Tarnow's 5mm Rule
Black triangles are among the most common esthetic concerns.
One of the most important studies on this topic was published by Dennis Tarnow in 1992. The study showed a strong relationship between papilla presence and the distance from the contact point to the interproximal bone crest.
The famous takeaway:
β€5 mm = highly predictable papilla fill.
Once that distance increases, the chance of a visible black triangle rises dramatically.
In many cases, restorative dentistry can help by modifying tooth shape and moving the contact area more apically to support the papilla visually.
Understanding biology is what makes esthetic dentistry predictable.

Lingual Retainers β A Simple Chairside Tip
β I've tried both floss and ligature wire for placing bonded lingual retainers.
β’ Floss: Works well initially, but if the procedure takes longer, it tends to loosen.
β’ Ligature wire: Secure, but I find it more time-consuming.
β Recently, I started using two 3/16-inch, 3.5 oz intraoral elastics positioned at the level of the lateral incisors to hold the retainer wire in place. It has worked extremely well, keeping the wire stable while freeing up both hands.
β This simple modification reduced my average bonding time from around 10 minutes to just 3β4 minutes, including etching.
β The choice of adhesive also makes a noticeable difference. My preference is:

Hey everyone,
Just wanted to share a positive follow-up from a tooth autotransplantation case I've been monitoring. Today, I had a 6-month recall appointment with a patient who underwent this procedure. This was only the second autotransplantation I've ever performed, so I was particularly invested in this case.
The patient is a 15.8 year old male who was referred for root canal re-treatment on his lower right first molar. Honestly, I wasn't entirely on board with the initial treatment plan and felt the tooth was questionable to hopeless. Instead, I saw a good opportunity for a tooth autotransplantation, especially since his lower right third molar was only a soft-tissue impaction and a viable donor.
Fast forward six months, and the follow-up is really encouraging! Radiographically, we're seeing significant thickening of the transplanted tooth's root and even a slight increase in its length. This strongly suggests continued vitality and successful integration.
I'm genuinely excited to see how this progresses over the next 2-3 years as the root fully forms. It's moments like these that make the work so rewarding!

Hi everyone, I'm excited to share the results of the 9-month recall, which I conducted today, for my very first case involving autotooth transplantation, on a 15-year-old female patient.
Autotransplanted Tooth #48 (Replacing Severely Decayed #47)
Tooth #48, was autotransplanted to the site of tooth #47. Tooth #47 was extracted due to severe decay during the same visit as the transplantation. After the extraction, the recipient site was prepared with some bone guttering. This was my first time performing an autotransplantation procedure.
Fixation was achieved using sutures only.
There is some tilted angulation of #48 post-transplantation, likely due to insufficient bone guttering on the mesial site.
At today's 9-month recall, the recipient site has fully healed clinically and radiographically. However, there is no visible radiographic evidence of root formation on the transplanted tooth #48.
Clinically, tooth #48 is currently out of occlusion (under-occlusion). Percussion test was within normal limits (eliciting a normal sound and response), and the cold test was also within normal limits (producing a transient, normal sensation).
