudden cardiac arrest.
و الله اعلم
ممكن تكون أسباب ثانيه.

Medullary carcinoma diarrhea in induced by calcitonin & 1/3 of the cases diarrhea is the presenting feature.

يلا شباب، ابداعاتكم.
He presented with drowsness, after correction of glucose, the pt regained his consciousness.
Few minutes later, he passed!!!!!!!

انا ليش اذكرلك اشكد الامبوله و شلون تنطي و كذا، لان اعرف الاغلبيه عنده صعوبه بهاي الجزئية

راح اشرح المنجمنت لل status epilipticus بغض النظر عن ال underlying cause مثلاً menngitis او غيرها .
اول ما يجيك مريض عنده SE، مباشرة صير انت ال team leader و لا تخاف من المسؤولية لأن هي اول و تالي عليك.
جيب الطفل و خليه بال prone position او left lateral position حتى تحمي ال Airways من ال aspiration of gastric contents، اهواي اطفال يموتون بسبب ال pulmonary complications.
گل للمرض يجيك الكلوكوز، كخطوة بدائية مهمة جدا، و يجيكة بال glucometer و اذا عندك suspicion لل hypoglycaemia انطي glucose 10% 2-4ml/kg . الكلوكوز ابو 10% موجود بمستفيات الاطفال ، بمستشفيات الكبار موجود عدنا ابو 50%.
ابدي بال diazepam : الامبولة الموجودة هي 10mg/2ml ، تخليها ب 8cc NS ، صار عندك 10mg ب 10cc، يعني كل 1cc=1mg ، و انطي الجرعة 0.3mg/kg وريديا (ال max dose 10mg)، و هي نفسها بالسيسي و بامكانك تعيدها كل 5 دقائق اذا ما تحسن المريض .
اذا ما كدرتوا تحصلون وريد، اسحب النيدل من الكانولا و خلي بس البلاستك و insert it rectally، و انطي الفاليوم بجرعة 0.5mg/kg و ال max dose هي 20mg .
ما استجاب للفاليوم، انتقل لل phenytoin و الامبولة الموجودة عدنا هي 250mg/5ml ، تنطيه بدوز 20mg/kg (مثلا كان وزنه ٢٠، ف انت تنطي 400mg) تخليهن ب 20cc (عادي حتى لو 50, 100,150) و تخليه بال microdropper و تمشيه خلال 20 دقيقة
من تنطي الفنتويين ، كون مريضك ع المونتر لان هذا العلاج هو pro arrhythmic و خطر .
اذا ما استجاب، انتقل لل phenobarbital و الامبوله غالبا تجي 200mg/1ml ، و تنطي بجرعه 20mg/kg خلال 20 دقيقة ب 20 سيسي نورمل سلاين .
اذا ما استجاب، ف انطي midazolam هذا غالبا ما متوفر عدنا، فما راح اشرحة .
خلي بالك لا يكون المريض هو عنده hypocalcaemia، و علاجها 100-200 mg لكل كيلو . قنينة ال Calcium بيها 90mg/10cc، فراحسب الجرعه و انطي .
اذا ما استجاب، فلازم تخدير عام و mechanical ventilation .
هاي الي راح تشوفها بالواقع العملي و الي هي الاقرب الى ال guideline.

من يجيك مريض و عنده vomiting قوي، و انت ما عندك لا بلاسيل metoclopromide و لا زوفران ondasteron
ف تكدر تنطيهم امبولة hydrocortisone HC، و راح يشتغل شغل anti emetic و شغله يخبل.
بس هاا، رجاءاً مو تسيئون استعماله بهذا ال indication.
هذا كتبته الكم للحالات الضرورية، أو إذا مُرافق المريض رجال جبير بالعمر ما يكدر يجيب من برة
عادي اصرف هالشي و راح تشوف ال magic response و تدعيلي بوقتها.

⏩ Loperamide may cause dangerous prolongation of illness in patients with some forms of bloody or inflammatory diarrhea and, therefore, should be restricted to patients with nonbloody stool.

📗 If diarrhea weak up your pt at NIGHT
Ddx:
1. Autonomic neuropathic diarrhea of diabetes
2. Inflammatory Bowel Disease(IBD)
📗 If first thing in the MORNING is diarrhea:
Think about Irritable Bowel Syndrome(IBS).
"Not only IBS, all functional disorders are more sympathetic at Morning"
📗 If fasting improve diarrhea:
Ddx:
1. Irritable Bowel Syndrome
2. Coeliac
3. Lactose Intolerance

هلوووو
احنه شارحين قبل ال status epilipiticus، و اكتب بالبحث راح تلكاها. و گلنا بهذاك الوقت لازم تعبر 30 دقيقة يلا نكول عليها SE، بس نبدي نعالج من ١٥ دقيقة As a case of SE.
حالياً تغير تعريفها، صارت ال SE إذا استمرت أكثر من ٥ دقايق.
و ان شاء الله اليوم بالليل نشرح عن ال
