اليوم نسولف على الـ Anticoagulant بالـ pregnancy شوكت وشكد الدوز و المده الي نستمر عليها .. الي راح اسولفه من اخر كايدلاين وابديت فيما يخص هل موضوع .. ⚫️ اول شي اكو Risk factors هاي كل نوع منها يمثل نقطه او 3 نقاط .. وعلى حسب الـ Score حنعرف شوكت نبلش الـ Anticoagulant وشكد نستمر عليه .. هسه نجي علـ Score الي تكون على حسب الـ Risk factors خليتها ( اول كومنت ) الي جمعناها هسه كل نقاط تعني شي والي هي ↙️↙️ .. - 2 point ➡️ Clexane 10 days postpartum - 3 point ➡️ Clexane from 28week of pregnancy and. continue 6 week postpartum. - 4 or more ➡️ Clexane throughout antenatal period and continue 6 week postpartum .. 🔴اكو حالات مؤقته مثل الـ Hyperemesis gravidarum هاي ننطي بيها الـ Anticoagulant خلال هل فتره وراها من يصير الـ remission نقطعها ونرجع نمشي الـ Risk factors مال كل بيشنت و الي سولفتها فوك .. 🟣 بالنسبه للـ Dose مالـ Clexane فهي كالاتي ↙️↙️ - < 50 kg ➡️ 2000 Clexane daily - 50–90 kg 4000 Clexane daily - 91–130 kg 6000 Clexane daily - 131–170 kg 8000 Clexane daily - > 170 kg 0.6 mg/kg/day #Mohammed_Khader

بسبب الازمه بين ايران واسرائيل هوايه بوستات نزلت عن ( اليود - Potassium Iodide ) تعال اخذ يود واحمي نفسك وشكلها حتصير بدعه مثل كوفيد بحيث ماظلت صفحه ولا كروب WhatsApp مانشره والناس بدت تتوافد عليه بالصيدليات ويريدون ياخذوا !! حسب ماسمعت صار تريند عل تيكتوك هاي اذا ما الريلتف مالتكم وصللهم الخبر وسألوكم عليه 😂 ⚫️ ياترى الامر بهل سهوله !!!؟ خل نسولف بي ساينتفكلي .. ⚫️ اول شي مثل منعرف ايران تمتلك مفاعلات نوويه وفي حال تم قصفها وعدم احتواء الاشعاع من قبل منشأتهم النووية راح يتسرب وحسب الرياح واتجاهها حيتقرر حتجي هل اشعاعات النا ام لا .. ☣️ الاشعاعات المنبعثه انواع واهمها ↙️↙️ Iodine-131 Cesium-137 Strontium-90 Plutonium / Uranium الي نكدر نحمي نفسنا منه هو اشعاع iodine - 131 طيب شنو الفكره من استخدام ال Potassium iodide ؟! ⚫️ مثل منعرف الـ Thyroid Gland هي الي تمتص الـ Iodine لهذا الفكره بدال ما تمتص يود مشع وتسويلنا Cancer cell ليش منخليها تمتص يود سليم ( غير مشع ) !! ونتجنب الضرر الاكبر مال Iodine-131 🟣وهذا الي احنا حنسوي طيب شلون ناخذه وشوكت ؟! الـ iodine 131 تقريبا الـ half life مالته يوصل 8 ايام وبعدها يصير بي disassociation - احنا ناخذ الـ Potassium iodide قبل التعرض للأشعاع ( 48 - 1 ساعة ) بعده التعرض للأشعاع بساعه او ساعتين كحد اقصى ( اكيد مراح نعرف انو الاشعاع وصلنا الا عن طريق الحكومه سواء بأعلان رسمي من قبل الوزاره او بغير طرق ) 🔴 بالنسبه للدوزات و شكد نستمر عليه ؟! الدوزات ومايخصها الديتيلز ( نزلتها بالقناه ) بس بشكل عام ممكن ناخذ دوز وحده او اثنين لان شده الخطر هي اول ( 48 - 24 ساعه ) و في حال اعلان الحكومه بأستمرار التعرض ممكن نستمر عليه 10 ايام حسب قرار الحكومه بفحص الاشعاع ومدى استمراريتة بالجو .. اخذه بدون سبب يسوي ضرر عل Thyroid Gland ويدخلها بـ Suppression لهذا بدايه البوست كلت ( دفع ضرر اكبر بضرر اقل ) لهذا نخلي للاوقات الحرجه وناخذه بطريقه صحيحة بأقل فتره ممكنه 🟤 وبالاخير الوقايه بهيج ظروف اهم من كلشي من ناحية الملابس الكمامة والتعقيم والغسل والخ لان الـ Potassium iodide يحميك من نوع واحد من الاشعاع والبقيه لا ، لهذا الحذر مطلوب .. نوافيكم بمعلومات صحية في حال استمرار ذلك لا سامح الله الله يحفظ العراق واهله إن شاء الله .. #Mohammed_Khader

من نريد نجيك المريض اذا عنده allergy للـ iron او لا مو نجي نسويله skin test !!! اكيد حيطلع بوزتف هاي اذا مصار بي Skin necrosis !! بدال منسوي Skin test , ننطي انفيوشن بطيىء ونراقبه وهاهيه .. الـ test dose مال iron كالاتي ↙️↙️ - administration of a 25 -mg test dose followed by a 1- hour observation period before the total dose of iron is infused .. - طبعا نسبه الـ anaphylaxis تفرق بأختلاف نوع الـ Iron بحيث اكو انواع متحتاج test dose .. مع الاسف احنا بالـ 2025 ولازلنا نشوف هيج شي .. 🔴 نفس الحال للـ Vancomycin ممنوع منعا باتا نسويله Skin test لان 100% حيطلع بوزتف .. Onset of Symptoms: Delayed: usually 24 to 72 hours post-ingestion (due to existing clotting factor stores). 4. Duration of Anticoagulant Effect: Can last for weeks to months without treatment.

سامعين بالـ Super Warfarin ؟! وشيصير اذا استخدمناه عل بشر ؟! اليوم خل نسولف عليه شويه .. ⚫️ اليوم عدنا الـ Bromadiolone يعتبر Super Warfarin بسبب الـ Potency الي يملكها .. وايضا نعتبره من الـ ↙️ Long-acting anticoagulant rodenticide (LAAR). ⚫️ من تروح تشتري ( سم فئران ) حينطيك المعشب او اي محل منتج بالعاده يحتوي على الـ Bromadiolone او اخوانه يستخدم لقتل الفئران والجرذ .. ⚫️ اجتنا كيس فيميل للطوارىء بالعه منه للأسف عبالها ( جلي محلاة !! ) بسبب طعمه ولونه المميز !!! ( وهذا خطأ من شركه التصنيع للاسف ) .. خلي نسولفلكم عل كيس شويه .. 🟤 Clinical Presentation: Young female with no past medical history, presented with bleeding from the mouth for one day. Recently married and previously in good health. One day prior to her ER visit, she developed bleeding from the mouth and nose, with no history of trauma. She noticed multiple red spots (non-painful, non-itchy, varying in size) on both upper and lower limbs, unrelated to trauma. She also experienced hematuria (red urine with clots) and dysuria. Additionally, she had vaginal bleeding with clots. She reported nausea over the previous two days, but no vomiting or hematemesis. She experienced mild abdominal pain, primarily in the flanks, radiating to the back. No change in bowel habits, no change in stool color. No jaundice, no abdominal distension. 🟤 History: No previous bleeding tendencies. No contact with sick or febrile individuals. Past medical and surgical history: negative. No psychiatric history. No history of drug use, including illicit substances. No family history of similar illness, hematologic disorders, or malignancy. 🔴 Important note: She had ingested rat poison 3 days prior to ER admission, mistaking it for juice !!! 🟤 Examination Findings in ER: Distressed but conscious and oriented . Dried blood present around the mouth and nose. Multiple ecchymotic spots on both upper and lower limbs. Mild conjunctival pallor, no jaundice. No lymphadenopathy. Wet purpura noted on oral mucosa. Soft abdomen, no palpable organomegaly. Cardiopulmonary examination: normal 🟤 Lab test : Everything is normal except bleeding profile INR : 22 aPTT : 101.7s 🟣 management : We deal with this case like dealing with Warfarin toxicity with close monitoring #Mohammed_Khader

سؤال هوايه شفنا وهو ياترى يصير انطي ادويه الـ Nebulizer مثل الـ Pulmocort وغيرها بالجهاز مال اوكسجين ؟! خلي نجي نفسرها علميا ونشوف وبالاخير نجاوب عل سؤال حسب الرفرنس .. ⚫️ الـ drugs الي تكون على شكل Nebulizer تحتاج pressure & force معينه حتى تتحول الى droplets وتنتشر للـ Lungs وهاي معيره حسب جهاز الـ Nebulizer ⬇️⬇️ - Nebulizer therapy requires a source of pressurized gas, such as compressed air or oxygen, delivered through a specialized nebulizer device Oxygen delivery systems (such as concentrators or cylinders) are not substitutes for nebulizer compressors unless specifically equipped with a nebulizer function .. ⚫️ جهاز الاوكسجين ( العادي ) والي اعتقد كل مستشفياتنا منه هو ممكن يحول الـ drugs الى droplets بس الفكره انو هاي العمليه مراح تكون بالكفاءه المطلوب مقارنة بجهاز الـ Nebulizer 🔴 ناهيك عن كل هذا اكو شي مهم ومذكور بالـ guideline وعليه trials صارت كذلك والي هو ↙️↙️ - air-driven bronchodilator nebulization is preferable to oxygen-driven in acute exacerbations of COPD in order to avoid the potential risk of increasing the PaCO2 associated with oxygen-driven bronchodilator administration .. وايضا ↙️↙️ - Oxygen-driven nebulisation leads to an increase in PtCO2 in exacerbations of COPD We propose that air-driven bronchodilator nebulisation is preferable to oxygen-driven nebulisation in exacerbations of COPD .. طبعا اكو بعض أجهزة الأوكسجين يكون بيها نيبيولايزر بورت أو فتحة مخصصة، وتلكاها مكتوب عليها Nebulizer Port or Nebulizer Outlet الجهاز اللي يدعم نيبيولايزر عادة يكون مكتوب عليه بالمواصفات أنه: (Compressed Air for Nebulization) .. وماعتقد عدنا بالعراق .. #Mohammed_Khader

نسولف اليوم عن الـ Colistin هو من عائله الـ Polymyxins نستخدمه mainly for Resistance gram -ve infections بحيث الـ Spectrum مالته يصير Good: Many GNRs, including multidrug-resistant A baumannii, P aeruginosa, and K pneumoniae Moderate: Stenotrophomonas maltophilia Poor: All gram-positive organisms, anaerobes, Proteus, Providencia, Burkholderia, Serratia, and gram-negative cocci .. ملاحظات مهمه - دائما دائما نخلي اخر لاين للتريتمنت والاهم من هذا كله ممنوع منعا بااتا ينكتب وحده بالطبله لازم وضرووري يكون with combination with other antibiotic - last-line antibiotics, they are frequently used in combination with other drugs . - رهيب من تكون الـ infection بالـ urinary tract infection لان ال excretion is renally - ضعيف للاسف بالـ Tissues infection لان poorly Penetration in the tissue لان بال Lung infection ممكن يستخدموا like in nebuliser .. بالنسبه لل Carbapenem-resistant Enterobacterales فللأسف ميستخدم عدا ال uncomplicated cystitis يعتبر alternative agent .. - colistin are not suggested for the treatment of infections caused by CRE - هوايه كالوا ويكولون ممكن نستخدمه as nebuliser اخر كايدلاين IDSA والي نزل 11/2024 يكول suggests against the use of nebulized antibiotics as adjunctive therapy for DTR P. aeruginosa pneumonia due to the lack of benefit observed in clinical trials, concerns regarding unequal distribution in infected lungs, and concerns for respiratory complications such as bronchoconstriction with use of aerosolized antibiotics .. وبالاخير ↙️↙️ ، Finally in the largest clinical trials (over 300 patients in each trial) evaluating the role of colistin monotherapy, mortality was relatively high at 46% in both trials .. #Antibiotics #Mohammed_Khader
