📝 Beta blocker is not recommended to be first drug in management of Hypertension , however we use it in case of failure of triple therapy as add on drug (4th line ) adding to : 👉 Thyrotoxicosis 👉 ACS , OMI 👉 HOCM 👉 AF 🔖 In hypertensive emergency , the drug of choice especially with CNS problem is Labetalol IV or Nicardipine(combined alpha-beta blocker). #pharma

Eclampsia management بأختصار 1. General mx: left lateral position + aspiration of saliva 2. Specific mx: A) anti HT نبدء بـ labetalol بـ هذا الرجيم 20 mg, 40mg, 80mg, 80 mg, 80 mg بين كل جرعة و ثانية، ننتظر ١٠ دقايق اذا وصلنا 300 mg و ماكو استجابة، نحول ع الـ hydralyzine. نبدء بـ 5mg, ثم 5mg, ثم 10 mg بين كل جرعة و ثانيه، ننتظر ٢٠ دقيقه. ماكو داعي ننطي hydralyzine infusion. المكزمم دوز هي 20mg بس هاي عادة توارثوها الأجيال 🙄🙄. B. Anti convulsant نبدي بـ mg sulfate و كالتالي Loading dose: 4-6 g within 15 min. Manitenance dose: 1-3 g/hr for 24 hr. اذا هو ع منتننس و صار عنده attack، نرجع ننطي 2-4 غرام خلال 5 دقائق. اذا صار عندة attack ثاالثه هنا لازم ننطي Benzodiazepine + anti seizure Diazepam : 10 mg over 3 min, can be repeated 👉👉 maximun is 20 mg Phyntoin 20 mg/kg 👉👉 50 mg/min هذا بأختصار و الفويس بيه تفاصيل اكثر و اعمق، اكيد. #emergency

Case2 21yo F in her first pregnancy at 38wks was brought to the ED with generalized tonic clonic seizure. IV MgSO4 was given but fits was not controlled. What's your action plan? هلووو شباب كم ملاحظة سريعه عن شراب الاوكمنتين Augmentin 125 mg/5 ml بالأطفال جرعة الاوكمنتين بالأطفال بشكل عام 20 mg/kg/day in three divided doses or 25 mg/kg/day in 2 divided doses. ما عدا في حالتين، ما ننطي هاي الجرعه 1. Pneumonia: H. Influenza: 45 mg/kg/day in 3 divided doses Other bacteria: 90 mg/kg/day in 2 divided doses 2. Acute otitis media : 90 mg/kg/day in 2 divided doses اليوم شفت وصفة اوكمنتين لطفل عنده Acute suppurative otitis media ASOM بجرعة 15mg/kg كلش حزنت، لان هجي جرع فقط راح تسوي bacterial resistance بدون أي فائدة. #pediatric

١. اول شي اعذروني اليوم نشرت اهواي، بس علمود تستفادون و تشوفون الكيسات. و شكرا لكل الاخوه الي شاركوا و دزوا الكيسات. اعتذر لان ما نشرت كل الكيسات، لان اكو بيهن بدون هستري و اكو بيهن ما واضحات. ٢. لحد الان اكثر من 150 كيس مختلف عن الـ ECG تم نشرة بالقناة. الفويسات الاولى، باديه من البيزك، تكدرون تسمعونها، للأشخاص الضعيفين بالـ ECG. كلش Unlikely انه تشوف تخطيط خارج هذن التخطيطات الموجودات بالقناة. حتى الكيسات الـ rare المتعلقه بالـ ECG هم ناشرينها، و كلها real cases. و أهم شي الكل يستفاد ❤️❤️😍😍 ٣. هسه نروح نتابع أنمي ناروتو 🤩🤩 @AASmedicalbot

#مشاركة 56 years old female with htn dm presented with genralized fatiguabilty and knee pain. ما اعتقد الـ knee pain هو acute onset. بس هم الناس الكبار بطبيعتهم يحبون يشكون كلشي حتى يزيدون الاهتمام. صعب knee pain و اني افكر بالـ MI!!!! ماكو recommendation لا بالـ ESC و لا AHA على الـ knee pain and MI. عاشت الايادي و هجي كيس ما فلت منكم. Inferoposterior MI with distal lateral ischaemia + reciprocal ST depression in aVL. اني كلت دزولي كيسات، العالم بدت تدز بس ecg 😭😭😭 #ECG

#مشاركة هذا ايضاً Complete heart block بس اكو كذا signs توحي ان هذا البيشنت عنده Advanced heart failure صراحة ما عدنا هستري عن البيشنت. هذا الـ CHB يختلف عن السابق، شكلة Nodal type CHB. لأن عندي regular Narrow complex bradycardia شو طلعت كيساتكم كلها صعبة 😂😂، لو هستري وياه جا صار هذا التخطيط موضع نقاش يخبل 😋😋 #ECG
