⭕️تذكر جيدا في حالة تمزق الأغشية قبل الأوان عند الأم الحامل
Preterm Premature Rupture of Membranes
نستخدم المضادات الحيوية كالتالي
✅أولا نبدا ب( Ampicillin 2 g IV)كل 6 ساعات لمدة 48 ساعة
ثم نتبعها ب ( amoxicillin 250 mg) ثلاث مرات يومياً لمدة 5 أيام
✅أو نستخدم الاريثروميسين 250 مجم في الوريد كل 6 ساعات لمدة 48 ساعة وثم نبدأ بجرعة
333 مجم
ثلاث مرات يوميا لمدة 5 أيام..

⭕️معالجة ال Constipation عند المرأة الحامل :
✅Cause: Decreased peristalsis
✅Nonmedical treatment ➡️Increase high-fiber foods.
➡️Increase fluid intake to eight 8-oz glasses of water a day.
➡️ Increase exercise.
✅ Symptomatic relief
➡️ Stool softeners (check that prenatal vitamin does not already contain docusate)
➡️ Bulk laxatives: Not absorbed.
➡️Stimulants: Not recommended as first-line therapy.
➡️Avoid mineral oil: Impairs vitamin K absorption and could cause hypoprothrombinemia.

✅يجب أن نأخذ بعين الاعتبار تبديل LMWH إلى ال heparin عند ال 36 أسبوع من الحمل
من أجل السماح بتحفيز ال
(neuroaxial anesthesia)
خلال ال( labor and delivery )
✅ يجب قطع استخدام أدوية ال (LMWH or UFH ) التي كانت تستخدم ك Prophylactic قبل ال cesarean section or
vaginal delivery
بفترة (12–24 ساعة)..
⬅️أما بالنسبة للجرعة اللتي كانت تستخدم كجرعة علاجية يجب قطع استخدماها قبل (24–36 ساعة )قبل ال
(cesarean section or
vaginal delivery.)
✅بعدها نستمر با ستخدام ال anticoagulation لمدة 6 أسابيع بعد الولادة
✅ بالنسبة للمرأة اللي يحصل عندها
heparin-induced thrombocytopenia.
تستخدم ال fondaparinux..

✅نتكلم الليلة عن معالجة ال Coagulation Disorders عند المرأة الحامل ،
✅أولا يجب أن يكون هناك ضرورة لاستخدام ال
Anticoagulation
وتتمثل في الحالات التالي
➡️ History of DVT
➡️Prosthetic heart valve
➡️ Deficiencies of clotting factors
➡️ Antiphosantibodies tibodies
✅أيضا عند المعالجة نتجنب ال warfarin
✅أما بالنسبة لل Heparin أو ال
( low-molecular-weight heparin)
⬅️يجب مراقبة للosteoporosis إذا استخدمنا ال heparin لفترة طويلة ..
✅ أيضا يجب أن نعلم بأن ال Low-molecular-weight heparin يُفضل على ال heparin
✅ بالنسبة لجرعة الأدوية يجب التفريق إذا كنا بانستخدمها ك Prophylactic أو ك Therapy..
♻️لما نستخدمها ك Prophylactic تكون كالتالي :
➡️Enoxaparin 40 mg subcutaneously daily
➡️ Dalteparin 5000 units subcutaneously daily
➡️ Tinzaparin 4500 units subcutaneously daily .
➡️ Heparin 5000–7500 units subcutaneously
كل 12 ساعة في المرحلة الأولى من الحمل first trimester
وبجرعةال (7500–10,000 )
units subcutaneously
كل 12 ساعة في فترة المرحلة الثانية من الحمل ( second trimester)
وبجرعة (10,000)
units subcutaneously
كل 12 ساعة لفترة المرحلة الثالثة من الحمل third trimester ما عدا إذا ال
(aPTT)
مرتفع ..
♻️أما إذا كنا نستخدمها لغرض علاجي ستكون الجرع كالتالي :
➡️ Enoxaparin 1 mg/kg subcutaneously every 12 hours
➡️ Dalteparin 200 units/kg subcutaneously daily or 100 units/kg subcutaneously every 12 hours .
➡️ Tinzaparin 175 units/kg subcutaneously daily.
➡️ Heparin 10,000 units subcutaneously every 12 hours with target activated partial thromboplastin time (aPTT) range (1.5–2.5) 6 hours after injection or adjusted-dose
unfractionated heparin subcutaneously every 12 hours, with mid-interval aPTT target of
2 times the control .

✅social anxiety disorder
⬅️ويعتبر الsertraline أفضل واحد منهم بسبب إنه FDA-approved
في هذه الحالة خصوصا ..
✅نبدأ بجرعة 50mg/day
ثم نزيد كل أربعة أسابيع 50mg حتى نصل لل 200mg كل ع حسب الإستجابة ..

A recent Iraq war veteran has been treated successfully with paroxetine for his major depression for the past 3 weeks. He presents to the clinic experiencing nightmares, “feeling on edge all the time,” and having flashbacks of his time in the war. He is evaluated and given a diagnosis of posttraumatic stress disorder (PTSD).
He has no history of substance dependence and no significant medical history. Which recommendation is
most appropriate for this patient at this time?
