Quick note about lasix ..

سؤال هوايه شفنا وهو ياترى يصير انطي ادويه الـ Nebulizer مثل الـ Pulmocort وغيرها بالجهاز مال اوكسجين ؟! خلي نجي نفسرها علميا ونشوف وبالاخير نجاوب عل سؤال حسب الرفرنس .. ⚫️ الـ drugs الي تكون على شكل Nebulizer تحتاج pressure & force معينه حتى تتحول الى droplets وتنتشر للـ Lungs وهاي معيره حسب جهاز الـ Nebulizer ⬇️⬇️ - Nebulizer therapy requires a source of pressurized gas, such as compressed air or oxygen, delivered through a specialized nebulizer device Oxygen delivery systems (such as concentrators or cylinders) are not substitutes for nebulizer compressors unless specifically equipped with a nebulizer function .. ⚫️ جهاز الاوكسجين ( العادي ) والي اعتقد كل مستشفياتنا منه هو ممكن يحول الـ drugs الى droplets بس الفكره انو هاي العمليه مراح تكون بالكفاءه المطلوب مقارنة بجهاز الـ Nebulizer 🔴 ناهيك عن كل هذا اكو شي مهم ومذكور بالـ guideline وعليه trials صارت كذلك والي هو ↙️↙️ - air-driven bronchodilator nebulization is preferable to oxygen-driven in acute exacerbations of COPD in order to avoid the potential risk of increasing the PaCO2 associated with oxygen-driven bronchodilator administration .. وايضا ↙️↙️ - Oxygen-driven nebulisation leads to an increase in PtCO2 in exacerbations of COPD We propose that air-driven bronchodilator nebulisation is preferable to oxygen-driven nebulisation in exacerbations of COPD .. طبعا اكو بعض أجهزة الأوكسجين يكون بيها نيبيولايزر بورت أو فتحة مخصصة، وتلكاها مكتوب عليها Nebulizer Port or Nebulizer Outlet الجهاز اللي يدعم نيبيولايزر عادة يكون مكتوب عليه بالمواصفات أنه: (Compressed Air for Nebulization) .. وماعتقد عدنا بالعراق .. #Mohammed_Khader

نسولف اليوم عن الـ Colistin هو من عائله الـ Polymyxins نستخدمه mainly for Resistance gram -ve infections بحيث الـ Spectrum مالته يصير Good: Many GNRs, including multidrug-resistant A baumannii, P aeruginosa, and K pneumoniae Moderate: Stenotrophomonas maltophilia Poor: All gram-positive organisms, anaerobes, Proteus, Providencia, Burkholderia, Serratia, and gram-negative cocci .. ملاحظات مهمه - دائما دائما نخلي اخر لاين للتريتمنت والاهم من هذا كله ممنوع منعا بااتا ينكتب وحده بالطبله لازم وضرووري يكون with combination with other antibiotic - last-line antibiotics, they are frequently used in combination with other drugs . - رهيب من تكون الـ infection بالـ urinary tract infection لان ال excretion is renally - ضعيف للاسف بالـ Tissues infection لان poorly Penetration in the tissue لان بال Lung infection ممكن يستخدموا like in nebuliser .. بالنسبه لل Carbapenem-resistant Enterobacterales فللأسف ميستخدم عدا ال uncomplicated cystitis يعتبر alternative agent .. - colistin are not suggested for the treatment of infections caused by CRE - هوايه كالوا ويكولون ممكن نستخدمه as nebuliser اخر كايدلاين IDSA والي نزل 11/2024 يكول suggests against the use of nebulized antibiotics as adjunctive therapy for DTR P. aeruginosa pneumonia due to the lack of benefit observed in clinical trials, concerns regarding unequal distribution in infected lungs, and concerns for respiratory complications such as bronchoconstriction with use of aerosolized antibiotics .. وبالاخير ↙️↙️ ، Finally in the largest clinical trials (over 300 patients in each trial) evaluating the role of colistin monotherapy, mortality was relatively high at 46% in both trials .. #Antibiotics #Mohammed_Khader

Interesting position .. 🟣 الصوره بموافقه الاهل اني ودامشي بردهه الهيماتو بالاخص ال Leukemia  لكيت طفل يصيح وميقبل ينام ويكعد  الا هيج والدته تكول ميرتاح الا هيج وبس ينام عدل ( بالاخص على ظهره الالم يزيد )   .. فالوضعيه شويه interesting  🟣 This is a common posture seen in patients with ( acute pancreatitis )  especially in children. 🟣 Why this position? - Patients often assume a forward-leaning position to relieve the pain, as it reduces tension on the inflamed pancreas and minimizes peritoneal irritation .. - Lying flat often worsens the pain, while this position reduces tension on surrounding structures and provides temporary relief. 🔴  When a patient reports that "the pain improves when I lean forward" it’s a classic clue that supports the diagnosis of pancreatitis .. 🔴 فعلا الطفل طلع عنده Pancreatitis بسبب الادويه الي دياخذها والحمدلله وضعيته صايره احسن ..

Facing this is a part of our daily routine almost every time ! .. وصل بينا الحال انو صرنا مننصدم اذا نشوف Resistance to all بس صرنا نفرح اذا نشوف Sensitive الوضع منهار متكدر تسيطر عليه من خلال بوست على وسائل التواصل الاجتماعي لو توصيات خاصه او محاذير الوضع يحتاج اعاده تأهيل وتنظيم وسيطره من ال A to Z على جميع الفئات الطبية والا حيبقى ويزداد بالانهيار يوما بعد الاخر ..

عيد سعيد وكل عام وانتم بخير اليوم خلي نرجع للحرب العالمية ونسولفكم قصه بيها معلومة لطيفه ⚫ تعرفون بكل حرب لازم يصير اسرى عند الاطراف المتخاصمة فبالحرب العالميه الثانية WWII الي استمرت تقريبا حوالي 6 سنوات كان هناك اسرى عند بعض القوات المتنازعه .. - استمر اسر هؤلاء الجنود لمده طويله من الزمن ، فاثناء انتهاء االحرب وتحريرهم ، شافوا انو هؤلاء الاسرى حالتهم جدا ضعيفه و كلش تعبانين وجوعانين وعطشانين ومنتهين بشكل كلي .. ⚫ فهنا بالحاله الطبيعيه راح يقدمولهم كلشي (( من طعام وشراب وما تجود به انفسهم )) .. ولكن للأسف حدثت مصيبه لم تكن بالحسبان 😣 المصيبه الي صارت هي وره فتره من بدوا ياكلون ويشربون صار عند هذوله الاسرى اعراض وحالات لم تكن على البال ابدا !! صار عدهم الاتي ↙️↙️ hypertension , cardiac insufficiency , seizures , coma , and death !!! .. هنا العالم ظنوا انو هذوله الجنود ماتوا من وره التعذيب او النزف الداخلي والخ من ال DDX ومخطر على بالهم غير شي .. ولكن سووا كلشي documented و وثقوا للمستقبل .. تكررت هاي الحاله في عام 1970 وعام 1980 وهنا بدأ العلم يتطور والدراسات تبحث عن السبب ؟؟؟! شنو السبب الي يخلي اي شخص يكون malnutrition من يبدأ ياكل ويشرب ينتهي بمضاعفات ممكن تؤدي الى فقدانه حياته ؟!؟! 🧐 🟤 وهنا تم اكتشاف الـ Refeeding Syndrom .. لاحظوا الـ malnutrition pt يصير عدهم electrolyte disturbance وبالاخص بالـ (( phosphorus , potassium, magesium )) فمن تدخل Calories للجسم راح يصير عدنا shifting لهاي الالكترولايت من extracellular space الى into the cell وبالتالي راح تقل بشكل كبير بالـ circulation .. وهذا راح يدخلني بالـ complications مال هاي الالكترولايت وبالاخص ↙️↙️ 🔴 Decrease Potassium ➡️ AF 🔴 Decrease Magnesium ➡️ convulsions , Muscle cramps , acute organic brain syndrome 🔴 Decrease phosphorus ➡️ irritability , confusion .. وبالتالي كل هاي المسببات واخرى راح تزيد حاله المريض سوء وكأنما احنا كاعدين نطفي التماس الكهربائي بالمي !! .. ⚫ لهذا دائما احنا نجيك الـ electrolyte disturbance قبل لا نبلش بالـ TPN لاي مريض بالاخص الي صارله ايام راقد بدون اي تغذيه خوفا من الـ Refeeding Syndrom - وكذلك ننصح زملائنا الدكاتره والصيادلة ابقيه الطوابق انو قبل لا تبلش TPN للمريض دز على الـ Electrolyte وجيكها مخسران شي لان ممكن تشوفها بالـ lower border وتغض البصر عليها وبالتالي تبدي كلش تنزل من تبلش TPN وتبدي المشاكل تظهر .. وهمين من تبلش TPN دز routinely على الـ Electrolyte افضل حتى تشوف شديصير بجسم المريض وشلون ديستجيب .. حتى نتجنب مشكله احنا في غنى عنها .. #Mohammed_Khader

#كيسات_الردهة ⚫️ الـ chemotherapy بي انواع خير من الله كل نوع لازم نعرف شلون نسويله monitoring وشنو يسوي adverse effects وشلون نتابعها وشلون نسويلها management ⚫️وحده من كيسات الردهه هي مريضه تشخصت على انها Molar pregnancy فحسبنا السكور مالتها فصار اللاين مال تريتمنت هو Methotrexate ⚫️ الـ Methotrexate يسويلنا هوايه مشاكل و وحده من المشاكل هي Mucositis فلازم دائما نتابع البيشنت لا يصير عدها هل شي لان ممكن نوصل انو نوقف ال Methotrexate عن البيشنت وهاي تصير بال Grade 4 Mucositis فأثناء متابعتنا لكيت البيشنت بده يصير عدها sloughing and mouth ulcer in the buccal .. وبدت تشتكي متكدر تاكل بس تشرب 🔴 فأهنا واحد من اللاين مال تريتمنت هو Magic Solution هو خبطه من عده مواد نحضرها بتراكيز معينة و نخلي البيشنت يتغرر بيها ( وفي بعض الحالات يتم بلعها حسب الـ Severity ) ونتابع وهكذا الحمدلله البيشنت كتبتلها Magic Solution مع المتابعه والاجراءات الاخرى تماثلت بالشفاء التام ( الصور مرفقة ) #Mohammed_Khader

مازال الاختلاف قائم على جماعه الـ acute Pancreatitis  انو نبلش وياهم feeding  لو نبقيم فاستنك ؟! وليش الـ new recommendation  صارت تمشي ويه الـ feeding  عكس ماكان سابقا ! 🤔 ⚫️ اول شي سابقا كان مريض الـ Pancreatitis نبقي فاستنك ومنبلش feeding  لحد ما جتي اخر الـ recommendation انو نبلش راسا feeding  طول ما البيشنت can tolerate  .. والسبب ؟ ↙️↙️ ⚫️ Prevention of Intestinal Barrier Dysfunction & Bacterial Translocation - Fasting leads to mucosal atrophy and increased intestinal permeability, allowing bacteria to translocate into the bloodstream, increasing the risk of infections and sepsis. So, Early enteral nutrition (EN) helps maintain gut integrity and reduces these complications. ⚫️ Better Clinical Outcomes & Reduced Need for Parenteral Nutrition ( shorter hospital stay ) - Early feeding decreases the need for total parenteral nutrition (TPN), which is associated with infection risks and metabolic complications (e.g., hyperglycemia). So, Studies show that patients receiving early oral or enteral feeding have shorter hospital stays and improved recovery rates. ⚫️ Reduction of Systemic Inflammatory Response Syndrome (SIRS) - Early enteral nutrition helps modulate the inflammatory response, reducing the risk of multi-organ failure. ⚫️ Minimal Stimulation of the Pancreas - Contrary to old beliefs, research indicates that enteral feeding does not significantly stimulate pancreatic enzyme secretion in a way that worsens inflammation, particularly when using nasojejunal or nasoduodenal feeding tubes. 🔴 Practical Recommendations - Oral feeding: Initiate as soon as tolerated by the patient. - Enteral feeding: If oral intake is not feasible, nasoenteric feeding (preferably nasojejunal) should be considered over parenteral nutrition. 🟣 These recommendations are supported by guidelines, including (  AGA, ASPEN, and IAP/APA ) #Mohammed_Khader
