On neurologic examination, you note a snarling appearance when the patient is asked to smile, and a nasal tone is heard in her voice. Jaundiced patient presented to you with behavioral changes 👉🏽👉🏽 you must think about these ddx : 1) hepatic encephalopathy: there is astrexisis (flapping tremer), very characteristic. 2) Wernke-Korsakoff syndrome : occur in alcoholic persons due to thiamine deficiency . Any alcoholic person presented to you with neuronal abnormality👉🏽👉🏽 give Thiamine. DO NOT GIVE GLUCOSE BEFORE THIAMINE . 3) Wilson disease : there is KF ring (may be non-observable clinically and need slit lamp) . 4) subdural haemorrhage SDH : sudden in onset if it was acute SDH and there is headache . هاي اهم اربع نقاط لازم تخليهن ع بالك ، و كلهن موجودات بالكتاب الي راح انزله الكم من اكمل من عنده ان شاء الله .

Kidney function test in context of liver failure : ١- اكيد تعرفون ان اليوريا تتصنع بالكبد، فلو واحد عند liver failure و نريد نقيس ال kidney function , ف urea level ما راح يعكس ال kidney function لان اصلا ناكو تصنيع لليوريا ٢- الكرياتينين، بعض الكرياتيتين يتصنع بالكبد و بعض منه بال muscles ، و جماعة ال liver failure عدهم muscle wasting , ف بالتالي creatinine production راح يكون قليل، فبالتالي ما يعكس ال kidney function . اتمنى هالنقطتين صارن واضحات عدكم الي بعده ما فاهمهن، يراسل ع البوت اعلاه .

خلي هالملاحظه ب بالك The first neurons that are affected by diabetes is neurons that responsible for vibratory sense . طبعاً هالملاحظه، اقل ما يقال عنها انها عظيمه . لو فرضاً اجاك شخص عمره 51 سنه ، و عنده Lower limb tingling due to peripheral neuritis فجداً وارد انه انت راح بال diabetic neuropathyك اول ddx بقائمتك . حتى تتأكد افحص ال vibration : 1) if it was intact, so your suspicion is wrong . و ممكن يكون سببه هو peripheral neuritis due to B12 deficiency بسبب المتفورمين (ع سبيل المثال) 2) if it was lost 👉🏽👉🏽 so your suspicion is highly likely .

It is a case of right pneumothorax + left cavitary lesion (just focus, and you will see it) . So it is a secondary pneumothorax . The cavitary lesion may be : TB, lung abscess, or lung CA . TENSION PNEUMOTHORAX IS CLINICAL TERM NOT RADIOLOGICAL TERM . IT IS MEAN PNEUMOTHORAX + SCHOCK .

خلي ببالك نقطة مهمه ، و لا تنساها ابد : Non- cirrhotic ascites in females is due to ovarian carcinoma till proven otherwise . فيميل و عمرها 50s و اجتك ascites و ال LFT طبيعي، خلص لازم تفكر بال ovarian cancer حتى لو هي قبل عدها رسك فاكتر، يعني حتى لو عدها مثلا هارت فيلر بس ما عدها cardiac cirrhosis، فتبقى الاولوية لل ovarian CA .

Thanks for all answers . It is a case of tinea versicolor or pityriasis versicolor . Rx : clotrimazole cream + selsum shamboo .
