شغل ال CPR الصحيح يعتمد على الـ team leader اذا كان الشخص الي ينطي ال order هو شخص مضبوط و نبه، حتى الستاف راح يصيرلهم واهس و بس ينتظرون تنطيهم اوردر حتى يشتغلون. و حاول قدر المستطاع ان تقلل ال interruption، هاي اهم نقطة لازم تتذكرها. انت لا تشتغل ب ايدك، خليك فقط تنطي الأوردر للتيم و رتب شغلهم، و صدگني راح تشوف اغلب الكيسات ترجع. مو طبيب الي ما يعرف بديهيات الـ CPR.

Initially to start with redness in the oral cavity and over lips to develop edematous and erythema with burning sensation then spread symmetrical on the extensor surface of distal extremities especially upper limbs. The patient mentioned hx of polycystic ovarian cyst and drugs(Metronidazole and Ciprofloxacin). O/E showed target lesions or bullseye and skin involvement less than 10% and the trunk is spared. No other mucosal surface involvement. The nikolsky sign is negative. Dx Ddx Tx? 🤔🤔

بالطوارئ اجانه حجي عنده DM type 2 فوك 10 سنوات ومستمر على العلاج ومسوي قص معدة على مود وزنة يشتكي من خدر برجلية وبلكوة يمشي حجي شنو أول شي صار عندك كال صداع وافحط ورجلية خدر وما احس بيهن O/E Pallor Dyspneic حجي غمض عيونك وحدد اتجاة اصبعك big toe من احركه حركته جانبي كال للفوك.. حركته للجوة كال جانبي ! سويت بال stethoscope (حسب الامكانيات المتوفرة😁) - Positive babinski reflex - Hyporeflexia to absent ankle reflex - Normal knee reflex شنو العلاجات تاخذها حجي طلعهن من العلاكة من ضمنهن Metformin شكد صارلك تأخذ منه كال فوك 6 سنوات + قص معدة نفكر ب Vit B12 deficiency ومسويلنا subacute combined degeneration رسلته ع CBC طلع عدنا Macrocytic anemia حولته للجملة العصبية وشجعته ع شغلتين 😁 الرنين للظهرك و صورة الدم كلش ضروريات اقفل عليهن 😅 شنو بالكم من Ddx 🤔 ؟

عدنا بيشنت دائما تجينا للطوارئ حوالي بالاسبوع لاحظتها ب3-4 مرات اليوم اجتنا نحو الاسوء A young age female presented with dizziness ومن نقيس ضغطها يطلع نازل دائما تكول عندي prolonged menstrual bleeding وتاخذ فلود كالعادة 😅 هل مرة اجتنا مختنكة بحيث ما تكدر تحجي قسنا ضغطها طلع نازل جان لود قليل ركزت وياها بالهستري وسويت الها CBC + RFT طلع عندها microcytic anemia ركزت وياها ك GIT bleeding طلع عندها Black stool & offensive هنا يتماشى ك melena رجعت قست الها الضغط Supine position 120/60mmHg Upright position 80/50mmHg بحيث ما تكدر توكف وهذا يعني صاير عندها postural hypotension اي خاسرة حوالي 20% فالكيس بشكل عام يتماشى مع Upper GIT bleeding حولت ع اخصائي git شلون تسوون ابروج لل UGITB 🤔
