How does smoking set the stage for lung cancer—even before cancer-causing mutations appear? For decades, scientists have known that smoking increases the risk of lung cancer. But a fundamental question remained: How does cigarette smoke prepare otherwise healthy lung cells to become cancerous? A new study published in the Proceedings of the National Academy of Sciences (PNAS) offers an important clue. Using laboratory-grown miniature lungs, or organoids, researchers found that prolonged exposure to cigarette smoke can alter lung stem cells, weaken their natural safety mechanisms and disrupt local immune defences. Interestingly, smoke exposure alone did not produce tumours in the experimental model. Neither did introducing certain cancer-related mutations into healthy tissue. Tumours developed only when those mutations were introduced into tissue already altered by chronic smoke exposure.
