| 2 | Helicobacter pylori (H. pylori) is a bacterium that infects the stomach lining. It can cause chronic gastritis, peptic ulcers, and over many years can increase the risk of certain stomach cancers.
🔹 How infection occurs
⠀
➟ Usually spreads through oral–oral or faecal–oral contact
➟ Contaminated food or water may contribute
➟ Infection is often acquired during childhood
➟ Household transmission can occur
➟ Many infected people remain asymptomatic for years
⠀
🔹 Common signs
⠀
➟ Burning or aching pain in the upper abdomen
➟ Bloating or early fullness
➟ Nausea
➟ Frequent belching
➟ Reduced appetite
➟ Symptoms may worsen when the stomach is empty
➟ Some people have no symptoms at all
⠀
🔹 Signs of ulcer disease
⠀
➟ Recurrent upper abdominal pain
➟ Pain related to meals
➟ Nausea or vomiting
➟ Black, tarry stools from gastrointestinal bleeding
➟ Vomiting blood in severe cases
⠀
🔹 Diagnosis
⠀
➟ Urea breath test is commonly used for active infection
➟ Stool antigen test can also detect active infection
➟ Endoscopy with biopsy may be needed if alarm symptoms are present
➟ Blood antibody tests are less useful for confirming current active infection because antibodies can remain positive after treatment
⠀
🔹 Important testing note
⠀
➟ PPIs can reduce the accuracy of breath and stool tests
➟ Antibiotics and bismuth can also cause false-negative results
➟ Medication timing before testing should follow clinician or laboratory instructions
⠀
🔹 Core management
⠀
➟ H. pylori should usually be treated with combination therapy
➟ Treatment includes acid suppression plus multiple antibiotics
➟ Bismuth-based quadruple therapy is commonly used in many settings
➟ The exact regimen depends on local antibiotic resistance, previous antibiotic exposure, allergy history, and availability
➟ Complete the full treatment course exactly as prescribed
⠀
🔹 Test of cure
⠀
➟ Eradication should be confirmed after treatment
➟ Urea breath test or stool antigen testing is commonly used
➟ Testing is usually performed several weeks after antibiotics are finished
➟ PPIs generally need to be stopped beforehand for an appropriate interval if medically safe
⠀
🔹 Lifestyle / self-care
⠀
➟ Avoid smoking
➟ Limit alcohol if it worsens symptoms
➟ Avoid frequent NSAID use unless medically necessary
➟ Eat smaller meals if large meals trigger discomfort
➟ Maintain good hand hygiene
➟ Use safe food and drinking water practices
⠀
🔹 What not to do
⠀
➟ Don’t stop antibiotics early when symptoms improve
➟ Don’t treat H. pylori with acid suppressants alone
➟ Don’t repeatedly use antibiotics without confirming an appropriate regimen
➟ Don’t assume a positive blood antibody test means infection is still active
➟ Don’t ignore persistent weight loss, bleeding, or swallowing difficulty
⠀
🔹 Possible complications
⠀
➟ Chronic gastritis
➟ Peptic ulcer disease
➟ Gastrointestinal bleeding
➟ Iron-deficiency anaemia in some people
➟ Gastric MALT lymphoma
➟ Increased risk of gastric adenocarcinoma
⠀
🔹 When to see a doctor
⠀
➟ Persistent upper abdominal pain
➟ Recurrent indigestion despite treatment
➟ Unexplained iron-deficiency anaemia
➟ Recurrent peptic ulcers
➟ Unexplained weight loss or persistent vomiting
⠀
🔹 Emergency / referral warning
⠀
➟ Vomiting blood
➟ Black, tarry stools
➟ Sudden severe abdominal pain
➟ Fainting, marked weakness, or dizziness with suspected bleeding
➟ Persistent vomiting with dehydration
➟ These may indicate ulcer bleeding or perforation and require urgent medical assessment
⠀
🔹 High-Yield Points
⠀
➟ H. pylori is a major cause of chronic gastritis and peptic ulcers
➟ Urea breath test and stool antigen test detect active infection
➟ Treatment requires combination antibiotic therapy plus acid suppression
➟ A test of cure is important after treatment
➟ Untreated infection can increase the risk of gastric cancer and MALT lymphoma

(Blisters).
(Bone, Tendon, Muscle).
(The Golden Prevention Bundle):
(Repositioning):
(Early Mobilization)
(Daily Skin Assessment):
(Fatal Nursing Errors):
(Sacrum).
