A 27-year-old man is brought to the operating room after a gunshot wound to the right upper quadrant. At laparotomy, a deep liver laceration involving segments VII and VIII is identified with massive hemorrhage. Perihepatic packing is performed, but significant bleeding persists. The surgeon places a vascular clamp across the structures contained within the free edge of the lesser omentum. Despite complete occlusion, brisk hemorrhage continues and The patient remains hypotensive despite ongoing massive transfusion. What is the next best step in management? A. Extend the duration of inflow occlusion and reassess B. Perform selective ligation of the right hepatic artery C. Proceed with right hepatectomy D. Obtain suprahepatic and infrahepatic caval control to achieve total hepatic vascular isolation E. Transfer the patient for hepatic angiography and embolization
