Ans: B. Micturating cystourethrogram (MCU)
Posterior urethral valves are the commonest cause of bladder outlet obstruction in male infants. MCU (VCUG) is the investigation of choice. It shows a dilated, elongated posterior urethra with an abrupt calibre change at the valve, and a trabeculated bladder, often with VUR. Antenatal USG gives the 'keyhole' sign; treatment is endoscopic valve ablation.
Ans: B. Westermark sign
Westermark sign is focal oligaemia (hyperlucency) distal to an occluding pulmonary embolus. Hampton's hump is a peripheral wedge-shaped opacity from pulmonary infarction, and Fleischner sign is an enlarged central pulmonary artery. Most chest X-rays in PE are normal or non-specific; CT pulmonary angiography is the investigation of choice.
Ans: B. Nephrogenic systemic fibrosis
Gadolinium in patients with eGFR < 30 mL/min/1.73 m² (or on dialysis, or with AKI) can cause nephrogenic systemic fibrosis — skin thickening and fibrosis of internal organs. Risk is highest with linear (group I) agents; macrocyclic agents at the lowest dose are preferred if MRI contrast is essential. Contrast-induced nephropathy and iodine-induced thyrotoxicosis are linked to iodinated CT contrast; lactic acidosis relates to metformin with iodinated contrast.
Ans: A. Brodie's abscess
Brodie's abscess is a subacute pyogenic osteomyelitis (usually Staph. aureus) seen as a well-defined metaphyseal lytic lesion with a thick sclerotic rim, often in the tibia, in a child without systemic features. A serpiginous tract towards the growth plate ('penumbra sign' on MRI) supports it. Chondroblastoma is epiphyseal, an ABC is expansile with fluid–fluid levels, and a simple bone cyst is central with a 'fallen fragment' sign. Treatment is curettage and antibiotics.
Ans: B. Battered baby syndrome (non-accidental injury)
Classic metaphyseal lesions (corner/bucket-handle fractures) in a non-ambulatory infant are highly specific for non-accidental injury. Other high-specificity fractures are posterior rib, scapular, sternal and spinous-process fractures, especially when of different ages. A single fall cannot explain multiple metaphyseal fractures, and birth trauma usually involves the clavicle or humerus. Skeletal survey is the initial imaging of choice in suspected child abuse under 2 years.
