الموضوع الثاني هو febrile seizure او الصرع الحراي جدا موضوع مهم، و راح أدز فويس و يمكن راح يكون مطول 😑😑 أكثر موضوع حبيتة ، لأن فعلا بوقتة ، و هاي فترة الفايرل انفكشن و شلون تسوي فيفر و الي تسبب كونفلشن

و هاي ال WAP للي ما يطيق يسمع الفويس، بس أني افضلك تسمع الفويس ، و لو يكولون الذاكره البصرية اقوى من السمعية ، بس أني اعتقد السمعية اقوى و تبقى بالبال لفترة أطول .

Summary for PE

أول موضوع طُلِب هو pre-eclampsia أو ما يسمى خاطئاً بتسمم الحمل toxiaemia of pregnancy (خل نشرحها ببساطة و اذا نحتاج فويس ، ندز) 1- definition : it is a state of hypertension diagnosed after the week 20 of pregnancy with at least one of the followings: — proteinuria : dipstick urine test show +1 proteinuria . — symptoms like headache, blurred vision, epigastric pain, lower abdominal pain, and edema. و خلي ببالك أذا هاي النقاط ما موجودة، فالتشخيص هو gestational HT و ليس pre-eclampsia. و بالمناسبة قبل يكولون ال proteinuria is a must، يعني لازم نشوفها، بس هسه حاليا ممكن نشخص ال PE بدون البروتين يوريا. 2- pathogenesis : many factors : بالحالة الطبيعية، البلاسنتا او المشيمة تسوي invasion لل endometrium بس ما تعبر لل myometrium، و هذا الانفيشن يكمل عند أسبوع 20. الي يصير بال PE, أنه الأنفيشن يكون poor، فبالتالي ما يكمل ، فبالتالي ما ينتقل دم كافي من الرحم للمشيمة بعد أسبوع 20 (لذلك حدنا الضغط بعد أسبوع عشرين ممكن بري كلامسيا)، و يترتب على ذلك : A— placental ischaemia will stimulate macrophages in fetal-maternal barrier to produce inflammatory mediators👉🏽👉🏽 will destruct the blood vessels 👉🏽👉🏽 this will cause 2 thing : fluid will move from blood vessels into the tissues (edema)+ platelets will adhere to blood vessels causing thrombosis and ischaemia . انت بس تصور، أرال vesseks مال الكدني تحطمت، فاكيد يصير بروتين باليورن، و نفس الفيسل صار بيهل تجلطات ، فاكيد يصير رينل فيلر، و هذا الكلام ينطبق على اي عضو اخر مثل الكبد و الماغ و الرئة و القلب ، ف اكيد يصير مشكلة بالدماغ و احتمال حتى جلطة و تصير مشكلة بالكبد و ترتفع انزيماتها، و هكذا B— stimulation of endothelium to produce vasoconstrictors 👉🏽👉🏽 HT . 3) Rx : — if BP less than 160 (systolic) and 110(diastolic) 👉🏽👉🏽 mild PE if no other symptoms are present — otherwise, you should give anti HT + Mg sulfate + steroid (for lung maturity) . If not improved, so terminate the pregnancy .

gout ملاحضات عن 🌸 acute gouty arthritis (monoarthritis in most cases) ddx osteoarthritis septic /traumatic arthritis psoriatic arthritis 🌸 regarding investigation normal serum uric acid does not exclude gout normal value(2-7 mg) 🌸 hypouricemic drugs never ever given for acute gouty arthritis treatment because it cause rebound effect ,increase uric acid at first 🌸 TTT of asymptomatic hyperuricemia with allopurinol is indicated only in :family history of gout serum uric acid more than 11 mg

Certain comorbid conditions are associated with a higher incidence of gout, including the following: • Hypertension • Diabetes mellitus • Renal insufficiency • Hypertriglyceridemia • Hypercholesterolemia • Obesity • Anemia
