🧠 CNS CLINICAL CASE 2 | Multiple Sclerosis ━━━━━━━━━━━━━━━━━━
🩺 Case Presentation
A 27-year-old woman presents with blurred vision in her right eye, accompanied by weakness and numbness in her left leg. Six months earlier, she experienced a similar episode of visual disturbance that gradually resolved. Neurological examination reveals mild spasticity and reduced sensation in the left lower limb.
Investigations:
- Brain MRI: Multiple periventricular white matter lesions. - CSF analysis: Positive oligoclonal IgG bands. ━━━━━━━━━━━━━━━━━━
Clinical Challenge ❔
1. What is the most likely diagnosis? 2. What is the underlying pathological mechanism? 3. What are the characteristic morphological changes? ━━━━━━━━━━━━━━━━━━
✅ Diagnosis
Multiple Sclerosis (MS) An autoimmune demyelinating disease of the CNS characterized by recurrent neurological deficits involving different anatomical locations.
🔬 Clinical Reasoning
The combination of recurrent neurological episodes, visual disturbances, motor and sensory deficits, and multifocal white matter lesions strongly suggests MS. The characteristic pattern is dissemination in time and space. CSF oligoclonal IgG bands provide additional diagnostic support.
🧬 Pathogenesis
MS results from an autoimmune response against CNS myelin in genetically susceptible individuals. - Th1 cells: Activate macrophages through IFN-γ. - Th17 cells: Promote inflammatory cell recruitment. - B cells: Contribute to the immune response. These mechanisms lead to inflammation, destruction of myelin, and impaired nerve conduction.
🔬 Morphological Changes
Gross: Multiple, well-defined gray-tan demyelinating plaques, particularly in the periventricular white matter. Microscopic: - Loss of myelin with relative axonal preservation. - Perivascular lymphocytic infiltration. - Macrophages containing myelin debris. - Gliosis in chronic inactive plaques.
🚨 Clinical Red Flag
Recurrent neurological deficits affecting different CNS regions, particularly when associated with optic neuritis, should raise suspicion of MS.
💡 High-Yield Pearl
Multiple Sclerosis = Autoimmune CNS Demyelination The hallmark is the presence of multifocal demyelinating plaques with neurological manifestations separated in time and space. ━━━━━━━━━━━━━━━━━━ 🧠 PubMed | CNS Clinical Cases 🔗 t.me/Pub_Med1

🧠 CNS CLINICAL CASE | 01 ━━━━━━━━━━━━━━━━━━ 🩺 Case Presentation A 42-year-old man is admitted following a severe head injury. Over the next several hours, he develops worsening headache, repeated vomiting, and progressive deterioration in consciousness. Brain CT reveals diffuse cerebral swelling, narrowing of the cerebral sulci, and compression of the ventricles. ❔ Clinical Challenge What is the most likely diagnosis, and what pathological mechanisms explain this condition? ━━━━━━━━━━━━━━━━━━ 🎯 Diagnosis Cerebral Edema Abnormal accumulation of excess fluid within the brain parenchyma. 💡 Clinical Reasoning The progressive neurological deterioration, together with diffuse brain swelling, narrowed sulci, and compressed ventricles, strongly suggests cerebral edema. Because the skull is rigid, increasing brain volume can cause a dangerous rise in intracranial pressure (ICP). 🧬 Pathogenesis Cerebral edema develops through two major mechanisms: 1️⃣ Vasogenic Edema - Disruption of the blood–brain barrier (BBB). - Increased vascular permeability. - Accumulation of fluid in the extracellular space. 2️⃣ Cytotoxic Edema - Injury to neurons and glial cells. - Disturbance of normal cellular ionic gradients. - Intracellular fluid accumulation and cellular swelling. Both mechanisms may coexist following severe brain injury. 🔬 Morphological Changes Generalized cerebral edema produces: - Flattened cerebral gyri. - Narrowed cerebral sulci. - Compressed ventricular cavities. ⚠️ Clinical Red Flag Severe cerebral edema may progress to brain herniation, causing compression of vital brain structures and potentially fatal neurological deterioration. 💎 High-Yield Pearl Vasogenic edema → Extracellular fluid accumulation (BBB disruption). Cytotoxic edema → Intracellular fluid accumulation (cellular injury).

✅Tourette Syndrome (TS) 🧠 Tourette syndrome is a neurodevelopmental disorder characterized by sudden, repetitive, involuntary movements and sounds known as tics. It typically begins in childhood between ages 5 and 10 and is three to four times more common in males. ✅Types of Tics - Motor Tics: Sudden movements such as eye blinking, facial grimacing, shoulder shrugging, or head jerking. - Vocal Tics: Sounds or words like throat clearing, sniffing, grunting, or rarely, involuntary swearing (coprolalia, seen in only 10–15% of patients). ✅Key Clinical Features: - Premonitory Urge: An uncomfortable physical sensation or tension that precedes the tic and is relieved once the tic is performed. - Triggers: Symptoms typically worsen with stress, anxiety, or fatigue, and improve during sleep or focused activities. Diagnostic Criteria (DSM-5) - Combined Tics: Presence of both multiple motor tics and at least one vocal tic (not necessarily at the same time). - Duration: Tics must persist for more than 1 year. - Onset: Symptoms must appear before 18 years of age. Pathophysiology & Comorbidities - Mechanism: Linked to basal ganglia dysfunction and hyperactivity of the neurotransmitter dopamine. - Associated Conditions: Strongly associated with ADHD (50–60%) and OCD (30–50%). ✅Management: - Behavioral Therapy: Comprehensive Behavioral Intervention for Tics (CBIT) is the first-line treatment. - First-Line Medications: Alpha-2 adrenergic agonists (Clonidine or Guanfacine), especially useful when ADHD is present. - Severe Cases: Dopamine antagonists/modulators (Aripiprazole, Risperidone, or Haloperidol) and VMAT2 inhibitors (Tetrabenazine). #CNS
